1.1 Summary Statement<br />
SECTION 1 - GENERAL INFORMATION<br />
1.1.1 The <strong>Maryland</strong> Department of Health and Mental Hygiene (<strong>DHMH</strong>), Family Health Administration is<br />
issuing this <strong>Request</strong> for Proposals to provide direct breast and cervical cancer screening services, education, and<br />
follow up of abnormal results to low income, uninsured or underinsured women aged 40 to 64 years in Calvert<br />
County.<br />
1.1.2 It is the State's intention to obtain services, as specified in this <strong>Request</strong> for Proposals, from a Contract between the<br />
successful Offeror and the State.<br />
1.1.3 The Department intends to make a single award to the Offeror whose proposal is deemed to be the most<br />
advantageous to the State.<br />
1.1.4 Offerors, either directly or through their sub-contractor(s), must be able to provide all services and meet all of the<br />
requirements requested in this solicitation.<br />
1.2 Abbreviations and Definitions<br />
For purposes of this RFP, the following abbreviations or terms have the meanings indicated below:<br />
a. BCCP – <strong>Maryland</strong> Breast and Cervical Cancer Early Detection Program<br />
b. COMAR – Code of <strong>Maryland</strong> Regulations available on-line at www.dsd.state.md.us<br />
c. Contract – The Contract awarded to the successful Offeror pursuant to this RFP. The Contract will be in the form of<br />
Attachment A.<br />
d. Contract Monitor – The State representative for this project that is primarily responsible for contract administration<br />
functions, including issuing written direction, compliance with terms and conditions, monitoring this project to ensure<br />
compliance with the terms and conditions of the contract, and in achieving on budget/on time/on target (e.g., within<br />
scope) completion of the project.<br />
e. Contractor – The selected Offeror that is awarded a Contract by the State.<br />
f. <strong>DHMH</strong> – <strong>Maryland</strong> Department of Health and Mental Hygiene.<br />
g. Fiscal <strong>Year</strong> – The <strong>DHMH</strong> fiscal year runs from July 1 through June 30.<br />
h. LAN – Local Area Network.<br />
i. Local Time – Time in the Eastern Time Zone as observed by the State of <strong>Maryland</strong>.<br />
j. Low Income – An individual having an income of 250% or less of the federal poverty guidelines.<br />
k. MBE – A Minority Business Enterprise certified by the <strong>Maryland</strong> Department of Transportation under COMAR<br />
21.11.03.<br />
l. MCEs- The Minimal Clinical Elements, developed by the <strong>DHMH</strong> Breast and Cervical Medical Advisory<br />
Committees, serve as the standard of care for the program. The MCEs are available at:<br />

m. Medically Underserved – An individual or sub-population who may not have a doctor, who are not users of<br />
preventive health care or regular health care, and for whom barriers exist to accessing personal health services.<br />
