MELATONIN AND SLEEP PROBLEMS IN <strong>ASD</strong><br />

Introduction<br />

Children with <strong>ASD</strong> have problems with sleep. Improv<strong>in</strong>g<br />

sleep habits helps many children, <strong>in</strong>clud<strong>in</strong>g those<br />

with <strong>ASD</strong>, to get better sleep. For some children with<br />

<strong>ASD</strong>, sleep problems happen even with good sleep<br />

habits. <strong>Melaton<strong>in</strong></strong> is a common medic<strong>in</strong>e your doctor<br />

or healthcare provider may suggest to help improve<br />

sleep. This tool kit is designed to provide parents<br />

with <strong>in</strong><strong>for</strong>mation about melaton<strong>in</strong>.<br />

What is melaton<strong>in</strong>?<br />

<strong>Melaton<strong>in</strong></strong> is made <strong>in</strong> the p<strong>in</strong>eal gl<strong>and</strong> of the bra<strong>in</strong>.<br />

It is part of the bra<strong>in</strong>’s <strong>in</strong>ternal clock system. This<br />

system tells the body when to go to sleep <strong>and</strong> when<br />

to be awake. <strong>Melaton<strong>in</strong></strong> levels usually beg<strong>in</strong> to rise<br />

<strong>in</strong> the early even<strong>in</strong>g <strong>and</strong> are highest <strong>in</strong> the hours<br />

after midnight. They slowly get lower by morn<strong>in</strong>g <strong>and</strong><br />

rema<strong>in</strong> low dur<strong>in</strong>g the day. <strong>Melaton<strong>in</strong></strong> is also important<br />

<strong>for</strong> other time-related changes such as seasonal<br />

changes <strong>and</strong> puberty.<br />

<strong>Melaton<strong>in</strong></strong> is made by the bra<strong>in</strong> when there are<br />

changes <strong>in</strong> light. <strong>Melaton<strong>in</strong></strong> is sometimes referred to<br />

as the ‘hormone of darkness’ as it is high when light<br />

levels are low, like <strong>in</strong> the even<strong>in</strong>g. <strong>Melaton<strong>in</strong></strong> is low<br />

when light is high, like <strong>in</strong> the morn<strong>in</strong>g.<br />

Why might supplemental<br />

melaton<strong>in</strong> help children with<br />

autism spectrum disorders<br />

(<strong>ASD</strong>)?<br />

Children <strong>and</strong> teens with Autism Spectrum Disorder<br />

(<strong>ASD</strong>) often have problems with sleep; about two<br />

thirds have sleep difficulties. This can <strong>in</strong>clude<br />

problems fall<strong>in</strong>g asleep, wak<strong>in</strong>g up overnight, <strong>and</strong><br />

wak<strong>in</strong>g too early <strong>in</strong> the morn<strong>in</strong>g. Low or uneven<br />

levels of melaton<strong>in</strong> <strong>in</strong> people with <strong>ASD</strong> may be part<br />

of the problem, although this needs more study.<br />

Tak<strong>in</strong>g supplemental melaton<strong>in</strong> helps some children<br />

with <strong>ASD</strong> to sleep better. For children with poor sleep<br />

that did not get better with improved sleep rout<strong>in</strong>es,<br />

melaton<strong>in</strong> may decrease the time it takes to fall<br />

asleep.<br />

There is no <strong>in</strong><strong>for</strong>mation on the effect of melaton<strong>in</strong><br />

on sleep <strong>in</strong> children with <strong>ASD</strong> younger than 2 years<br />

of age.<br />

How is melaton<strong>in</strong> used <strong>for</strong><br />

sleep problems?<br />

<strong>Melaton<strong>in</strong></strong> is taken by mouth to help with sleep<br />

problems. It is used to help with jet lag <strong>and</strong> is sometimes<br />

used by shift workers to assist with fall<strong>in</strong>g<br />

asleep dur<strong>in</strong>g daytime hours. It is also used by<br />

people who cannot fall asleep until very late at night.<br />

The idea is that tak<strong>in</strong>g melaton<strong>in</strong> will help to reset<br />

the bra<strong>in</strong>’s <strong>in</strong>ternal clock or add to the bra<strong>in</strong>’s<br />

natural melaton<strong>in</strong> to help improve sleep.<br />

<strong>Melaton<strong>in</strong></strong> stays <strong>in</strong> the blood <strong>for</strong> a short time so it is<br />

taken by mouth every night. It is taken 30 m<strong>in</strong>utes<br />

be<strong>for</strong>e the <strong>in</strong>tended sleep time <strong>and</strong> started at a low<br />
