It must:ooobe signed and datedrefer to the last 12 monthsnot be from a family memberA completed fit and proper person form, whichmust have been signed and dated no more thanthree months before we receive your applicationA completed NZ Police vetting service request andconsent form so that the New Zealand Police cancheck your record.
