Medication management safety technology ® ® The Pyxis PARX system and the Pyxis MedStation ® system can help hospitals improve medication safety, comply with regulations and increase staff productivity and satisfaction by integrating automated barcoding during pick, check and delivery with profiled ADCs. Bringing these technologies together provides a closed-loop medication replenishment and reporting system with redundant checks, standardized processes and actionable information (Figure 1) that can increase a sense of trust and security among staff. In today’s tight resource environment, automation can also help pharmacists free up time for the development of clinical services. Closed-loop replenishment The newest version of the pharmacy auto-replenishment system lets you use automated scanning technology to measurably improve safety and accuracy during the pick-check-refill process. The system is able to read twodimensional (2D) barcodes and operates on the hospital’s wireless network. Closed-loop reporting The system’s reporting capabilities allow staff to view actionable information across the entire medication refill process to help identify trends, pinpoint opportunities to improve safety, reinforce standardized practice and reduce diversion. Data automatically gathered by the system also help staff to identify opportunities to make process improvements related to refill, staff utilization, turnaround times and more. The different reports available on these systems (Table 2) can help hospitals to meet regulatory requirements and to identify opportunities for CQI. Figure 1 Pick Deliver Closed Loop Reporting Check
Helping to ensure accuracy at every step Picking A technician responsible for picking medications powers up a system’s handheld device and selects an ADC in a particular area, for example 5 West, to begin the process. Drawing from a real time queue of medications, the handheld device displays the first medication to be replenished. The technician selects the medication from the shelf and scans the medication’s barcode to ensure the right medication has been selected. Although most hospitals are taking precautions to separate lookalike/sound-alike medications in compliance with National Patient Safety Goals, with thousand of doses of medications available there is still extensive opportunity for error. If the wrong medication is selected, the system alerts the technician to the error and prompts the technician to go back and select the correct medication. If the right medication is selected, the user is prompted to enter the number of medications to be picked, and a small transportable printer prints out a label to be placed on a bag that will contain the medication. Compared with a manual system, this automated system helps to increase safety during the pick process and introduces a standardized practice for replenishing medications. The documentation trail also helps to reduce diversion by documenting picker, item and quantity, among others. Checking During the check process, medications picked using the automated system are systematically reviewed at a system check station, which comprises of a touch-screen computer and an attached barcode scanner. The person checking the medication scans the label on the bag and information on the contents of the bag is displayed on the touch screen for the reviewer to approve. Given the hectic pharmacy environment, with a manual system, there is a risk that meds might unknowingly leave the pharmacy without being checked. An automated replenishment system provides the capability to document that 100 percent of the medications picked were checked. This systematic approach to checking medications is the second point where a hospital can improve safety, introduce standardization and decrease diversion. Delivering To close the medication replenishment loop, medications selected in the pharmacy using the automated system are delivered to the profiled ADC. This end to end refill process helps ensure that when nursing staff comes to remove medications, the right medication will be in the right pocket. For high-risk medications such as heparin, additional technology provides single-pocket, closed-lid access to the medication. The refill/stocking process begins when the technician scans the automated system label. If the wrong medication is scanned, the technician is alerted to the error. If the right medication is scanned, the correct drawer slides out and the pocket containing the medication opens automatically. The medication is then loaded into the ADC. When the medication is required for a patient, nursing and pharmacy staff have the added security that the medication was verified in the pharmacy, doublechecked using a systematic approach and scanned to ensure placement in the right station, right drawer and right pocket. Through connectivity between the automated replenishment checking system and the ADC system, a real time queue of medications needing to be replenished is maintained, so the process continually cycles to meet a hospital’s medication needs. Finally, the closed-loop reporting capabilities can help staff to identify opportunities for CQI.
