Selecting Your Dentist<br />
The following are tips to help you select a dentist:<br />
· Can the dentist coordinate all of the dental services you and your family members may<br />
need?<br />
· Is the dentist accepting new patients?<br />
· What are the office hours? Are there evening or weekend appointments?<br />
· What are the procedures for urgent or emergency care after hours?<br />
· How long will you have to wait to get an appointment for routine exams and cleanings ?<br />
Acute or same-day care?<br />
After-Hours Care<br />
Call your dental office, even if it is after regular office hours. Your dental office may have an<br />
emergency number for assistance if an emergency occurs after hours. For routine and<br />
non-emergency care, it is best to call during regular business hours.<br />
Seeing a Specialist<br />
You do not need a referral to see a <strong>Delta</strong> <strong>Dental</strong> <strong>PPO</strong> network specialist. When special services<br />
not provided by our regular dentist are needed, your dentist will help you find a specialist to treat<br />
your condition. You may also select a specialist yourself. Remember that you will receive the<br />
most comprehensive benefits when you select a <strong>Delta</strong> <strong>Dental</strong> <strong>PPO</strong> participating specialist.<br />
Quality Assurance<br />
Having a dentist you can trust is important in your overall health. All of the dentists listed in this<br />
directory meet network and credentialing standards, and have signed agreements with <strong>Delta</strong><br />
<strong>Dental</strong> of <strong>Minnesota</strong> to provide services, and submit claims for covered services directly to <strong>Delta</strong><br />
<strong>Dental</strong> of <strong>Minnesota</strong>. In addition, a participating dentist may not balance bill any amount over<br />
the allowed or approved amount on the Explanation of Benefits (EOB) issued to Participating<br />
dentists and the <strong>Delta</strong> <strong>Dental</strong> Subscriber. The subscriber’s only responsibility is for any<br />
co-payment, deductible or other non-covered service identified on the EOB. If, for any reason,<br />
you are not satisfied with the quality of care you receive, please call Customer Service.<br />
I.D. Card<br />
Your <strong>Delta</strong> <strong>Dental</strong> I.D. card provides important benefit membership information. Keep this card<br />
with you at all times and present it each time you see your <strong>Delta</strong> <strong>Dental</strong> dentist.<br />
Your I.D. card does not guarantee eligibility for services. It is an information tool to be used by<br />
you and your dentist. Dentist verification and pre-estimate of benefits may still be necessary.
