The best thing about memories...
... is making them.

This year, the theme of National Hospice & Palliative Care Month is
“It’s About How You Live”.
This book highlights some of the ways we’ve been involved in the lives of our
patients and partners over the last year.
Cherish your yesterdays, dream your tomorrows and live your todays.
Happy New Year!

Hospice Care
Our purpose is to provide compassionate endof-life
care through our quality team of hospice
professionals. We embrace the whole person
and their family. Their emotional, physical and
spiritual needs are our primary focus.
The decision to accept hospice care can be
a difficult one. Assisted will meet the patient
and their family to explain hospice. The benefits of hospice are best realized when there
are months rather than days to establish an effective plan of care for comfort and support.
You might want to consider hospice care when one or more of the following occur:
• Progressive Loss of Function
• Repeated Hospitalizations
• Current Medical Treatment Outweighs the Benefit
• Decline Despite Curative Treatments
• Decision to Stop Curative Measures

Palliative Care
Physical pain leads to emotional pain and
when you’re hurting, so are your loved ones.
This is why palliative care by Assisted focuses
on the patient’s comfort.
Assisted’s palliative care professionals help
patients carry on with daily life in spite of their
illness and improves their ability to tolerate
medical treatments. Overall, palliative care helps our patients make the most of life.
Palliative Care:
• Can begin at the same time as the diagnosis
• Focuses on helping people live longer, happier lives
• Is about managing the symptoms and side-effects of life-limiting and chronic illness
• Can be performed at the same time you receive treatment meant to cure your illness
• Can begin at any stage of an illness
• Treatments are tailored to the individual based on his or her medical needs
• Emotional and psychosocial support
