Welcome to Delta Dental PPO Dentist Directory This directory is intended as a guide for selecting a Delta Dental PPO dentist. Selecting a dentist listed in the Delta Dental PPO directory will ensure that you receive your maximum benefits. The dentists listed in this directory are current as of the printing date shown on the bottom of each page of the directory. This means that any changes to the dentist network after this date will not be reflected in this directory. Role of Your Dentist Your dentist's job is to coordinate the dental services you receive, from preventive dental care such as exams and cleanings to dental treatments such as repairing a broken tooth, removing decay from a tooth and restoring the tooth with a filling. An ongoing relationship with your dentist increases your chances of receiving the best care possible. Choosing a Dentist Delta Dental of Minnesota wants you to play an active role in decisions about your dental care . While you do not need to pre-select a dentist, selecting a dentist from the Delta Dental PPO directory will ensure that you receive the maximum benefits. In-Network: When you choose a dentist from this directory, your dental care is called “in-network” and you will receive important benefit advantages, including lower costs by receiving the maximum level of benefits available to you. Out-of-Network: If you receive dental care from a dentist not listed in this directory, your care is considered “out-of-network” and will not be covered. Remember to verify your dentist’s participation status at the time you make an appointment . Checking with your dentist or calling Customer Service to verify participation assures you that your dentist is still participating in the Delta Dental PPO network. If your dentist's status has changed and he or she is no longer part of the network, you may continue seeing your dentist, however, this will result in a higher out-of-pocket cost for you as out of network providers are not covered by this dental plan. Or you may choose another Delta Dental PPO dentist and receive maximum benefits. Consult your Employee Benefits department or a benefit booklet for more information regarding your specific plan design.
Selecting Your Dentist The following are tips to help you select a dentist: · Can the dentist coordinate all of the dental services you and your family members may need? · Is the dentist accepting new patients? · What are the office hours? Are there evening or weekend appointments? · What are the procedures for urgent or emergency care after hours? · How long will you have to wait to get an appointment for routine exams and cleanings ? Acute or same-day care? After-Hours Care Call your dental office, even if it is after regular office hours. Your dental office may have an emergency number for assistance if an emergency occurs after hours. For routine and non-emergency care, it is best to call during regular business hours. Seeing a Specialist You do not need a referral to see a Delta Dental PPO network specialist. When special services not provided by our regular dentist are needed, your dentist will help you find a specialist to treat your condition. You may also select a specialist yourself. Remember that you will receive the most comprehensive benefits when you select a Delta Dental PPO participating specialist. Quality Assurance Having a dentist you can trust is important in your overall health. All of the dentists listed in this directory meet network and credentialing standards, and have signed agreements with Delta Dental of Minnesota to provide services, and submit claims for covered services directly to Delta Dental of Minnesota. In addition, a participating dentist may not balance bill any amount over the allowed or approved amount on the Explanation of Benefits (EOB) issued to Participating dentists and the Delta Dental Subscriber. The subscriber’s only responsibility is for any co-payment, deductible or other non-covered service identified on the EOB. If, for any reason, you are not satisfied with the quality of care you receive, please call Customer Service. I.D. Card Your Delta Dental I.D. card provides important benefit membership information. Keep this card with you at all times and present it each time you see your Delta Dental dentist. Your I.D. card does not guarantee eligibility for services. It is an information tool to be used by you and your dentist. Dentist verification and pre-estimate of benefits may still be necessary.
