Team gives new impetusfor End of Life CareTHE Trust’s End of LifeCare Team has recentlybeen highlighted asbeing the second bestperforming unit in thecountry in terms ofdischarging people fromhospital in time to die intheir homes or preferredvenue. Over the last 17months the team hasdischarged more than 130patients in time for themto fulfil their dying wish.End of Life CareCoordinator, Lorraine Tymon,said the Trust was making great stridesin allowing people to die in their chosenvenue.She said: “An audit of patients in2009 revealed a lot of possibilities forimprovements in this area. There wereno clear pathways between the variousagencies, poor documentation and a lackof early discussions with patients andtheir families about a preferred place ofdeath.“Since then we have made a numberof improvements with much morecollaboration with outside agencies suchas GPs, care homes and social servicesas well as a lot of training with <strong>hospitals</strong>taff to recognise the deterioratingpatient and have the relevantconversations with their families andagreeing a care plan.’’End of Life TeamEmma Montgomery leads the Trust’sRapid Discharge Team and she said allfamilies should have a conversationabout how they would rather be treatedat the end of their life.She said: “Current figures show that60% of people currently die in hospitalwhen 90% of people would choose todie in their preferred venue.“We would encourage relatives to tellhospital staff their wishes as soon aspossible so the team can do all theycan to make sure the dying wishes arecarried out. It is not possible in everycase but we want to do all we can togive people the death they deserve.“The project is an excellent example ofhow several agencies can work togetherto put together a package of care thatreally does make a difference.’’The newsletter for Foundation Trust membersPAGE | Thirteen
