IN TREATING ACROMEGALY What Matters Most ABBREVIATED PRESCRIBING INFORMATION Indications: Treatment of patients with acromegaly: For symptomatic control and reduction of GH and IGF 1 levels in patients with acromegaly: • who are adequately controlled on s.c. treatment with Sandostatin, in whom surgery, or radiotherapy is inappropriate or ineffective, or in the interim period until radiotherapy becomes fully effective. • In short term (3-12 months) treatment prior to pituitary surgery GEP Tumours: Relief of symptoms of gastroenteropancreatic (GEP) tumours including carcinoid tumours, VIPomas and glucagonomas, in patients whose symptoms are adequately controlled on s.c. treatment with Sandostatin. Sandostatin is not antitumour therapy and is not curative in these patients. Presentation: Long-acting depot injection form of octreotide (as acetate): 10, 20 or 30 mg per vial each supplied with vehicle. Dosage and Administration: Sandostatin LAR may only be administered by deep intragluteal injection. The site of repeat intragluteal injections should be alternated between the left and right gluteal muscle. Acromegaly Adults: Deep intragluteal injection starting with 20 mg at 4 weekly intervals for 3 months in patients who have had a test dose of s.c. Sandostatin or who are adequately controlled with s.c. Sandostatin. Adjust according to clinical and biochemical response. Dose can be increased to 30 mg every 4 weeks. GEP tumours Adults: Deep intragluteal injection starting with 20 mg at 4 weekly intervals for 3 months in patients adequately controlled GH Control IGF-I Control Tumour Shrinkage Symptom Control on usual therapeutic range of s.c. Sandostatin. Treatment with s.c. Sandostatin should be continued at the previously effective dosage for 2 weeks after the first injection of Sandostatin LAR. Assess response after 3 months. Adjust according to clinical and biochemical response (range 10 mg to 30 mg every 4 weeks). Children: Limited data available. Contra-Indications: Hypersensitivity to octreotide or any components of formulation. Precautions: Monitor glucose tolerance and antidiabetic treatment in patients with diabetes mellitus. The depth and duration of hypoglycaemia may be increased in patients with insulinoma. As GH-secreting pituitary tumours may sometimes expand, causing serious complications (e.g. visual field defects), it is essential that all patients are carefully monitored. Ultrasonic examination of the gallbladder before and at about 6 monthly intervals during Sandostatin LAR therapy is recommended. Do not use during pregnancy or lactation. Interactions: Cyclosporine, cimetidine, bromocriptine. Somatostatin analogues may decrease the clearance of drugs metabolised by CYP3A4, particularly those which have a low therapeutic index (e.g. carbamazepine, digoxin, warfarin and terfenadine). Caution should be exercised during coadministration of octreotide and these drugs. Side-Effects: Pain and rarely, swelling and rash at injection site. Anorexia, nausea, vomiting, abdominal pain or bloating, flatulence, loose stools, diarrhoea and steatorrhoea. In rare instances, gastrointestinal side-effects may resemble acute intestinal obstruction, with progressive abdominal distension, severe epigastric pain, abdominal tenderness and guarding. Gallstones can develop during long-term treatment. Persistent hyperglycaemia, hepatic dysfunction, hypoglycaemia, transient alopecia and acute pancreatitis have been reported rarely.
