assistance in smoking cessati<strong>on</strong>, whe<strong>the</strong>r nurse, physician, or dentist, is not an important<br />

factor in influencing <strong>the</strong> level of success attained. It is <strong>the</strong> methods used with patients that<br />

count. A combinati<strong>on</strong> of clinical interventi<strong>on</strong> services increase chances for success, that is,<br />

motivati<strong>on</strong>al, skills development, pharmaco<strong>the</strong>rapies, and follow up after individuals quit to<br />

help prevent relapse.<br />

Dr. Mecklenburg outlined a highly successful cessati<strong>on</strong> programme that can be<br />

implemented with little effort <strong>on</strong> behalf of <strong>the</strong> health professi<strong>on</strong>al, <strong>the</strong> <strong>Public</strong> <strong>Health</strong><br />

Service clinical practice guideline, “Treating Tobacco Use and Dependence.” 25, 26 The<br />

guideline is based <strong>on</strong> an analysis of over 6,000 studies c<strong>on</strong>ducted throughout <strong>the</strong> world and<br />

published in peer-reviewed journals. The guideline is becoming <strong>the</strong> “gold standard” for all<br />

health care providers. It recommends that clinicians use <strong>the</strong> five “As,” “Ask, Advise, Assess,<br />

Assist and Arrange.” Ask <strong>the</strong> pers<strong>on</strong> about <strong>the</strong>ir tobacco use, advise <strong>the</strong>m to quit, assess<br />

<strong>the</strong>ir willingness to quit, assist those who are ready, and arrange follow up for <strong>the</strong>m. The<br />

clinical practice guideline methods recommended to assist patients quit are given<br />

irrespective of sex, age, or type of tobacco product used. Success rates are usually higher<br />

when patients have more educati<strong>on</strong> and higher income and lower if educati<strong>on</strong>ally and<br />

ec<strong>on</strong>omically disadvantaged. Individuals who are mentally or emoti<strong>on</strong>al disadvantaged and<br />

those who have multiple drug dependencies usually have lower success rates, thus might<br />

have to be referred to tobacco cessati<strong>on</strong> specialists.<br />

Although adopti<strong>on</strong> of clinical practice guideline-recommended methods can produce a<br />

double to three-fold increase in patient l<strong>on</strong>g-term quit rates compared to self-help<br />

methods, physicians and dentists often err<strong>on</strong>eously believe that <strong>the</strong>ir help would not be<br />

effective. Patients, however, usually expect and would appreciate such help. It appears that<br />

professi<strong>on</strong>als and patients differ in <strong>the</strong>ir expectati<strong>on</strong>s during this transiti<strong>on</strong> time. The<br />

clinical practice guideline is now accompanied by several supplemental materials for<br />
