Student registration
Once registered, students should provide
by fax (+33 1 44 64 15 16)
or email (eao2012@clq-group.com)
a valid student identifi cation confi rming their
undergraduate student status before October 1 st ,
2012, otherwise the higher registration fee will apply
onsite.
Group registrations
A special rate is available for group registrations
according to the following conditions:
� Minimum of 5 participants
� Deadline for submitting a purchase order: August
31 st , 2012
� Invoice settlement upon receipt
� Deadline for payment: September 16 th , 2012
� Unique rate for all registration types (590 € VAT
included): no reduced fees will be applied
� Your registration interface will be available until
September 30 th , 2012
Children
To provide all EAO attendees an optimal environment,
children under the age of 18 are not permitted into
the scientifi c sessions or the exhibition hall during
show hours, installation or dismantling.
Childcare is not available at the conference and
exhibition center; however your hotel concierge may
be able to assist you.
Payment
Payment must accompany the registration form.
Registrations received without payment will not be
processed nor confi rmed.
Your credit card transaction will be processed through
Colloquium Paris (EAO Organizing Secretariat).
Please note that your monthly credit card statement
will indicate payment to «12EAO - CLQ».
Confi rmation
Receipt of your registration form and credit card
payment will directly be acknowledged by email. If
you have not received a confi rmation, please contact
eao2012@clq-group.com or call +33 1 44 64 15

VAT
Danish VAT is applicable on the registration fees
(25%). Please note that VAT on registration fees
paid by companies and other taxable entities can
only be reclaimed if the customer’s VAT number is
mentioned on the invoice. If applicable, please fi ll
out your company’s VAT number in the section “VAT
number” on the registration form.
Invoice
Only digital version of invoices will be sent to
participants. The invoice will be issued within one
week after the registration and cannot be changed,
so please make sure you have provided us with the
correct information (company name, address and
VAT number if applicable). In case of any enquiries,
please contact eao2012@clq-group.com.
Cancellation policy
Cancellation policy for individual registrations
All cancellations must be made in writing (email or
fax) to:

� Before July 16 th , 2012: Reimbursement of 50%
of the fees paid. With no payment received before
cancellation, this amount will still remain due.
� After July 16 th , 2012: No refund. With no payment
received before that date, the total amount will still
remain due to the Congress offi ce.
All refunds will be issued after the congress.
Requests will not be accepted after the 15 th of
November 2012.
Name change
Registered participant who are unable to attend the
congress will not have the possibility to nominate a
substitute participant. Name changes will constitute
a new registration.
