Excellent primary stability<br />
The elliptical shape of the <strong>Ellistra</strong> Cup enhances the peripheral press-fit and allows for excellent initial<br />
stability (a) .<br />
By smoothly increasing the diameter from dome to rim the clamping forces are distributed over a large<br />
peripheral area.<br />
Outside grooves in the press-fit area allow a greater interlock with the acetabular bone.<br />
The flattened dome prevents the cup from resting on the acetabular floor before establishing optimal<br />

Due to the self-guiding design all inserts 0° and 10° can be inserted easily, even in conditions of<br />

Wide and flexible choice of implant components<br />
The modular concept of the <strong>Ellistra</strong> cup system brings a solution<br />
for a variety of clinical demands.<br />
The <strong>Ellistra</strong> cup is available in grit-blasted and HA-coated<br />
versions.<br />
The system offers a comprehensive variety of polyethylene,<br />
ceramic/ polyethylene liners, each with a 0° and 10° shoulder.<br />
After impacting the cup and depending on the bone situation<br />
encountered, extra primary support can be provided by the<br />
insertion of one or more screws.<br />

position normale du bassin ou de la colonne vertébrale.<br />
La planification préopératoire suivante concerne un cas type de coxarthose. Si l’on ne peut pas<br />
déterminer avec précision la cupule de taille appropriée du côté à opérer, on procèdera à la<br />
planification préopératoire sur le côté non opéré.<br />
To achieve a successful outcome, it is indispensable to restore the normal position and kinematics of<br />
the hip joint, taking into account any leg length discrepancy, contractures, and axial deviations of the<br />
pelvis or spine.<br />
This preoperative planning is for a typical case of osteoarthritis of the hip. If sizing of the acetabular<br />
component cannot be achieved on the affected hip, the contralateral hip should be used as a reference.<br />
1 Préparation<br />

Draw a reference line on the x-ray, that connects the<br />
inferior margins of the teardrop. Mark the floor of the<br />
acetabulum and identify the sclerotic bone.<br />

2 Drawing the acetabular shell
− Overlay the template on the x-ray.<br />
− Determine the appropriate size and correct position of the implant.<br />
− Determine the normal position of the centre of rotation and mark it on a tracing paper.<br />
− Determine whether bone grafting will be necessary.<br />
Using the template, the appropriate size shell is that which outlines optimally match the subchondral<br />
sclerosis line, and which marker wire groove is aligned with the reference line. Use a tracing paper to<br />
draw the outlines of the pelvis and the selected shell, and mark the centre of rotation and dimensions of<br />
the shell. Draw a line to reference the position of the centre of rotation.<br />
3 Résultat de la planification<br />

The <strong>Ellistra</strong> Press-fit Acetabular Component can be inserted through any standard approach.<br />
1 Préparation du cotyle<br />
Exciser la capsule articulaire sur toute sa circonférence<br />
de manière à pouvoir identifier clairement les limites<br />
osseuses du cotyle et à pouvoir introduire la fraise.<br />
Eliminer toutes les structures fibreuses, chondrales et<br />
ossifiées qui empêchent la préparation du cotyle.<br />
1 Acetabular preparation<br />
Excise the joint capsule circumferentially in order to<br />
define the landmarks of the bony acetabulum and allow<br />
insertion of the reamer. Clear any fibrous/chondral<br />
tissue and osteophytes that may interfere with the<br />
preparation.<br />
2 Alésage du cotyle<br />
Aléser le cotyle avec la fraise acétabulaire sphérique<br />
qui pénètre facilement dans le cotyle préparé.<br />

2 Acetabular reaming<br />
Use a spherical acetabular reamer which bites easily into bone. The reamer should be 2 mm higher than<br />
the acetabular rim.<br />
Medialize the floor of the acetabulum. Ream sequentially upward in size (in 2 mm increments) until a<br />
symmetrical, hemispherical cavity has been created in the freshened bone.<br />
The size of the last reamer used should correspond to that of the templated shell.<br />
Note : During reaming, keep in mind the final position of the acetabular shell, and make sure that a stable<br />
and even support is provided at the periphery of the implant.<br />

3 Vérification de la cavité (option)<br />

3 Assessment of the prepared cavity (optional)<br />
Select the trial shell size that corresponds to the size of the last reamer used, and thread the trial shell<br />
onto the universal impactor.<br />
1 mm teeth have been designed on the outer surface of the trial. Assess the prepared cavity ; a stable<br />
and even support must be provided at the periphery of the implant.<br />
If support is inadequate, repeat reaming.<br />
4 Réduction d’essai (option)<br />
Visser l’insert d’essai correspondant (0° ou 10°)<br />
directement sur l’impacteur universel. Afin d’identifier<br />
l’épaulement de 10°, aligner les repères de l’impacteur<br />
(bout carré et manche) et le repère de l’insert d’essai<br />
10°.<br />
Introduire l’insert d’essai dans la cupule d’essai.<br />
Effectuer une réduction d’essai en vérifiant la sécurité<br />
antiluxation. Marquer la position optimale de<br />
l’épaulement à 10° le cas échéant.<br />
Retirer l’insert d’essai à l’aide de l’extracteur vissé sur<br />
l’impacteur universel.<br />
Lors de la réduction d’essai, la position de l’insert<br />
d’essai 0° est illimitée. Par contre, la forme carrée de<br />
l’insert d’essai 10° doit s’engager dans le carré situé<br />
dans le dôme de la cupule (ou de la cupule d’essai) afin<br />
de garantir la stabilité antirotatoire. Le positionnement<br />
des inserts permanents est illimité qu’ils soient à 0° ou<br />
à 10°.<br />
4 Trial reduction (optional)<br />
Thread the appropriate trial liner (0° or 10°) onto the universal impactor. To easily identify the 10°<br />
elevated rim in situ, take care to align the scribe marks on the impactor (square end and handle) and the<br />
scribe mark on the 10° trial liner.<br />
Insert the trial liner into the trial cup. Perform a trial reduction and check that no dislocation occurs. If<br />
necessary, mark the optimal position of the 10° elevated rim.<br />
Remove the trial liner with the extractor threaded on the universal impactor.<br />
The 0° trial liner can be inserted in any position. In contrast, due to its square shape, the 10° trial liner<br />
must mate with the square housing designed in the dome of the shell (or trial shell) to ensure rotatory<br />
stability. 0° and 10° final acetabular liners can be inserted in any position.<br />

5 Choix de la taille de la cupule press-fit<br />

5 Selection of the appropriate press-fit shell<br />
The press-fit shell should have the same size as the last reamer used.<br />
The actual diameter, as measured at the periphery of the shell, is 2 mm larger than the nominal diameter.<br />
For instance, Size 52 reamer corresponds to Size 52 shell (with a peripheral diameter of 54 mm).<br />
6 Positionnement et impaction de la cupule press-fit<br />
Visser à bloc la cupule sur l’impacteur universel. Les<br />
repères situés sur l’impacteur doivent pointer en<br />
direction du trou à vis supérieur. Orienter les trous dans<br />
la région supéro-interne de l’acétabulum lors de<br />
l’impaction.<br />
Les valeurs correctes d’inclinaison et d’antéversion<br />
dépendent de la voie d’abord choisie par le chirurgien,<br />
des conditions anatomiques et de l’implant fémoral.<br />
Impacter la cupule. Plus l’ajustement est serré, plus le<br />
cotyle hémisphérique (profil obtenu par alésage)<br />
s’adapte à la forme de la cupule et plus la congruence<br />
entre la cupule et le cotyle est bonne.<br />
Lorsque la densité osseuse n’est pas idéale, retirer les<br />
vis obturateur avant l’impaction de la cupule afin de<br />
réduire les contraintes indésirables dues au dévissage<br />
(voir l’étape 8, pages 11&12).<br />
Option : Utiliser le guide de positionnement “Sputnik”<br />
40° pour positionner la cupule.<br />
Insérer le sputnik sur le manche de l’impacteur puis<br />
fixer la cupule. Impacter la cupule.<br />
6 Insertion and impaction of the press-fit shell<br />
Screw the shell fully home onto the universal impactor. The markings on the impactor should point<br />
towards the superior screw hole. During impaction, the screw holes should be positioned in the<br />
superomedial portion of the acetabulum.<br />
The amount of abduction and anteversion depends on the approach used, the patient's anatomy, and the<br />
femoral stem.<br />
Impact the shell into place. With interference fit, the hemispherical cavity takes the exact shape of the<br />
shell.<br />
In patients with poor bone density, remove the screw hole covers prior to impaction in order to decrease<br />
the undesirable stresses generated during unscrewing (see Step 8, Pages 11 & 12).<br />
Option : It is recommended to use the Sputnik 40° positioner to achieve proper shell positioning.<br />
Put the Sputnik positioner to the impactor handle, and assemble the acetabular shell to the impactor.<br />
Impact the shell into place.<br />

7 Vérification de l’assise de la cupule<br />
Passer l’extrémité de la jauge de profondeur par le trou<br />
du dôme de la cupule, afin de s’assurer que l’on a<br />
poussé l’implant à environ 1mm du fond du cotyle.<br />
Vérifier l’assise de la cupule en appliquant une charge<br />
modérée de traction, de rotation et de basculement<br />
(test du manche à balai).<br />
Lorsque la stabilité primaire est suffisante, l’ancrage<br />
par autoblocage est suffisant. On pourra alors renoncer<br />
à utiliser des vis.<br />
Le cas échéant, retirer la cupule à l’aide de l’extracteur<br />
vissé sur l’impacteur et répéter les étapes 3 à 6 pour<br />
placer éventuellement une cupule de taille supérieure.<br />
7 Assessment of stability<br />
Insert the tip of the depth gauge into the apex hole to make sure that the shell is positioned within about 1<br />
mm of the floor of the acetabulum.<br />
Assess stability of the shell by applying moderate traction, rotation and tilt, and note any motion ("broom<br />
handle test").<br />
If adequate stability has been achieved, press-fit fixation is sufficient and there is no need for screws.<br />
Where necessary, remove the shell and repeat Steps 3 through 6 to insert the next larger shell.<br />

Connect the screwdriver handle to the universal selftightening<br />
screwdriver. Then remove the preassembled<br />
screw hole covers.<br />

8B Perçage des avant-trous pour les vis<br />
Avant de percer, s’assurer que le bout de la mèche est<br />
en contact avec le fond du cotyle et qu’elle ne touche<br />
pas la paroi du guide-mèche. Percer des avant-trous à<br />
la longueur voulue, en se servant du double guidemèche<br />
et d’une mèche à arbre flexible Ø 2,5mm.<br />
8B Drilling pilot holes<br />
Before drilling, check that the drill is in contact with the<br />
floor of the acetabulum and does not touch the wall of<br />
the drill guide. Drill the pilot holes to the desired depth,<br />
using the double drill guide and a 2.5 mm flexible drill<br />
bit.<br />
8C Détermination de la longueur des vis à utiliser<br />
Déterminer la longueur des vis à utiliser au moyen de la<br />
jauge de profondeur à douille coulissante. Insérer la<br />
jauge dans l'avant trou et pousser le manchon en<br />
direction de la cupule jusqu’à rencontrer une<br />

8C Screw length<br />
Determine the appropriate screw length using the depth<br />
gauge with its sliding sleeve. Insert the depth gauge<br />
into the pilot hole and push the sleeve until resistance<br />
is met. Size is read off from the scale on the back of the<br />
instrument handle.<br />
8D Mise en place des vis<br />
Poser les vis à cotyle autotaraudantes de Ø 5,0mm à<br />
l’aide de la pince porte-vis ainsi que du tournevis<br />
autoserrant à cardan assemblé à la poignée pour<br />
tournevis.<br />
Remarque : la géométrie des trous à vis et des têtes de<br />
vis offre un jeu angulaire de 24°.<br />
Important : visser entièrement les vis de sorte que les<br />
têtes de vis ne dépassent pas.<br />
8D Screw insertion<br />
Insert the 5.0 mm self-tapping cancellous screws using<br />
the screw holding forceps and the universal selftightening<br />
screwdriver/handle assembly.<br />
Note : Screw holes and screw heads have been<br />
designed to provide up to 24° screw angulation.<br />
Important : Screws should be inserted fully home so<br />
that screw heads do not protrude.<br />

9 Obturation du trou du dôme<br />
Visser la vis obturatrice dans le trou du dôme au moyen<br />
du tournevis à cardan autoserrant monté sur la poignée<br />
pour tournevis. Une fois la vis obturatrice posée, rincer<br />
abondamment l’aire de la plaie et nettoyer<br />
soigneusement la cupule.<br />
9 Closing the apex hole<br />
Thread the scew hole cover into the apex hole using<br />
the universal self-tightening screwdriver/handle<br />
assembly. Once the apex hole is closed, copiously<br />
irrigate the wound and thoroughly clean the shell.<br />
A ce stade, il est possible d’effectuer une réduction d’essai au moyen de l’insert d’essai que l’on introduira<br />
directement dans la cupule. La réduction d’essai est décrite à l’étape 4 de la page 9.<br />
At this stage, a trial reduction can be performed with the trial acetabular liner and the trial shell in place.<br />
Trial reduction is described in Step 4, Page 9.<br />
10 Solidarisation de l’insert et de la cupule<br />
Eliminer tout résidu de tissus dans la cupule en place<br />
ou au bord du support afin de permettre la<br />
solidarisation de l’insert et de la cupule.<br />
Pour les inserts en polyéthylène à sertissage<br />
céramique, visser le disque de positionnement<br />
approprié (0° ou 10°) sur l’impacteur universel. Une<br />
rainure située sur le rebord de l’insert facilite le<br />
centrage des trois chevilles du disque de sorte qu’elles<br />
pénètrent dans les trous du rebord de l’insert.<br />
Positionner l’insert de manière à respecter les données<br />
anatomiques, en se servant des repères situés sur les<br />
instruments et sur l’implant. Introduire l’insert dans le<br />
support propre. Un petit coup sur l’impacteur universel<br />
permet de solidariser l’insert et la cupule. La cupule et<br />
l’insert sont assemblés à demeure.<br />
Remarque : il est possible d’extraire l’insert au moyen<br />
d’une vis à spongieux ou d’un ciseau à os. Ne pas<br />
réutiliser l’insert extrait. Choisir un nouvel insert et<br />
répéter l’étape 10.<br />
10 Liner insertion<br />
Before inserting the acetabular liner, the rim and interior of the shell should be carefully cleaned of any<br />
remaining soft tissue which would compromise locking of the liner within the shell.<br />
For polyethylene / ceramic acetabular liners, thread the appropriate (0° or 10°) impactor plate onto the<br />
universal impactor. The peripheral groove designed in the rim of the liner facilitates positioning of the<br />
three studs of the impactor plate, which must seat in the corresponding holes in the rim of the liner.<br />
Position the liner in an anatomic position, based on the scribe marks on the instruments and the implant.<br />
Insert the liner into the acetabular shell. Apply a moderate mallet blow to the universal impactor to<br />
permanently lock the liner within the shell.<br />
Note : If necessary, the liner can be removed using a cancellous screw or a bone chisel. Once removed,<br />
the liner MUST NOT BE reused. A new acetabular liner must be selected and inserted as described in<br />
Step 10.<br />

Oter l’insert à l’aide d’un ciseau à os. Puis, retirer les vis à cotyle optionnelles et la vis obturatrice du<br />
dôme avec le tournevis (manche + embout). Puis visser l’impacteur universel à fond dans la cupule et<br />
visser l’extracteur sur le manche de l’impacteur universel. Enfin, extraire la cupule.<br />
L’explant ne doit en aucun cas être réutilisé.<br />
Extraction of the prosthesis<br />
First, the liner can be removed using a bone chisel. Then, remove the optional cancellous screws and<br />
the scew hole cover on the apex hole using the universal self-tightening screwdriver/handle assembly.<br />
Then, screw the shell fully home onto the universal impactor, and screw the universal impactor handle<br />
onto the extractor. Extract the cup.<br />
Once removed, the prosthesis MUST NOT BE reused.<br />
