Health Ambassador Programme (HAP)
Between Oct 2010 to Dec 2011, 1622 patients were documented to have counselling sessions on chronic disease
management and health promotion by community pharmacists. 2687 recommendations were given to patients, and
1591 follow-up were conducted. This gave an excellent follow-up rate of 98.1%. The outcome of pharmacist work
remained encouraging.
Majority of patients (64%) reported improved outcomes due to pharmacist advice. Since the change of focus
in 2010 is to encourage patient follow-up (instead of brief counselling), community pharmacists have
continuously produced great results.
