engaging a provider.
File your long-term care claim through Lincoln
• Call 800-487-1485.
A dedicated claims specialist is here for you
five days a week during normal business hours.
• You can also log on to LincolnFinancial.com.
• Click CONTACTS, FORMS & CLAIMS in the top
navigation bar.
• In the dropdown menu under Forms, choose
Long-term care.
• Select the Start a claim tab.
• Click START CLAIMS PROCESS.
• Lincoln will contact you within two days of submitting
your online form.
