SLIDEX ® Staph-Kit est un test rapide d'agglutination de particules de latex et d'hématies pour l'identification des souches<br />
de Staphylococcus aureus à partir des milieux d'isolement.<br />

INTRODUCTION ET OBJET DU TEST<br />
Les staphylocoques font partie des bactéries les plus<br />
fréquemment rencontrées en pathologie humaine. Les<br />
infections les plus communes provoquées par<br />
Staphylococcus aureus sont cutanées, mais des atteintes<br />
sévères (septicémies, endocardites, méningites,<br />
pneumonies, ostéomyélites) peuvent être observées en<br />
milieu hospitalier (1).<br />
La méthode la plus répandue pour l'identification de<br />
Staphylococcus aureus a longtemps été la recherche de<br />
la coagulase. Ce test est basé sur la capacité des<br />
souches de Staphylococcus aureus à produire cette<br />
enzyme extra-cellulaire qui coagule le plasma de lapin.<br />
Ce test permet ainsi de différencier Staphylococcus<br />
aureus des autres espèces de staphylocoques,<br />
majoritairement coagulase négative. Cependant, cette<br />
méthode nécessite 4 à 24 heures d'incubation.<br />
D'autres méthodes, basées sur l'agglutination d'hématies<br />
ou de particules de latex, sont actuellement reconnues<br />
pour l'identification de Staphylococcus aureus (1, 2).<br />
En effet, Staphylococcus aureus possède un récepteur<br />
protéique pour un fragment du fibrinogène. Ce récepteur<br />
est présent sur 96 % des souches d'origine humaine (3)<br />
et sur la plupart des autres biotypes. Certaines souches<br />
résistantes aux Pénicillines M ne présentent pas ce<br />
facteur d'affinité pour le fibrinogène (4).<br />
A la surface de Staphylococcus aureus une protéine, la<br />
protéine A, réagit électivement avec le fragment Fc de la<br />
plupart des immunoglobulines d'isotype G (IgG). Cette<br />
protéine est produite sur tous les milieux mais faiblement<br />
sur gélose au sang. Elle est présente sur environ 90 %<br />
des souches (5). Enfin, des protéines ou glycoprotéines et<br />
des acides téichoïques dont les spécificités antigéniques<br />
sont extrêmement variables (1) existent à la surface de<br />
Staphylococcus aureus.<br />
SLIDEX Staph-Kit permet l’identification des souches de<br />
Staphylococcus aureus à partir de milieux d’isolement.<br />
PRINCIPE<br />
SLIDEX Staph-Kit est un réactif constitué d’un mélange :<br />
- d’hématies de mouton stabilisées, sensibilisées par du<br />
fibrinogène et agglutinées lorsqu'elles sont mises en<br />
présence d'une souche de Staphylococcus aureus<br />
portant le récepteur d’affinité pour le fibrinogène,<br />
- de particules de latex sensibilisées par un anticorps<br />
monoclonal (IgG) et agglutinées lorsqu'elles sont<br />
mises en présence d'une souche de Staphylococcus<br />
aureus portant la protéine A et/ou des antigènes<br />
protéiques de surface.<br />

Les prélèvements, cultures bactériennes et produits<br />
ensemencés doivent être considérés comme<br />
potentiellement infectieux et doivent être manipulés de<br />
façon appropriée. Les techniques aseptiques et les<br />
précautions usuelles de manipulation pour le groupe<br />
bactérien étudié doivent être respectées tout au long de<br />
la manipulation ; se référer à "CLSI/NCCLS M29A,<br />
Protection of Laboratory Workers from Occupationally<br />
Acquired Infections, Approved Guideline – révision en<br />

dans le pays d'utilisation.<br />
Ne pas utiliser les réactifs après la date de péremption<br />
indiquée sur l’étiquette coffret.<br />
Eviter le contact avec les yeux, la peau ou les<br />
vêtements.<br />
Ne pas intervertir ou mélanger les réactifs de lots<br />
différents.<br />
Ne pas pipeter à la bouche les réactifs.<br />
Ne pas utiliser d'oese métallique pour réaliser le test<br />
d'agglutination au latex.<br />
Ne pas réutiliser les cartes jetables.<br />
Les réactifs du coffret contiennent un conservateur<br />
(azoture de sodium), susceptible de réagir avec les<br />
tuyauteries en plomb ou en cuivre et de former des<br />
azotures métalliques explosifs. Il est recommandé de<br />
rincer à l'eau tout rejet.<br />
L'interprétation des résultats du test doit être faite en<br />
tenant compte du contexte clinique, de l'origine du<br />
prélèvement, des aspects macro et microscopiques de<br />
la souche et éventuellement des résultats d'autres tests.<br />
CONDITIONS DE STOCKAGE<br />
Conserver les réactifs à 2-8°C.<br />
Ne pas congeler les réactifs.<br />
Si les réactifs ont été congelés accidentellement, ils ne<br />
doivent pas être utilisés.<br />
Tous les composants sont stables jusqu'à la date de<br />
péremption indiquée sur l'étiquette coffret, s'ils sont<br />
conservés dans les conditions préconisées.<br />
ECHANTILLONS (prélèvements et préparation)<br />
Ensemencer le prélèvement sur un milieu d'isolement<br />
sélectif (gélose Chapman, gélose Baird Parker, gélose<br />
tellurite glycine) ou non sélectif [(gélose Columbia au<br />
sang de mouton (5 %), gélose Trypcase soja avec ou<br />
sans sang de mouton (5 %), gélose Mueller Hinton avec<br />
ou sans sang de mouton, gélose CLED, gélose BCP,<br />
gélose CPS ID] : les performances peuvent varier en<br />
fonction du type de milieu utilisé.<br />
Une étude interne réalisée sur 191 souches de S. aureus<br />
(MRSA et MSSA) et 89 souches de S. non aureus montre<br />
une sensibilité identique en fonction du milieu utilisé.<br />
L’étude montre également que les résultats de spécificité<br />
ne sont pas statistiquement différents en fonction du<br />
milieu utilisé (voir tableau ci-dessous). Cependant, il est<br />
recommandé d’utiliser les milieux suivants : Gélose<br />
Columbia au sang, Columbia ANC + sang, TSA, TSA +<br />
sang, CPS et Baird Parker.<br />

MODE OPERATOIRE<br />
1. Laisser les réactifs à température ambiante (18-25°C)<br />
avant utilisation.<br />
2. Bien remettre en suspension les réactifs. Chasser les<br />
bulles retenues dans les compte-gouttes.<br />
3. Sur une carte jetable ou lame de verre porte-objet<br />
parfaitement propre, dans 2 cercles, déposer :<br />
- 1 goutte de R1 (réactif anti-Staphylococcus aureus) ;<br />
- 1 goutte de R2 (réactif contrôle négatif).<br />
Veiller à tenir les flacons compte-gouttes verticalement<br />
lors de la distribution des gouttes.<br />
4. Dans chacune des gouttes, disperser très<br />
soigneusement et complètement pendant 10 secondes,<br />
1 à 2 colonies suspectes de taille moyenne issues<br />
d’une gélose non sélective ou 3 à 6 colonies très<br />
petites issues d’une gélose sélective (ex. : gélose<br />
Chapman) à l'aide d'une oese ou d’un bâtonnet<br />
jetable.<br />
5. Donner à la lame un léger mouvement de rotation<br />
pendant 20 secondes et lire la réaction sous éclairage<br />
normal sans utiliser de loupe.<br />
LECTURE ET INTERPRETATION DES RESULTATS<br />
Un résultat positif est indiqué par l'apparition dans le<br />
réactif R1 d'une agglutination dans les 30 secondes<br />
(temps du mélange + temps de rotation de la lame) soit<br />
des hématies sensibilisées (coloration rouge), soit des<br />
particules de latex (coloration blanche), ou des deux<br />
simultanément (coloration rouge orangé).<br />
Un résultat négatif est indiqué par l'absence<br />
d'agglutination avec les réactifs R1 et R2.<br />
La réaction est ininterprétable si le réactif R2<br />
présente une agglutination.<br />
L'agglutination des hématies sensibilisées indique la<br />
présence de facteur d'affinité pour le fibrinogène.<br />
L'agglutination du latex indique que la souche possède<br />
la protéine A ou l'antigène contre lequel est dirigé<br />
l'anticorps monoclonal.<br />
Si l'agglutination est très faible avec le réactif anti-<br />
Staphylococcus aureus, si le réactif contrôle est<br />
agglutiné ou si des grumeaux sont obtenus, l'identification<br />
de la souche doit alors être effectuée par la<br />
recherche de la coagulase ou par l'étude des caractères<br />
biochimiques.<br />
CONTROLE DE QUALITE<br />
Un contrôle de qualité des réactifs doit être effectué à<br />
l'ouverture d'un nouveau coffret avec les souches cidessous<br />

Suivre les étapes de la méthodologie et de la lecture à<br />
partir des colonies correspondant aux souches ci-dessus.<br />
Les souches servant au contrôle de qualité doivent être<br />
cultivées sur les milieux habituellement utilisés au<br />
laboratoire.<br />
Après incubation 18-24 heures à 33-37°C, repérer les<br />
colonies suspectes. S'assurer dans un premier temps<br />
qu'il s'agit bien d'un staphylocoque par les réactions<br />
classiques (morphologie, Gram, catalase).<br />

Une détérioration des réactifs doit être suspectée si :<br />
- le réactif R1 n'agglutine pas une souche de<br />
Staphylococcus aureus connue et servant de<br />
référence positive ;<br />
- le réactif R1 agglutine une souche de Staphylococcus<br />
epidermidis connue et servant de référence négative ;<br />
- les réactifs sont autoagglutinés dans les flacons ;<br />
- les réactifs sont contaminés, évaporés ou hémolysés.<br />
Remarque :<br />
Il est de la responsabilité de l'utilisateur de s'assurer que<br />
le contrôle de qualité est mis en œuvre conformément à la<br />
législation locale en vigueur.<br />
LIMITES DU TEST<br />
Certaines souches peuvent être difficiles à émulsionner<br />
et donner des grumeaux ou des filaments dans le<br />
réactif. Ces images ne doivent pas être confondues<br />
avec une agglutination. Néanmoins, une agglutination<br />
peut être visible entre les grumeaux.<br />
Les réactions peuvent être faussement négatives si :<br />
- un nombre insuffisant de colonies est utilisé pour le<br />
test (dans ce cas, il faut effectuer une subculture pour<br />
avoir une croissance suffisante) ;<br />
- la souche ne produit pas le facteur d'affinité pour le<br />
fibrinogène, la protéine A (gélose sang) ;<br />
- l'antigène contre lequel est dirigé l'anticorps<br />
monoclonal est absent.<br />
D'autres tests doivent alors être mis en oeuvre (6).<br />
Des réactions positives peuvent être obtenues avec les<br />
autres espèces de Staphylococcus qui possèdent<br />
habituellement un facteur d'affinité pour le fibrinogène<br />
et/ou une coagulase : S. schleiferi, S. lugdunensis,<br />
S. intermedius, S. delphini, S. hyicus (1, 7).<br />
Des réactions faussement positives sont possibles avec<br />
certaines souches de streptocoques (A, C et G)<br />
possédant des sites récepteurs pour les immunoglobulines<br />
(8) et pour le fibrinogène (9).<br />
Des réactions ininterprétables peuvent être observées<br />
avec certaines souches d'entérocoques et de microcoques.<br />
SLIDEX ® Staph-Kit ne peut pas être utilisé directement<br />
sur hémoculture en raison des interférences possibles<br />
avec le fibrinogène et/ou les immunoglobulines<br />
apportées par le sang.<br />
PERFORMANCES<br />
L'évaluation des réactifs a été menée à partir de souches<br />
fraîchement isolées (centres hospitaliers) et à partir de<br />
souches congelées dans plusieurs centres hospitaliers et<br />
les résultats de l'étude multicentrique ont été publiés (10).<br />
Toutes les souches de Staphylococcus ont été confirmées<br />
par la coloration de Gram, le test de catalase, la<br />
coagulase libre (tube) et éventuellement par identification<br />
biochimique.<br />
Toutes les souches ont été testées pour leur sensibilité à<br />
la méticilline.<br />
Dans cette étude, 940 souches de S. aureus et<br />
441 souches de S. non aureus ont été testées. SLIDEX<br />
Staph-Kit a été également comparé à deux autres réactifs<br />
du commerce. SLIDEX Staph-Kit a présenté la meilleure<br />
sensibilité pour les souches de S. aureus résistant à la<br />
méticilline (en excluant les résultats ininterprétables).<br />
La sensibilité est de 95,4 % pour les souches MRSA et de<br />
100 % pour les souches MSSA :<br />

ELIMINATION DES DECHETS<br />
Les réactifs du coffret contiennent un dérivé mercuriel.<br />
Il incombe à chaque laboratoire de gérer les déchets et<br />
les effluents qu'il produit selon leur nature et leur<br />
dangerosité, et d'en assurer (ou faire assurer) le<br />
traitement et l'élimination selon les réglementations<br />
applicables.<br />

SLIDEX ® Staph-Kit is a rapid latex and red blood cell agglutination test for the identification of Staphylococcus aureus<br />
strains from culture media.<br />
IVD<br />
SUMMARY AND EXPLANATION<br />
Staphylococci represent one of the bacterial species most<br />
frequently encountered in human infections. Skin<br />
infections caused by Staphylococcus aureus are the most<br />
common human staphylococcal infections, but severe<br />
forms (septicemia, endocarditis, meningitis, pneumonia,<br />
osteomyelitis) may be observed in the hospital<br />
environment (1).<br />
For a long time, the coagulase test has been the most<br />
widely accepted method for identifying Staphylococcus<br />
aureus. It is based on the ability of Staphylococcus aureus<br />
strains to produce the extracellular enzyme coagulase<br />
which initiates the clotting of rabbit plasma. This test<br />
enables the differentiation of Staphylococcus aureus from<br />
other species of staphylococci which are mainly<br />
coagulase negative. However, this method requires an<br />
incubation period of 4-24 hours.<br />
Other agglutination methods using red blood cells or latex<br />
particles have also become generally recognized for the<br />
identification of Staphylococcus aureus (1, 2).<br />
Strains of Staphylococcus aureus possess a protein<br />
receptor capable of combining with fibrinogen. This<br />
receptor is found in 96% of strains of human origin (3) and<br />
in most other biotypes. Certain methicillin-resistant strains<br />
do not produce clumping factor (4).<br />
Protein A is present on the surface of most<br />
Staphylococcus aureus strains. It has an affinity for the Fc<br />
fragment of most isotype G immunoglobulins (IgG).<br />
Protein A is produced on all media, but in lesser quantities<br />
on blood agar plates. It is present in approximately 90% of<br />
strains (5). Various proteins or glycoproteins and teichoic<br />
acids are present on the surface of Staphylococcus<br />
aureus. Their antigenic specificities are extremely variable<br />
(1).<br />
The SLIDEX Staph-Kit enables the identification of<br />
Staphylococcus aureus strains from culture media.<br />
PRINCIPLE<br />
The SLIDEX Staph-Kit reagent is composed of:<br />
- stabilized red blood cells (sheep), sensitized with<br />
fibrinogen, and which will agglutinate if the<br />
Staphylococcus aureus strain produces clumping<br />
factor.<br />
- latex particles, sensitized with monoclonal IgG<br />
antibodies, and which will agglutinate if protein A<br />
and/or antigenic proteins are present on the surface of<br />
the Staphylococcus aureus strain.<br />

Sodium azide may react with lead and copper plumbing<br />
to form highly explosive metal salts. To dispose of<br />

Interpretation of test results should be made taking into<br />
consideration the patient's history, the source of the<br />
specimen, colonial and microscopic morphology and, if<br />
necessary, the results of any other tests performed.<br />

Store all reagents at 2-8°C.<br />
Do not freeze the reagents.<br />
The reagents should not be used if they have been<br />
accidentally frozen.<br />
If stored according to the recommended conditions, all<br />
components are stable until the expiration date indicated<br />
on the box label.<br />
SPECIMEN (collection and preparation)<br />
Culture specimens on a selective (Mannitol Salt agar,<br />
Baird Parker agar, Tellurite glycine agar) or non selective<br />
isolation medium [(Columbia sheep blood agar (5%),<br />
Trypcase (trypticase) soy agar with or without sheep<br />
blood (5%), Mueller Hinton agar with or without sheep<br />
blood, CLED agar, BCP agar, CPS ID agar]. Performance<br />
may vary depending on the type of medium used.<br />
An internal study carried out on 191 S. aureus strains<br />
(MRSA and MSSA) and 89 non-aureus staphylococcal<br />
strains shows the same sensitivity regardless of the<br />
medium used (see table below). The study also shows<br />
that the specificity results are not statistically different<br />
from one medium to another (see following table).<br />
However, it is recommended to use the following media:<br />
Columbia Blood Agar, Columbia CNA + blood, TSA, TSA<br />
+ blood, CPS and Baird Parker.<br />

After 18-24 hours of incubation at 33-37°C, suspect<br />
colonies should be tested with the classical methods<br />
of Gram stain, morphology and the catalase test, to<br />
confirm that they may be staphylococci.<br />
INSTRUCTIONS FOR USE<br />
1. Allow reagents to come to room temperature<br />
(18-25°C) before use.<br />
2. Re-suspend the reagents by mixing. Expel any<br />
remaining reagent in the dropper into bottle and mix<br />
again.<br />
3. To a disposable card or a thoroughly cleaned glass<br />
slide, add the following :<br />
- 1 drop of R1 (anti-Staphylococcus aureus reagent);<br />
- 1 drop of R2 (negative control reagent).<br />
Hold the bottles in a vertical position when<br />
dispensing the reagents.<br />
4. To each drop, carefully add 1-2 suspect colonies of<br />
medium size from a non selective agar or 3-6 very<br />
small colonies from a selective agar (e.g. Mannitol Salt<br />
agar). Mix well with a bacteriological loop or a Pasteur<br />
pipette for 10 seconds.<br />
5. Gently rock the slide for 20 seconds and read the<br />
reaction under normal lighting conditions without using<br />
a magnifying glass.<br />
READING AND INTERPRETATION<br />
A positive result is indicated by the development of an<br />
agglutination pattern in the R1 reagent within 30<br />
seconds (time taken to mix the colonies and reagent,<br />
then rotate the slide) of the sensitized red blood cells<br />
(red coloration), of the latex particles (white coloration),<br />
or of both simultaneously (red orange coloration).<br />
A negative result is indicated by the absence of<br />
agglutination with reagents R1 and R2.<br />
The test is uninterpretable if the control reagent (R2)<br />
shows agglutination.<br />
Agglutination of the sensitized red blood cells indicates<br />
the presence of clumping factor.<br />
Agglutination of the latex particles indicates the<br />
presence of protein A or the specific antigen against<br />
which the monoclonal antibody is directed.<br />
In the case of very weak agglutination with the anti-<br />
Staphylococcus aureus reagent, if the control reagent is<br />
agglutinated or if lumping occurs, further identification of<br />
the strain must be performed with the tube coagulase<br />
test or a biochemical range of tests.<br />

Quality control of the reagents must be performed each<br />
time a new kit is opened, using the following strains:<br />

Deterioration of the reagents should be suspected if:<br />
- reagent R1 does not agglutinate a known<br />
Staphylococcus aureus strain which is used as a<br />
positive reference;<br />
- reagent R1 agglutinates a known Staphylococcus<br />
epidermidis strain which is used as a negative<br />
reference;<br />
- the reagents have autoagglutinated in the bottles;<br />
- the reagents are contaminated or hemolyzed, or have<br />
evaporated.<br />

It is the responsibility of the user to perform Quality<br />
Control, taking into consideration any applicable local<br />
regulations.<br />
LIMITATIONS OF THE METHOD<br />
Certain strains may be difficult to emulsify and may<br />
show stringiness or lumps in the reagent. This should<br />
not be confused with true agglutination. Nevertheless,<br />
true agglutination may be visible between the lumps.<br />
False negative reactions will occur if:<br />
- an insufficient number of colonies is used for the test.<br />
In this case, the isolate should be subcultured and<br />
tested when there is sufficient growth;<br />
- the strain does not produce clumping factor or protein<br />
A (blood agar);<br />
- the specific antigen against which the monoclonal<br />
antibody is directed is not produced.<br />
Other tests should be used (6).<br />
Positive reactions may be found with other species of<br />
Staphylococcus which possess clumping factor and/or<br />
coagulase:<br />
S. schleiferi, S. lugdunensis, S. intermedius, S. delphini,<br />
S. hyicus (1, 7).<br />
False positive reactions may be found with certain<br />
strains of streptococci (A, C and G) which possess<br />
receptor sites for immunoglobulins (8) and for fibrinogen<br />
(9).<br />
Uninterpretable reactions may be observed with some<br />
strains of enterococci and micrococci.<br />
The SLIDEX ® Staph-Kit should not be used directly with<br />
blood culture broth. False reactions may take place due<br />
to interference from fibrinogen and IgG present in the<br />
blood.<br />
PERFORMANCE<br />
The evaluation of the reagents was performed with fresh<br />
hospital isolates and strains frozen in several hospitals.<br />
The results of the multicenter study have been published<br />
(10).<br />
All the Staphylococcus strains were confirmed by Gram<br />
stain, catalase test, tube coagulase and discrepancies<br />
resolved by biochemical procedures.<br />
All the strains were tested for their sensitivity to methicillin.<br />
In this study, 940 S. aureus strains and 441 non-aureus<br />
strains were tested. The SLIDEX Staph-Kit was also<br />
compared with two other commercially available reagents.<br />
The SLIDEX Staph-Kit proved to be more sensitive for<br />
methicillin-resistant strains (uninterpretable results were<br />
not taken into account).<br />

IVD<br />
EINFÜHRUNG UND TESTERKLÄRUNG<br />
Staphylokokken gehören zu den am häufigsten isolierten<br />
Bakterien in der Humanmedizin. Zu den häufigsten<br />
Krankheitsbildern von Staphylococcus aureus Infektionen<br />
gehören Hauterkrankungen, im Hospitalbereich kann es<br />
aber auch zu schweren Manifestationen wie Septikämie,<br />
Endokarditis, Meningitis, Pneumonie und Osteomyelitis<br />
kommen (1).<br />
Die Röhrchenkoagulase war lange Zeit der am häufigsten<br />
durchgeführte Test zur Identifizierung von Staphylococcus<br />
aureus. Er beruht auf dem Nachweis des extrazellulären<br />
Enzyms Koagulase von Staphylococcus aureus<br />
Stämmen, welches Kaninchenplasma koaguliert.<br />
Staphylococcus aureus kann auf diese Weise auch von<br />
den anderen, hauptsächlich Koagulase-negativen<br />
Staphylokokken unterschieden werden. Dieses Verfahren<br />
erfordert jedoch eine 4 bis 24-stündige Inkubationszeit.<br />
Andere Methoden, die auf der Agglutination von<br />
Erythrozyten oder Latexpartikel basieren, sind derzeit zur<br />
Identifizierung von Staphylococcus aureus anerkannt<br />
(1, 2).<br />
Staphylococcus aureus besitzt einen Proteinrezeptor für<br />
ein Fibrinogenfragment. Dieser Rezeptor ist bei 96% der<br />
Stämme humanen Ursprungs (3) und den meisten<br />
anderen Biotypen vorhanden. Einige Methicillinresistenten<br />
Stämme besitzen keine Fibrinogenaffinität (4).<br />
Auf der Zelloberfläche von Staphylococcus aureus<br />
befindet sich Protein A mit einer Affinität für das Fc-<br />
Fragment der meisten Immunglobuline der Klasse G<br />
(IgG). Dieses Protein wird auf allen Medien gebildet, auf<br />
Blutagar jedoch in geringerer Menge. Protein A ist bei ca.<br />
90 % der Stämme vorhanden (5). Auf der Zelloberfläche<br />
von Staphylococcus aureus befinden sich Proteine oder<br />
Glykoproteine und Teichonsäuren mit extrem variabler<br />
Antigenspezifität (1).<br />
SLIDEX Staph-Kit ermöglicht die Identifizierung von<br />
Staphylococcus aureus von festen Kulturmedien.<br />
PRINZIP<br />
SLIDEX Staph-Kit enthält:<br />
- stabilisierte Schaferythrozyten, die mit Fibrinogen<br />
sensibilisiert sind und in Anwesenheit eines<br />
Staphylococcus aureus Stammes mit Clumpingfaktor<br />
agglutinieren.<br />
- Latexpartikel, die mit einem monoklonalen Antikörper<br />
(IgG) sensibilisiert sind und in Gegenwart eines<br />
Staphylococcus aureus Stammes, der Protein A<br />

in die Kanalisation sollten die Reagenzien immer mit<br />
reichlich Wasser verdünnt werden.<br />
Bei der Interpretation der Ergebnisse müssen der<br />
klinische Hintergrund, die Probenherkunft, Kolonie- und<br />
mikroskopische Morphologie des Stammes sowie<br />
gegebenenfalls die Ergebnisse anderer Tests<br />
berücksichtigt werden.<br />

PROBEN (Gewinnung und Vorbereitung)<br />
Inkubieren Sie die Probe auf einen Selektivmedium<br />

Schafblut (5 %), Mueller Hinton Agar mit oder ohne<br />
Schafblut, CLED Agar, BCP Agar, CPS ID Agar]: Die<br />
Leistungsdaten können je nach Medium variieren.<br />
In einer internen Studie, die mit 191 S. aureus (MRSA und<br />
MSSA) und 89 non-aureus Stämmen durchgeführt wurde,<br />
wurden unabhängig von den verwendeten Medien<br />
dieselbe Sensitivität errreicht. Die Studie zeigt darüber<br />
hinaus für die verschiedenen Medien keine statistische<br />
Abweichung in der Spezifität. Es ist dennoch<br />
empfehlenswert, folgende Medien zu verwenden:<br />
Columbia Blutagar, Columbia CNA-Agar + Blutzusatz,<br />
TSA, TSA + Blut, CPS und Baird Parker.<br />

Nach 18-24 Stunden Inkubation bei 33-37°C werden die<br />
verdächtigen Kolonien mit klassischen Methoden wie<br />
Morphologie, Gramfärbung, Katalase-Test überprüft,<br />
um den Verdacht auf Staphylokokken zu erhärten.<br />
TESTDURCHFÜHRUNG<br />
1. Die Reagenzien vor Gebrauch auf Raumtemperatur<br />
(18-25°C) bringen.<br />
2. Die Latex-Reagenzien gut aufschütteln. In der Tropfpipette<br />
befindliche Tropfen gut mit dem Fläschcheninhalt<br />
mischen.<br />
3. Geben Sie auf zwei Felder einer Einwegkarte oder<br />
eines sauberen Glasobjektträgers:<br />
- 1 Tropfen R1 (anti-Staphylococcus aureus Reagenz)<br />
- 1 Tropfen R2 (Negativkontrolle).<br />
Halten Sie die Tropffläschchen beim Auftragen der<br />
Reagenzien senkrecht.<br />
4. Geben Sie in jeden Tropfen mit einer Kunststoff-Öse<br />
oder einem Einweg-Rührspatel 1 bis 2 verdächtige,<br />
mittelgroße Kolonien von einem nicht selektiven<br />
Nähragar oder 3 bis 6 sehr kleine Kolonien eines<br />
Selektivagars (z.B. Chapman-Medium). Verreiben Sie<br />
die Kolonien für 10 sec sehr sorgfältig und vollständig<br />
in dem Tropfen.<br />
5. Den Objektträger etwa 20 sec leicht rotierend<br />
bewegen und die Reaktion bei normalen Lichtverhältnissen<br />
ohne Lupe ablesen.<br />
ABLESUNG UND INTERPRETATION<br />
Ein positives Ergebnis liegt vor, wenn in R1 innerhalb<br />
von 30 Sekunden (Dauer des Mischens + Rotation des<br />
Objektträgers) eine Agglutination der sensibilisierten<br />
Erythrozyten (rote Färbung), der Latexpartikel (weiße<br />
Färbung) oder beider Reagenzien (rot/orange Färbung)<br />
sichtbar wird.<br />
Ein negatives Ergebnis wird durch eine fehlende<br />
Agglutination in R1 und R2 angezeigt.<br />
Die Reaktion ist nicht interpretierbar, wenn das<br />
Reagenz R2 agglutiniert.<br />
Eine Agglutination der sensibilisierten Erythrozyten zeigt<br />
die Anwesenheit von Clumpingfaktor an.<br />
Eine Agglutination der Latexpartikel weist auf das<br />
Vorhandensein von Protein A oder spezifischen<br />
Oberflächenantigenen, gegen die der monoklonale<br />
Antikörper auf den Latexpartikeln gerichtet ist, hin.<br />
Bei sehr schwacher Agglutination mit dem anti-<br />
Staphylococcus aureus Reagenz, wenn das<br />
Kontrollreagenz agglutiniert oder bei Klumpenbildung<br />
sollte die Identifizierung des Stammes mit der<br />
Röhrchenkoagulase oder mit biochemischen Methoden<br />
erfolgen.<br />
QUALITÄTSKONTROLLE<br />
Für jede neu angebrochene Packung sollte mit folgenden<br />
Kontrollstämmen eine Qualitätskontrolle der Reagenzien<br />
durchgeführt werden:<br />

laborüblichen Nährmedien angezüchtet werden.<br />

Anzeichen für einen Verfall der Reagenzien sind:<br />
- fehlende Agglutination von R1 mit bekannt positiven<br />
Staphylococcus aureus Referenzstämmen.<br />
- Agglutination von R1 mit bekannt negativen<br />
Staphylococcus epidermidis Referenzstämmen.<br />
- Autoagglutination der Reagenzien in der Flasche.<br />
- Sichtbare Kontamination, Hämolyse oder Verdunstungseffekt.<br />
Anmerkung:<br />
Es liegt in der Verantwortung des Anwenders, die<br />
Qualitätskontrolle in Übereinstimmung mit den jeweils<br />
gültigen Vorschriften durchzuführen.<br />
LIMITIERUNGEN<br />
Einige Stämme sind schwierig zu homogenisieren und<br />
können im Reagenz zu Klumpen- oder Filamentbildung<br />
führen. Dies darf nicht mit einer Agglutination<br />
verwechselt werden. Zwischen den Klümpchen kann<br />
jedoch eine Agglutinationsreaktion sichtbar sein.<br />
Falsch negative Reaktionen können auftreten:<br />
- wenn zu wenig Keimmaterial vorhanden ist (in diesem<br />
Fall muss der Stamm subkultiviert werden, um<br />
genügend Wachstum zu erhalten);<br />
- wenn der Stamm keinen Clumpingfaktor oder kein<br />
Protein A (Blutagar) enthält;<br />
- wenn der Stamm keine Antigene bildet, die von dem<br />
verwendeten monoklonalen Antikörper erkannt<br />
werden.<br />
In diesen Fällen müssen andere Tests durchgeführt<br />
werden (6).<br />
Positive Reaktionen können bei folgenden<br />
Staphylococcus Spezies auftreten, die normalerweise<br />
Clumpingfaktor und/oder Koagulase-positiv sind:<br />
S. schleiferi, S. lugdunensis, S. intermedius , S. delphini,<br />
S. hyicus (1, 7).<br />
Falsch positive Reaktionen werden bei einigen<br />
Streptokokken Stämmen (A, C und G) beschrieben, die<br />
Rezeptoren für Immunglobuline (8) und für Fibrinogen<br />
(9) besitzen.<br />
Nicht interpretierbare Reaktionen können bei einigen<br />
Enterokokken und Mikrokokken Stämmen beobachtet<br />
werden.<br />
SLIDEX ® Staph-Kit kann nicht direkt mit Proben aus<br />
Blutkulturmedien getestet werden (mögliche<br />
Störeinflüsse durch das im Blut vorhandene Fibrinogen<br />
und/oder Immunglobuline).<br />
PERFORMANCE<br />
Die Evaluierung der Reagenzien wurde mit frischen<br />
Isolaten (Krankenhäuser) und Stämmen aus<br />
Stammsammlungen mehrerer Krankenhäuser<br />
durchgeführt. Die Ergebnisse einer Multicenter-Studie<br />
wurden veröffentlicht (10).<br />
Alle Staphylococcus Stämme wurden durch Gramfärbung,<br />
Katalase, freie Koagulase (Röhrchen) und gegebenenfalls<br />
biochemische Identifizierung bestätigt.<br />
Alle Stämme wurden auf Methicillin-Resistenz überprüft.<br />
In dieser Studie wurden 940 Staphylococcus aureus und<br />
441 S. non aureus Stämme getestet. Der SLIDEX Staph-<br />
Kit wurde außerdem mit 2 anderen kommerziellen Tests<br />
verglichen. Der SLIDEX Staph-Kit zeigte eine bessere<br />
Sensitivität für die Methicillin-resistenten S. aureus<br />
Stämme (nicht interpretierbare Ergebnisse wurden nicht<br />
berücksichtigt).<br />
Die Sensitivität beträgt 95,4% für die MRSA (Methicillinresistente<br />
Staphylococcus aureus Stämme) und 100% für<br />
die MSSA (Methicillin-empfindliche Stämme).<br />

SLIDEX ® Staph-Kit es una prueba rápida de aglutinación de partículas de látex y hematíes para la identificación de<br />
cepas de Staphylococcus aureus a partir de los medios de aislamiento.<br />
IVD<br />
INTRODUCCION Y OBJETIVO DE LA PRUEBA<br />
Los estafilococos forman parte del grupo de bacterias<br />
más frecuentemente encontradas en patología<br />
humana. Las infecciones más comunes originadas por<br />
Staphylococcus aureus son cutáneas, pero también<br />
pueden observarse en el medio hospitalario afecciones<br />
más severas (septicemias, endocarditis, meningitis,<br />
pneumonías, osteomielitis) (1).<br />
Durante mucho tiempo el método más utilizado para la<br />
identificación de Staphylococcus aureus fue la prueba<br />
de la coagulasa. Esta prueba está basada en la<br />
capacidad de las cepas de Staphylococcus aureus de<br />
producir esta enzima extra-celular que coagula el<br />
plasma de conejo. Esta determinación permite por<br />
tanto, diferenciar Staphylococcus aureus del resto de<br />
las especies de estafilococos, mayoritariamente<br />
coagulasa negativo. Sin embargo, este método<br />
necesita de 4 a 24 horas de incubación.<br />
Actualmente son reconocidos otros métodos, basados<br />
en la aglutinación de hematíes o partículas de látex,<br />
para la identificación de Staphylococcus aureus (1, 2).<br />
Las cepas de Staphylococcus aureus poseen un<br />
receptor proteico para un fragmento del fibrinógeno.<br />
Este receptor está presente en el 96 % de las cepas de<br />
origen humano (3) y en la mayoría del resto de los<br />
biotipos. Algunas cepas meticilin-resistentes no<br />
presentan este factor de afinidad para el fibrinógeno<br />
(4).<br />
En la superficie del Staphylococcus aureus una<br />
proteína, la proteína A, reacciona selectivamente con el<br />
fragmento Fc de la mayoría de las inmunoglogulinas de<br />
isotipo G (IgG). Esta proteína se produce en todos los<br />
medios pero se produce en menor cantidad en los<br />
medios con sangre. Está presente en<br />
aproximadamente en el 90 % de las cepas (5). Además<br />
existen en la superficie de Staphylococcus aureus,<br />
proteínas o glicoproteínas y ácidos teicoicos cuya<br />
especificidad antigénica son extremadamente variables<br />
(1).<br />
SLIDEX Staph-Kit permite la identificación de las cepas<br />
de Staphylococcus aureus a partir de los medios de<br />
aislamiento.<br />
PRINCIPIO<br />
SLIDEX Staph-Kit es un reactivo constituido por una<br />
mezcla de :<br />
- Hematíes de cordero estabilizados, sensibilizados<br />
con fibrinógeno que aglutinan cuando se ponen en<br />
presencia de una cepa de Staphylococcus aureus<br />
que posea el receptor de afinidad por el fibrinógeno,<br />
- Partículas de látex sensibilizadas con un anticuerpo<br />
monoclonal (IgG) que aglutinan cuando se ponen<br />
en presencia de una cepa de Staphylococcus<br />
aureus que tenga la proteína A y/o los antígenos<br />
proteicos de superficie.<br />

conocido puede garantizar de forma absoluta que<br />
estos productos no contienen ningún agente<br />
patógeno transmisible. Se recomienda<br />
manipularlos con las precauciones de uso<br />
relacionadas con los productos potencialmente<br />
infecciosos (consultar Laboratory biosafety<br />
manual - OMS - Ginebra – última edición).<br />

El certificado del origen y/o del estado sanitario de los<br />
animales no pueden aportar la garantía absoluta que<br />

Los reactivos del equipo contienen un conservante<br />
(azida sódica), susceptible de reaccionar con las<br />
tuberías de plomo o cobre formando azidas metálicas<br />
explosivas. Se recomienda enjuagar abundantemente<br />
con agua cualquier residuo.<br />
La interpretación de los resultados debe hacerse<br />
teniendo en cuentra la historia clínica, el origen de la<br />
muestra, los aspectos macro y microscópicos de la<br />
cepa y eventualmente los resultados de otras<br />
pruebas.<br />
CONDICIONES DE CONSERVACION<br />
Conservar los reactivos a 2-8°C.<br />
No congelar los reactivos.<br />
Si los reactivos se han congelado de forma<br />
accidental, no deben utilizarse.<br />
Todos los componentes son estables hasta la fecha<br />
de caducidad indicada en la etiqueta del envase, si se<br />
conservan en las condiciones recomendadas.<br />
MUESTRAS (recogida de muestra y preparación)<br />
Inocular la muestra en un medio de aislamiento<br />
selectivo (medio Chapman, medio Baird Parker, agar<br />
telurito glicina) o no selectivo [(agar Columbia con<br />
sangre de cordero (5 %), agar Trypcase soja con o sin<br />
sangre de cordero (5 %), medio Mueller Hinton con o<br />
sin sangre de cordero, medio CLED, agar BCP, agar<br />
CPS ID] : las prestaciones técnicas pueden variar en<br />
función del tipo de medio utilizado.<br />
Un estudio interno realizado con 191 cepas de S.<br />
aureus (MRSA y MSSA) y 89 cepas de S. no aureus<br />
muestra una sensibilidad idéntica independientemente<br />
del medio utilizado. El estudio demuestra igualmente<br />
que los resultados de especificidad no son<br />
estadísticamente diferentes en función del medio<br />
utilizado (ver la siguiente tabla). Sin embargo, se<br />
recomienda utilizar los siguientes medios: Medio<br />
Columbia con sangre, Columbia ANC + sangre, TSA,<br />
TSA + sangre, CPS y Baird Parker.<br />

Después de incubar de 18-24 horas a 33-37°C, las<br />
colonias sospechosas deben ser estudiadas con los<br />
métodos clásicos de tinción de Gram, morfología y<br />
la catalasa, para confirmar que se trata de un<br />
estafilococo.<br />
INSTRUCCIONES DE USO<br />
1. Dejar que los reactivos alcancen la temperatura<br />
ambiente (18-25°C) antes de utilizarlos.<br />
2. Poner correctamente en suspensión los reactivos.<br />
Eliminar las burbujas retenidas en el cuentagotas.<br />
3. En una tarjeta desechable o porta de cristal<br />
perfectamente limpio, depositar en 2 círculos<br />
diferentes:<br />
- 1 gota de R1 (reactivo anti-Staphylococcus<br />
aureus) ;<br />
- 1 gota de R2 (reactivo control negativo).<br />
Mantener la posición vertical de los frascos<br />

4. En cada una de las gotas, añadir cuidadosamente<br />
1-2 colonias sospechosas de tamaño medio<br />
aisladas a partir de un medio no selectivo o de 3-6<br />
colonias de tamaño muy pequeño aisladas a partir<br />
de un medio selectivo (ej. Medio Chapman).<br />
Mezclar bien con un asa o con un bastoncillo<br />
desechable.<br />
5. Dar al porta un ligero movimiento de rotación<br />
durante 20 segundos y leer la reacción con luz<br />
normal sin utilizar lupa.<br />
LECTURA E INTERPRETACION DE LOS<br />
RESULTADOS<br />
Un resultado positivo viene indicado por la aparición<br />
en el reactivo R1 de una aglutinación en 30 segundos<br />
(tiempo de realizar la mezcla + tiempo de rotación del<br />
porta) bien en los hematíes sensibilizados (color rojo),<br />
o bien en las partículas de látex (color blanco), o en<br />
ambos simultáneamente (color rojo anaranjado).<br />
Un resultado negativo viene indicado por la<br />
ausencia de aglutinación con los reactivos R1 y R2.<br />
La reacción es ininterpretable si el reactivo R2<br />
presenta una aglutinación.<br />
La aglutinación de los hematíes sensibilizados indica<br />
la presencia del factor de afinidad por el fibrinógeno.<br />
La aglutinación del látex indica que la cepa posee la<br />
proteína A o el antígeno contra el cual está dirigido el<br />
anticuerpo monoclonal.<br />
Si la aglutinación es muy débil con el reactivo anti-<br />
Staphylococcus aureus, si el reactivo control está<br />
aglutinado o si se obtienen grumos, la identificación<br />
de la cepa debe efecturarse por la detección de la<br />
coagulasa o por el estudio de las características<br />
bioquímicas.<br />

CONTROL DE CALIDAD<br />
Cada vez que se abra un nuevo equipo debe<br />
efectuarse un control de calidad de los reactivos con<br />
las siguientes cepas:<br />

Realizar las etapas de metodología y de lectura a partir<br />
de las colonias correspondientes a las cepas citadas<br />
anteriormente. Las cepas que sirven de control de<br />
calidad deben cultivarse en los medios habitualmente<br />
utilizados en el laboratorio.<br />
Se debe sospechar un deterioro de los reactivos si :<br />
- el reactivo R1 no aglutina una cepa de<br />
Staphylococcus aureus conocida y usada como<br />
referencia positiva ;<br />
- el reactivo R1 aglutina una cepa de Staphylococcus<br />
epidermidis conocida y usada como referencia<br />
negativa;<br />
- los reactivos están autoaglutinados en los frascos;<br />
- los reactivos están contaminados, evaporados o<br />
hemolizados.<br />
Advertencia :<br />
Es responsabilidad del usuario garantizar que el control<br />
de calidad se realiza conforme con la normativa local<br />
en vigor.<br />
LIMITES DE LA PRUEBA<br />
Algunas cepas pueden ser difíciles de emulsionar y<br />
formar grumos o filamentos con el reactivo. Estas<br />
imágenes no deben confundirse con una aglutinación.<br />
Sin embargo, puede ser visible una aglutinación entre<br />
los grumos.<br />
Las reacciones pueden dar un resultado falso<br />
negativo si :<br />
- si se utiliza un número insuficiente de colonias para<br />
realizar la prueba (en este caso, debe efectuarse un<br />
subcultivo para obtener un crecimiento suficiente) ;<br />
- la cepa no produce el factor de afinidad para el<br />
fibrinógeno, la proteína A (medio con sangre) ;<br />
- el antígeno contra el cual va dirigido el anticuerpo<br />
monoclonal se encuentra ausente.<br />
Deberían realizarse otras pruebas (6).<br />
Pueden obtenerse reacciones positivas con otras<br />
especies de Staphylococcus que poseen<br />
habitualmente un factor de afinidad por el fibrinógeno<br />
y/o una coagulasa: S. schleiferi, S. lugdunensis,<br />
S. intermedius, S. delphini, S. hyicus (1, 7).<br />
Es posible obtener reacciones falsas positivas con<br />
algunas cepas de estreptococos (A, C y G) que<br />
poseen sitios receptores para las inmunoglogulinas<br />
(8) y para el fibrinógeno (9).<br />
Pueden observarse reacciones no interpretables con<br />
algunas cepas de enterococos y micrococos.<br />
SLIDEX ® Staph-Kit no puede utilizarse con<br />
hemocultivos debido a las posibles interferencias con<br />
el fibrinógeno y/o las inmunoglobulinas incluidas en la<br />
sangre.<br />
PRESTACIONES TECNICAS<br />
La evaluación de los reactivos se ha realizado a partir<br />
de cepas recién aisladas (centros hospitalarios) y a<br />
partir de cepas congeladas en varios centros<br />
hospitalarios y los resultados obtenidos en el estudio<br />
multicéntrico han sido publicados (10).<br />
Todas las cepas de Staphylococcus han sido<br />
confirmadas mediante la tinción de Gram, la prueba de<br />
la catalasa, la coagulasa libre (en tubo) y<br />
eventualmente por identificación bioquímica.<br />
En todas las cepas se estudió su sensibilidad frente a<br />
la meticilina.<br />
En este estudio, se analizaron 940 cepas de S. aureus<br />
y 441 cepas de S. no aureus. SLIDEX Staph-Kit fue<br />
también comparado con otros dos reactivos<br />
comercializados. SLIDEX Staph-Kit presentó la mejor<br />
sensibilidad para las cepas de S. aureus resistente a la<br />
meticilina (excluyendo los resultados no interpretables).<br />
La sensibilidad es del 95,4 % para las cepas MRSA y<br />
del 100 % para las cepas MSSA :<br />

naturaleza y su peligrosidad, y garantizar (o hacer<br />

IVD<br />
INTRODUZIONE E OBIETTIVO DEL TEST<br />
Gli stafilococchi fanno parte dei batteri riscontrati più<br />
frequentemente nelle patologie umane. Le infezioni più<br />
comuni provocate dallo Staphylococcus aureus sono a<br />
livello cutaneo, ma in ambito ospedaliero possono essere<br />
osservate patologie di notevole gravità (setticemie,<br />
endocarditi, meningiti, polmoniti, osteomieliti) (1).<br />
Il metodo più diffuso per l'identificazione dello<br />
Staphylococcus aureus è stato per molto tempo la ricerca<br />
della coagulasi. Questo test si basa sulla capacità dei<br />
ceppi di Staphylococcus aureus di produrre questo<br />
enzima extra-cellulare che coagula il plasma di coniglio.<br />
Questo test permette dunque di differenziare lo<br />
Staphylococcus aureus dalle altre specie di stafilococchi,<br />
che sono principalmente coagulasi negativi. Tuttavia<br />
questo metodo necessita dalle 4 alle 24 ore<br />
d'incubazione.<br />
Attualmente per la ricerca dello Staphylococcus aureus<br />
vengono utilizzate anche altre tecniche, basate<br />
sull’agglutinazione di emazie o di particelle di lattice (1, 2).<br />
In effetti, lo Staphylococcus aureus possiede un recettore<br />
proteico per un frammento del fibrinogeno. Questo<br />
recettore è presente sul 96 % dei ceppi di origine umana<br />
(3) e sulla maggior parte degli altri biotipi. Alcuni ceppi<br />
resistenti alle Penicilline M non presentano questo fattore<br />
di affinità per il fibrinogeno (4).<br />
Sulla superficie dello Staphylococcus aureus una<br />
proteina, la proteina A, reagisce elettivamente con il<br />
frammento Fc della maggior parte delle immunoglobuline<br />
dell'isotipo G (IgG). Questa proteina viene prodotta su tutti<br />
i terreni, ma debolmente su agar al sangue ed è presente<br />
nel 90 % dei ceppi (5). Infine, sulla superficie dello<br />
Staphylococcus aureus sono presenti proteine,<br />
glicoproteine e acidi teicoici le cui specifiche antigeniche<br />
sono estremamente variabili(1)<br />
SLIDEX Staph-Kit permette l’identificazione dei ceppi di<br />
Staphylococcus aureus a partire da terreni di isolamento.<br />
PRINCIPIO<br />
SLIDEX Staph-Kit è un reattivo costituito da una miscela :<br />
- di emazie di montone stabilizzate, sensibilizzate con<br />
fibrinogeno, che agglutinano in presenza di un ceppo<br />
di Staphylococcus aureus portatore del recettore di<br />
affinità per il fibrinogeno,<br />
- di particelle di lattice, sensibilizzate con un anticorpo<br />
monoclonale (IgG), che agglutinano in presenza di un<br />
ceppo di Staphylococcus aureus portatore della<br />
proteina A e/o degli antigeni proteici di superficie.<br />

L'interpretazione dei risultati del test deve essere fatta<br />
tenendo conto del contesto clinico, dell’origine del<br />
prelievo, degli aspetti macro e microscopici ed,<br />
eventualmente, dei risultati di altri test.<br />

Tutti i componenti, se correttamente conservati alle<br />
condizioni prescritte, sono stabili fino alla data di<br />

CAMPIONI (prelievo e preparazione)<br />
Seminare il prelievo su un terreno di isolamento selettivo<br />
(agar Chapman, agar Baird Parker, agar tellurito glicina) o<br />
non selettivo [agar Columbia al sangue di montone (5 %),<br />
agar Tripticasi soia con o senza sangue di montone (5 %),<br />
agar Mueller Hinton con o senza sangue di montone, agar<br />
CLED, agar BCP, agar CPS ID] : le performance possono<br />
variare in funzione del tipo di terreno utilizzato.<br />
Uno studio interno realizzato su 191 ceppi di S. aureus<br />
(MRSA e MSSA) e su 89 ceppi di S. non aureus mostra<br />
una sensibilità identica indipendentemente dal terreno<br />
utilizzato. Lo studio mostra inoltre che i risultati della<br />
specificità non sono statisticamente differenti se vengono<br />
utilizzati terreni di coltura differenti (vedere la tabella sotto<br />
riportata). Tuttavia si raccomanda di utilizzare i seguenti<br />
terreni : Agar Columbia al sangue, Columbia CNA +<br />
sangue, TSA, TSA + sangue, CPS e Baird Parker.<br />

Dopo 18-24 ore di incubazione a 33-37°C, prelevare le<br />
colonie sospette. Assicurarsi che si tratti realmente di<br />
uno stafilococco avvalendosi delle tecniche classiche<br />
(morfologia, Gram, catalasi).<br />
PROCEDIMENTO<br />
1. Lasciare i reattivi a temperatura ambiente (18-25°C)<br />
prima dell’uso.<br />
2. Omogeneizzare accuratamente i reattivi al lattice.<br />
Eliminare le bolle contenute nel conta-gocce.<br />
3. Deporre in 2 campi di reazione di un cartoncino<br />
monouso o di un vetrino porta-oggetto perfettamente<br />
pulito :<br />
- 1 goccia di R1 (reattivo anti-Staphylococcus aureus);<br />
- 1 goccia di R2 (reattivo di controllo negativo).<br />
Tenere i flaconi conta-gocce verticalmente durante<br />
la distribuzione delle gocce.<br />
4. Emulsionare accuratamente e completamente per 10<br />
secondi In ciascuna goccia 1-2 colonie sospette di<br />
media grandezza da un terreno nutritivo o 3-6 colonie<br />
molto piccole da un terreno selettivo (per es.: agar<br />
Chapman) con un’ansa o un bastoncino monouso.<br />
5. Imprimere al cartoncino un leggero movimento<br />
rotatorio per 20 secondi e leggere sotto una luce<br />
normale senza utilizzare lampade.<br />
LETTURA E INTERPRETAZIONE DEI RISULTATI<br />
Un risultato positivo è indicato dalla comparsa nel<br />
reattivo R1 di una agglutinazione entro 30 secondi<br />
(tempo del mescolamento + tempo di rotazione del<br />
vetrino) delle emazie sensibilizzate (colorazione rossa),<br />
delle particelle di lattice (colorazione bianca), o di<br />
entrambi simultaneamente (colorazione rosso arancio).<br />
Un risultato negativo è indicato dall’assenza di<br />
agglutinazione con i reattivi R1 ed R2.<br />
Reazione è ininterpretabile se il reattivo R2 presenta<br />
un’agglutinazione.<br />
L'agglutinazione delle emazie sensibilizzate indica la<br />
presenza del fattore di affinità per il fibrinogeno.<br />
L'agglutinazione del lattice indica che il ceppo possiede<br />
la proteina A o l'antigene contro cui è diretto l'anticorpo<br />
monoclonale.<br />
In caso di agglutinazione estremamente debole con il<br />
reattivo anti-Staphylococcus aureus, di agglutinazione<br />
del reattivo di controllo o di formazione di grumi,<br />
l’identificazione del ceppo deve essere effettuata<br />
mediante la ricerca della coagulasi o con lo studio delle<br />
caratteristiche biochimiche.<br />
CONTROLLO DI QUALITÀ<br />
Un controllo di qualità dei reattivi deve essere effettuato<br />
all’apertura di una nuova confezione con i ceppi seguenti:<br />

E’ possibile che i reattivi siano deteriorati se :<br />
- il reattivo R1 non agglutina più un ceppo di<br />
Staphylococcus aureus conosciuto ed utilizzato come<br />
riferimento positivo ;<br />
- il reattivo R1 agglutina un ceppo de Staphylococcus<br />
epidermidis conosciuto ed utilizzato come riferimento<br />
négativo ;<br />
- i reattivi si sono autoagglutinati nei flaconi ;<br />
- i reattivi sono contaminati, evaporati o emolizzati.<br />

E’ responsabilità dell'utilizzatore assicurare che il controllo<br />
di qualità corrisponda a quanto previsto dalla legislazione<br />
locale vigente.<br />
LIMITI DEL METODO<br />
Alcuni ceppi possono essere difficili da emulsionare e<br />
dare grumi o filamenti nel reattivo. Queste condizioni<br />
non devono essere confuse con un’agglutinazione.<br />
Tuttavia, un’agglutinazione può essere visibile tra i<br />
grumi.<br />
Le reazioni possono risultare falsamente negative se:<br />
- viene utilizzato un numero insufficiente di colonie (in<br />
questo caso, occorre eseguire una subcoltura per<br />
ottenere una crescita sufficiente) ;<br />
- il ceppo non produce il fattore di affinità per il<br />
fibrinogeno e la proteine A (agar al sangue) ;<br />
- l'antigene contro cui è diretto l'anticorpo monoclonale<br />
è assente.<br />
Nei suddetti casi occorre eseguire altri test (6).<br />
Reazioni positive possono essere ottenute con altre<br />
specie di Staphylococcus che possiedono abitualmente<br />
un fattore d'affinità per il fibrinogeno e/o una coagulasi:<br />
S. schleiferi, S. lugdunensis, S. intermedius, S. delphini,<br />
S. hyicus (1, 7).<br />
Reazioni falsamente positive sono possibili con alcuni<br />
ceppi di streptococchi (A, C e G) che possiedono siti<br />
recettori per le immunoglobuline (8) e per il fibrinogeno<br />
(9).<br />
Reazioni non interpretabili possono essere osservate<br />
con alcuni ceppi di enterococchi e di micrococchi.<br />
SLIDEX ® Staph-Kit non può essere utilizzato<br />
direttamente sulle emocolture a causa delle possibili<br />
interferenze con il fibrinogeno e/o le immunoglobuline<br />
presenti nel sangue.<br />
PERFORMANCE<br />
La performance dei reattivi è stata valutata su ceppi isolati<br />
di fresco (centri ospedalieri) e da ceppi congelati di più<br />
centri ospedalieri; i risultati dello studio multicentrico sono<br />
stati pubblicati (10).<br />
Tutti i ceppi di Staphylococcus sono stati confermati con<br />
la colorazione di Gram, con il test della catalasi, con la<br />
coagulasi libera (in provetta) ed, eventualmente, con<br />
l’identificazione biochimica.<br />
Tutti i ceppi sono stati saggiati per la sensibilità alla<br />
meticillina.<br />
In questo studio sono stati esaminati 940 ceppi di S.<br />
aureus e 441 ceppi di S. non aureus. SLIDEX Staph-Kit è<br />
stato inoltre comparato a due altri reattivi del commercio.<br />
SLIDEX Staph-Kit ha presentto la migliore sensibilità con i<br />
ceppi di S. aureus resistenti alla meticillina (escludendo i<br />
risultati non interpretabili).<br />
La sensibilità è del 95,4 % per i ceppi MRSA e del 100 %<br />
per i ceppi MSSA :<br />

I reattivi della confezione contengono un derivato<br />
mercuriale.<br />
E’ responsabilità di ogni laboratorio gestire i rifiuti e gli<br />
effluenti prodotti a seconda della loro natura e della loro<br />
pericolosità ed assicurarne (o farne assicurare) il<br />
trattamento e lo smaltimento conformemente alla<br />

SLIDEX ® Staph-Kit é um teste rápido de aglutinação de partículas de látex e de hemácias para a identificação de<br />
estirpes/cepas de Staphylococcus aureus a partir de meios de isolamento.<br />
IVD<br />
INTRODUÇÃO E OBJECTIVO DO TESTE<br />
Os estafilococos fazem parte das bactérias que se<br />
encontram com mais frequência em patologia humana.<br />
As infecções mais comuns provocadas por<br />
Staphylococcus aureus são cutâneas, mas podem<br />
também observar-se infecções severas (septicémias,<br />
endocardites, meningites, pneumonias, osteomielites) em<br />
meio hospitalar (1).<br />
O método que foi durante mais tempo escolhido para a<br />
identificação de Staphylococcus aureus foi a pesquisa da<br />
coagulase. Este teste baseia-se na capacidade das<br />
estirpes/cepas de Staphylococcus aureus produzirem<br />
esta enzima extra-ceclular que coagula o plasma de<br />
coelho. Este teste permite assim diferenciar<br />
Staphylococcus aureus das outras espécies de<br />
estafilococos, maioritariamente, coagulase negativa. No<br />
entanto, este método necessita de 4 a 24 horas de<br />
incubação.<br />
Outros métodos, baseados na aglutinação de hemácias<br />
ou de partículas de látex, são actualmente reconhecidos<br />
para a identificação de Staphylococcus aureus (1, 2).<br />
Com efeito, Staphylococcus aureus possui um receptor<br />
proteico para um fragmento do fibrinogénio. Este receptor<br />
apresenta-se em 96% das estirpes/cepas de origem<br />
humana (3) e, na maioria dos outros biotipos. Algumas<br />
estirpes/cepas resistentes às Penicilinas M não<br />
apresentam este factor de afinidade para o fibrinogénio<br />
(4).<br />
Uma proteína de superfície do Staphylococcus aureus, a<br />
proteína A, reage electivamente com o fragmento Fc da<br />
maioria das imunoglobulinas de isotipo G (IgG). Esta<br />
proteína é produzida em todos os meios, mas<br />
diminutamente na gelose de sangue. Está presente em<br />
cerca de 90% das estirpes/cepas (5). Enfim, as proteínas<br />
ou glicoproteínas e os ácidos teicóicos cujas<br />
especificidades antigénicas são extremamente variáveis<br />
(1) existem na superfície de Staphylococcus aureus.<br />
O SLIDEX Staph-Kit permite a identificação das<br />
estirpes/cepas de Staphylococcus aureus a partir de<br />
meios de isolamento.<br />
PRINCÍPIO<br />
SLIDEX Staph-Kit é um reagente constituído por uma<br />
mistura:<br />
- Hemácias de carneiro estabilizadas, sensibilizadas<br />
por fibrinogénio e aglutinadas quando colocadas na<br />
presença de uma estirpe/cepa de Staphylococcus<br />
aureus que contém o receptor de afinidade para o<br />
fibrinogénio,<br />
- Partículas de látex sensibilizadas por um anticorpo<br />
monoclonal (IgG) e aglutinadas quando colocadas na<br />
presença de uma estirpe/cepa de Staphylococcus<br />
aureus que contém a proteína A e/ou antigénios<br />
proteicos de superfície.<br />

Evitar o contacto com os olhos, a pele e a roupa.<br />
Não trocar ou misturar reagentes provenientes de<br />
números de lotes diferentes.<br />
Não pipetar com a boca os reagentes.<br />
Não utilizar ansa metálica para efectuar o teste de<br />
aglutinação em látex.<br />
Não voltar a usar cartas descartáveis.<br />
Os reagentes da embalagem contêm um conservante<br />
(azida sódica) susceptível de reagir com as<br />
canalizações de chumbo ou de cobre e formar azidas<br />
metálicas explosivas. É recomendado passar por água<br />
qualquer produto de rejeição.<br />
A interpretação dos resultados do teste deve ser<br />
efectuada tendo em conta o contexto clínico, a origem<br />
da amostra, os aspectos macro e microscópicos, e<br />
eventualmente, os resultados de outros testes.<br />
CONDIÇÕES DE ARMAZENAMENTO<br />
Conservar os reagentes a 2° - 8° C.<br />
Não congelar os reagentes.<br />
Se os reagentes tiverem sido congelados<br />
acidentalmente, não devem ser utilizados.<br />
Todos os componentes permanecem estáveis até à<br />
data de validade indicada na etiqueta da embalagem,<br />
se forem conservados nas condições exigidas.<br />
AMOSTRAS (colheitas/coletas e preparação)<br />
Semear a colheita/coleta num meio de isolamento<br />
selectivo (gelose Chapman, gelose Baird Parker, gelose<br />
telurito glicina) ou não selectivo [(gelose Columbia com<br />
sangue de carneiro (5 %), gelose Tripcase soja com ou<br />
sem sangue de carneiro (5 %), gelose Mueller Hinton com<br />
ou sem sangue de carneiro, gelose CLED, gelose BCP,<br />
gelose CPS ID] : o comportamento funcional pode variar<br />
em função do tipo de meio utilizado.<br />
Um estudo interno efectuado com 191 estirpes/cepas de<br />
S. aureus (MRSA e MSSA) e 89 estirpes/cepas de S. não<br />
aureus demonstra uma sensibilidade identica em função<br />
do meio utilizado. O estudo demonstra também que os<br />
resultados de especificidade não são estatisticamente<br />
diferentes em função do meio utilizado (consultar o<br />
quadro abaixo). No entanto, é recomendado utilizar os<br />
seguintes meios: Gelose Columbia com sangue,<br />
Columbia ANC + sangue, TSA, TSA + sangue, CPS e<br />
Baird Parker.<br />

Após incubação durante 18-24 horas a 33-37°C, verificar<br />
as colónias suspeitas. Assegurar-se primeiro que se<br />
trata de um estafilococo pelas reacções clássicas<br />
(morfologia, Gram, catalase).<br />
PROCEDIMENTO<br />
1. Deixar os reagentes à temperatura ambiente (18-<br />
25°C) antes de utilização.<br />
2. Efectuar correctamente a suspensão dos reagentes.<br />
Eliminar as bolhas de ar retidas no conta-gotas.<br />
3. Numa carta descartável ou lâmina de vidro limpa,<br />
colocar nos 2 círculos:<br />
- 1 gota de R1 (reagente anti-Staphylococcus aureus);<br />
- 1 gota de R2 (reagente de controlo negativo).<br />
Ter o cuidado de segurar os frascos com contagotas<br />
verticalmente quando dispensar as gotas.<br />
4. Com uma ansa ou um bastonete descartável,<br />
homogeneizar cuidaosa e completamente durante 10<br />
segundos, 1 a 2 colónias suspeitas de tamanho médio<br />
provenientes de uma gelose não selectiva ou 3 a 6<br />
colónias muito pequenas provenientes de uma gelose<br />
selectiva (ex.: gelose Chapman).<br />
5. Girar a lâmina com um ligeiro movimento rotativo<br />
durante 20 segundos e ler a reacção sob luz normal<br />
sem utilizar lupa.<br />
LEITURA E INTERPRETAÇÃO DOS RESULTADOS<br />
Um resultado positivo é indicado pelo aparecimento<br />
no reagente R1 de uma aglutinação em 30 segundos<br />
(tempo da mistura + tempo de rotação da lâmina),<br />
através das hemácias sensibilizadas (coloração<br />
vermelha) ou através das partículas de látex (coloração<br />
branca), ou pelas duas em simultâneo (coloração<br />
vermelha alaranjada).<br />
Um resultado negativo é indicado pela ausência de<br />
aglutinação com os reagentes R1 e R2.<br />
A reacção é ininterpretável se o reagente R2<br />
apresentar uma aglutinação.<br />
A aglutinação das hemácias sensibilizadas indica a<br />
presença do factor de afinidade para o fibrinogénio.<br />
A aglutinação do látex indica que a estirpe/cepa indica<br />
que a estirpe/cepa possui proteína A ou antigénio<br />
contra o qual está dirigido o anticorpo monoclonal.<br />
Se a aglutinação for muito fraca com o reagente anti-<br />
Staphylococcus aureus, se o reagente de controlo for<br />
aglutinado ou se obtiver grumos, a identificação da<br />
estirpe/cepa deve ser efectuada pela pesquisa da<br />
coagulase ou pelo estudo dos caracteres bioquímicos.<br />
CONTROLO DE QUALIDADE<br />
Deve ser efectuado um controlo de qualidade dos<br />
reagentes em cada abertura de uma nova embalagem<br />
com as estirpes/cepas abaixo indicadas :<br />

Seguir as etapas da metodologia e da leitura a partir de<br />
colónias que correspondem às estirpes/cepas acima<br />
indicadas. As estirpes/cepas que servem de controlo de<br />
qualidade devem ser cultivadas nos meios habitualmente<br />
utilizados no laboratório.<br />

Deve ser suspeita a deterioração dos reagentes se:<br />
- O reagente R1 não aglutinar uma estirpe/cepa de<br />
Staphylococcus aureus conhecida e que sirva de<br />
referência positiva;<br />
- O reagente R1 aglutinar uma estirpe/cepa de<br />
Staphylococcus epidermidis conhecida e que sirva de<br />
referência negativa;<br />
- Os reagentes estiverem auto-aglutinados nos frascos;<br />
- Os reagentes estiverem contaminados, evaporados<br />
ou hemolisados.<br />
Nota:<br />
É da responsabilidade do utilizador assegurar que o<br />
controlo de qualidade é efectuado em conformidade com<br />
a legislação local em vigor.<br />
LIMITES DO TESTE<br />
Algumas estirpes/cepas podem ser difíceis de<br />
emulsionar e dar grumos ou filamentos no reagente.<br />
Estas imagens não devem ser confundidas com uma<br />
aglutinação. No entanto, uma aglutinação pode ser<br />
visível entre os grumos.<br />
As reacções podem ser falsamente negativas se:<br />
- For utilizado um número insuficiente de colónias para<br />
o teste (neste caso, é necessário efectuar uma<br />
subcultura para obter um crescimento suficiente);<br />
- A estirpe/cepa não produz o factor de afinidade para o<br />
fibrinogénio, a proteína A (gelose de sangue) ;<br />
O antigénio contra o qual é dirigido o anticorpo<br />
monoclonal está ausente.<br />
Devem então ser efectuados outros testes (6).<br />
Podem ser obtidas reacções positivas com outras<br />
espécies de Staphylococcus que possuem<br />
habitualmente um factor de afinidade para o fibrinogénio<br />
e/ou uma coagulase: S. schleiferi, S. lugdunensis,<br />
S. intermedius, S. delphini, S. hyicus (1, 7).<br />
São possíveis algumas reacções falsamente positivas<br />
com algumas estirpes/cepas de estreptococos (A, C e<br />
G) que possuem locais receptores para as<br />
imunoglobulinas (8) e para o fibrinogénio (9).<br />
Podem ser observadas reacções ininterpretáveis com<br />
algumas estirpes/cepas de enterococos e de<br />
micrococos.<br />
O SLIDEX ® Staph-Kit não pode ser utilizado<br />
directamente em hemocultura devido a possíveis<br />
interferências com o fibrinogénio e/ou as<br />
imunoglobulinas fornecidas pelo sangue.<br />
COMPORTAMENTO FUNCIONAL<br />
A avaliação dos reagentes foi efectuada com<br />
estirpes/cepas recentemente isoladas (centros<br />
hospitalares) e com estirpes/cepas congeladas em vários<br />
centros hospitalares e os resultados do estudo<br />
multicêntrico foram publicados (10).<br />
Todas as estirpes/cepas de Staphylococcus foram<br />
confirmadas por coloração de Gram, teste de catalase,<br />
coagulase livre (tubo) e, eventualmente, por identificação<br />
bioquímica.<br />
Todas as estirpes/cepas foram testadas para a sua<br />
sensibilidade à meticilina.<br />
Neste estudo, 940 estirpes/cepas de S. aureus e<br />
441 estirpes/cepas de S. não aureus foram testadas. O<br />
SLIDEX Staph-Kit foi também comparado com outros dois<br />
reagentes comercializados. O SLIDEX Staph-Kit<br />
apresentou a melhor sensibilidade para as estirpes/cepas<br />
de S. aureus resistentes à meticilina (excluindo os<br />
resultados ininterpretáveis).<br />
A sensibilidade é de 95,4% para as estirpes/cepas MRSA<br />

SLIDEX ® Staph Kit är ett snabbt latex- och erytrocytagglutinationstest för identifiering av Staphylococcus aureusstammar<br />
som isolerats från odlingsmedia.<br />
IVD<br />
SAMMANFATTNING OCH FÖRKLARING<br />
Stafylokocker är en av de bakteriearter som oftast<br />
påträffas vid humaninfektioner. De vanligaste<br />
humaninfektionerna som orsakas av Staphylococcus<br />
aureus är hudinfektioner, men allvarliga former (septikemi,<br />
endokardit, meningit, pneumoni, osteomyelit) kan iakttas i<br />
sjukhusmiljö (1).<br />
Koagulastest har länge varit den mest accepterade<br />
metoden för att identifiera Staphylococcus aureus. Det<br />
bygger på Staphylococcus aureus-stammarnas förmåga<br />
att producera det extracellulära enzymet koagulas, som<br />
initierar koagulationen av kaninplasma. Detta test<br />
möjliggör differentiering av Staphylococcus aureusstammar<br />
från andra arter av stafylokocker, vilka<br />
huvudsakligen är koagulasnegativa. Denna metod kräver<br />
emellertid en inkubationsperiod på 4-24 timmar.<br />
Andra agglutinationsmetoder som använder sig av röda<br />
blodkroppar eller latexpartiklar har också blivit allmänt<br />
erkända för identifieringen av Staphylococcus aureus<br />
(1, 2).<br />
Staphylococcus aureus-stammar har en proteinreceptor<br />
som kan binda till fibrinogen. Denna receptor finns i 96%<br />
av stammarna av humant ursprung (3) och de flesta andra<br />
biotyper. Vissa meticillinresistenta stammar producerar<br />
inte ”clumping factor” (4).<br />
Protein A finns på ytan av de flesta Staphylococcus<br />
aureus-stammar. Det har affinitet för Fc-fragmentet i de<br />
flesta isotyp-G-immunoglobuliner (IgG). Protein A<br />
produceras på alla medier, men i mindre mängd på<br />
blodagarplattor. Proteinet finns i ungefär 90% av<br />
stammarna (5). Olika proteiner eller glykoproteiner och<br />
teichonsyror finns på ytan av Staphylococcus aureus.<br />
Deras antigena specificitet varierar kraftigt (1).<br />
SLIDEX Staph-Kitet möjliggör identifiering av<br />
Staphylococcus aureus-stammar från odlingsmedia.<br />
METOD<br />
SLIDEX Staph-Kit-reagenset består av:<br />
- stabiliserade erytrocyter (får), sensibiliserade med<br />
fibrinogen, som agglutinerar om Staphylococcus<br />
aureus-stammen bildar ”clumping factor”.<br />
- latexpartiklar, sensibiliserade med monoklonala IgGantikroppar,<br />
som agglutinerar om protein A och/eller<br />
antigena proteiner finns på ytan av Staphylococcus<br />
aureus- stammen.<br />

hälsotillståndet hos djuren garanterar inte total frånvaro<br />
av överförbara patogena agens. Det rekommenderas<br />
därför att dessa produkter behandlas som potentiellt<br />
infektiösa och handhas med sedvanliga<br />
försiktighetsåtgärder (får inte förtäras eller inandas).<br />
Alla prover, odlingar av mikroorganismer och<br />
inokulerade produkter skall anses infektiösa och<br />
behandlas på ett lämpligt sätt. Sterilteknik och<br />
sedvanliga försiktighetsåtgärder för att handha den<br />
speciella gruppen av bakterier skall iakttas under hela<br />

Använd inga reagenser efter sista förbrukningsdatum<br />
som anges på förpackningsetiketten .<br />
Undvik kontakt med ögon, hud eller klädesplagg.<br />
Blanda inte reagenser från olika batcher.<br />
Munpipettera inte reagenserna.<br />
Använd inte metallögla för att utföra latexagglutinationstestet.<br />
Återanvänd inte engångskartorna.<br />
Reagenserna innehåller ett konserveringsmedel<br />
(natriumazid). Natriumazid kan reagera med bly- och<br />
kopparrörsystem och bilda mycket explosiva<br />
metallsalter. För att göra sig av med natriumaziden<br />
spolas med stora volymer vatten för att förhindra<br />
ackumulering i avloppsrören.<br />
Tolkningen av testresultaten skall göras med hänsyn till<br />
patientens anamnes, provkällan, kolonimorfologi och<br />
mikroskopisk morfologi och, om nödvändigt, resultat av<br />
andra utförda tester.<br />
FÖRVARING<br />
Förvara alla reagenser vid 2-8°C.<br />
Frys inte reagenserna.<br />
Reagenserna bör inte användas om de har blivit<br />
oavsiktligt nedfrysta.<br />
Vid förvaring enligt rekommendationerna, är alla<br />
komponenter stabila till det utgångsdatum som anges<br />
på etiketten.<br />
PROVER (insamling och beredning)<br />
Odla prover på ett selektivt (Chapman agar, Baird Parker<br />
agar, Tellurite glycinagar) eller icke-selektivt<br />
isoleringsmedium [(Columbia fårblodsagar (5%), Trypcase<br />
Soy agar med eller utan fårblod (5%), Mueller Hinton agar<br />
med eller utan fårblod, CLED-agar, BCP-agar, CPS IDagar)].<br />
Prestanda kan variera beroende på typen av<br />
medium som används.<br />
En intern studie som utfördes med 191 S. aureusstammar<br />
(MRSA och MSSA) och 89 icke-aureus-stammar<br />
visade samma känslighet oavsett vilket medium som<br />
användes (se tabellen nedan). Studien visade också att<br />
resultaten för specificiteten inte statistiskt skiljer sig för<br />
olika medier (se tabellen nedan). Det rekommenderas<br />
emellertid att använda följande medier: Columbia<br />
blodagar, Columbia CNA + blod, TSA, TSA + blod, CPS<br />
och Baird Parker.<br />

BRUKSANVISNING<br />
1. Låt reagenserna anta rumstemperatur (18-25°C) före<br />
användning.<br />
2. Återupplös reagenserna genom blandning. Avlägsna<br />
de kvarvarande bubblorna i droppflaskans pipett.<br />
3. Tillsätt följande till en engångskarta eller ett noggrant<br />
rengjort objektglas:<br />
- 1 droppe R1 (anti-Staphylococcus aureus-reagens),<br />
- 1 droppe R2 (negativt kontrollreagens).<br />
Håll flaskorna vertikalt när reagenserna<br />
dispenseras.<br />
4. För varje droppe, tillsätt försiktigt 1-2 misstänkta<br />
kolonier av mellanstorlek från en icke-selektiv agar<br />
eller 3-6 mycket små kolonier från en selektiv agar<br />
(t.ex. Mannitolsalt agar). Blanda väl med en<br />
bakteriologisk ögla eller en pasteurpipett i 10<br />
sekunder.<br />
5. Rotera objektglaset försiktigt i 20 sekunder och avläs<br />
reaktionerna under normala ljusförhållanden och utan<br />
hjälp av förstoringsglas.<br />
AVLÄSNING OCH TOLKNING<br />
Ett positivt resultat påvisas av att ett<br />
agglutinationsmönster bildas i R1-reagenset inom 30<br />
sekunder (tid för att blanda kolonier och reagenser,<br />
sedan rotering av objektglasen) av de sensibiliserade<br />
erytrocyterna (röd färgning), av latexpartiklarna (vit<br />
färgning), eller av båda samtidigt (rödorange färgning).<br />
Ett negativt resultat påvisas av frånvaron av<br />
agglutination med reagensen R1 och R2.<br />
Testet är icke tolkningsbart om kontrollreagenset (R2)<br />
visar agglutination.<br />
Agglutination av de sensibiliserade erytrocyterna påvisar<br />
förekomsten av ”clumping factor”.<br />
Agglutination av latexpartiklarna påvisar förekomsten av<br />
protein A eller det specifika antigen som den<br />
monoklonala antikroppen är riktad mot.<br />
I fall med mycket svag agglutination med anti-<br />
Staphylococcus aureus-reagenset, om kontrollreagenset<br />
agglutineras eller om klumpbildning inträffar, måste<br />
vidare identifiering av stammen utföras med<br />
rörkoagulastestet eller olika biokemiska tester.<br />
KVALITETSKONTROLL<br />
Kvalitetskontroll av reagenserna måste utföras med<br />
följande stammar varje gång ett nytt kit tas i bruk:<br />

Följ stegen som beskrivs i avsnitten “Bruksanvisning” och<br />
“Avläsning och tolkning” med stammar från kolonierna<br />
angivna ovan. Kvalitetskontrollstammarna bör växa på det<br />
medium som normalt sett används i laboratoriet.<br />
Efter 18-24 timmars inkubation vid 33-37°C ska<br />
misstänkta kolonier testas med hjälp av klassiska<br />
metoder som Gramfärgning, morfologi och<br />
katalastest för att säkerställa att de är stafylokocker.<br />

Försämring av reagenserna ska misstänkas om:<br />
- R1-reagenset inte agglutinerar en känd<br />
Staphylococcus aureus-stam, vilken används som<br />
positiv referens,<br />
- R1-reagenset agglutinerar en känd Staphylococcus<br />
epidermidis-stam, vilken används som negativ<br />
referens,<br />
- reagenserna har autoagglutinerat i flaskorna,<br />
- reagenserna är förorenade eller hemolyserade, eller<br />
har avdustat.<br />
Obs:<br />
Det är användarens ansvar att utföra kvalitetskontroll i<br />
enlighet med lokalt tillämpliga bestämmelser.<br />
METODENS BEGRÄNSNINGAR<br />
Vissa stammar kan vara svåra att suspendera och kan<br />
ge stråkighet eller klumpar i reagenset. Detta bör inte<br />
uppfattas som äkta agglutination. Äkta agglutination kan<br />
däremot vara synlig mellan klumparna.<br />
Falskt negativa reaktioner kan inträffa om:<br />
- ett otillräckligt antal kolonier används för testet. I detta<br />
fall ska isolatet subodlas och testas när tillväxten är<br />
tillräcklig.<br />
- stammen bildar inte ”clumping factor” eller protein A<br />
(blodagar),<br />
- det specifika antigen som den monoklonala<br />
antikroppen är riktad mot bildas inte.<br />
Andra tester bör användas (6).<br />
Positiva reaktioner kan inträffa med stammar av andra<br />
arter av Staphylococcus, vilka har ”clumping factor”<br />
och/eller koagulas:<br />
S. schleiferi, S. lugdunensis, S. intermedius, S. delphini,<br />
S. hyicus (1, 7).<br />
Falskt positiva reaktioner kan inträffa med vissa<br />
streptokockstammar (A, C och G) som har receptorer för<br />
immunoglobuliner (8) och fibrinogen (9).<br />
Icke tolkningsbara reaktioner kan inträffa med vissa<br />
stammar av enterokocker och mikrokocker.<br />
SLIDEX ® Staph-Kit bör inte användas direkt med<br />
blododlingsbuljong. Falska reaktioner kan inträffa på<br />
grund av interferens med fibrinogen och IgG som finns i<br />
blodet.<br />
PRESTANDA<br />
Utvärderingen av reagenserna utfördes med färskt isolat<br />
från sjukhus och nedfrysta stammar från flera sjukhus.<br />
Resultaten av multicenterstudien har publicerats (10).<br />
Alla Staphylococcus-stammar konfirmerades med<br />
Gramfärgning, katalastest, rörkoagulas och avvikelser<br />
klargjordes med biokemiska procedurer.<br />
Alla stammarnas känslighet för meticillin testades.<br />
I denna studie testades 940 S. aureus-stammar och 441<br />
icke-aureus-stammar. SLIDEX Staph-Kit jämfördes också<br />
med två andra kommersiellt tillgängliga reagenser.<br />
SLIDEX Staph-Kit visade sig vara mer känsligt för<br />
meticillinresistenta stammar (icke-tolkningsbara resultat<br />
ignorerades).<br />
Känsligheten visade sig vara 95,4% för MRSA-stammar<br />
och 100% för MSSA-stammar.<br />

WPROWADZENIE<br />
Gronkowce nale do jednych z najczciej spotykanych<br />
w infekcjach czowieka gatunków bakteryjnych. Zakaenia<br />
najbardziej powszechne, wywoywane przez<br />
Staphylococcus aureus dotycz skóry, ale w rodowisku<br />
szpitalnym mona obserwowa cikie schorzenia<br />
(posocznica, zapalenie wsierdzia, opon mózgowordzeniowych,<br />
puc, koci) (1).<br />
Przez dugi okres czasu, najbardziej rozpowszechnion<br />
metod identyfikacji Staphylococcus aureus by test na<br />
wytwarzanie koagulazy. Jest on oparty na zdolnoci<br />
szczepów Staphylococcus aureus do wytwarzania<br />
zewntrzkomórkowego enzymu koagulazy, która inicjuje<br />
wytwarzanie skrzepu w osoczu króliczym. Test ten<br />
pozwala na odrónienie Staphylococcus aureus od innych<br />
gatunków gronkowców, które s na ogó koagulazoujemne.<br />
Metoda ta wymaga jednak 4-24 godzin inkubacji.<br />
Inne techniki aglutynacyjne wykorzystujce krwinki<br />
czerwone lub czsteczki lateksu s powszechnie<br />
uznawanymi metodami identyfikacji Staphylococcus<br />
aureus (1, 2).<br />
Szczepy Staphylococcus aureus posiadaj biakowy<br />
receptor zdolny do wizania z fibrynogenem. Receptor ten<br />
wykrywa si u 96% szczepów pochodzcych od ludzi (3)<br />
i w wikszoci innych biotypów. Niektóre szczepy<br />
meticylino-oporne nie wytwarzaj clumping factor (4).<br />
Biako A wystpuje na powierzchni wikszoci szczepów<br />
Staphylococcus aureus. Wykazuje ono powinowactwo do<br />
fragmentu Fc wikszoci izotypów immunoglobulin<br />
klasy G (IgG). Biako A jest wytwarzane na wszystkich<br />
podoach, ale w mniejszej iloci na pytkach z agarem<br />
krwawym. Wystpuje ono u okoo 90% szczepów (5). Na<br />
powierzchni Staphylococcus aureus obecne s róne<br />
biaka lub glikoproteiny i kwas tejchojowy. Ich<br />
specyficzno antygenowa bardzo si róni (1).<br />
Test SLIDEX Staph-Kit suy do identyfikacji szczepów<br />
Staphylococcus aureus po wyhodowaniu na podoach.<br />
ZASADA DZIAANIA<br />
Odczynnik SLIDEX Staph-Kit zawiera:<br />
- stabilizowane krwinki czerwone (owcze), uczulone<br />
fibrynogenem, które aglutynuj, jeli szczep<br />
Staphylococcus aureus wytwarza clumping factor.<br />
- czsteczki lateksu, uczulone przeciwciaami<br />
monoklonalnymi IgG, które aglutynuj, jeli na<br />
powierzchni szczepu Staphylococcus aureus<br />
wystpuje biako A i/lub biaka antygenowe.<br />

MATERIA DO BADA (pobieranie i opracowanie)<br />
Materia badany mona hodowa na podoach<br />
wybiórczych (agar Chapmana, Baird-Parkera, agar<br />
z telurynem i glicyn) lub niewybiórczych [(agar Columbia<br />
z krwi barani (5%), agar tryptozowo-sojowy z lub bez<br />
krwi baraniej (5%), agar Mueller-Hintona z lub bez krwi<br />

Przeprowadzona wewntrzna ocena na 191 szczepach<br />
S. aureus (MRSA i MSSA) oraz 89 szczepach innych ni<br />
S.aureus wykazaa tak sam czuo niezalenie od<br />
podoa (patrz tabela poniej). Badania specyficznoci nie<br />
wykazay równie znamiennych statystycznie rónic<br />
zalenych od uytego podoa (patrz tabela<br />
poniej).Jednake zaleca si uywanie nastpujcych<br />
podó: agar Columbia z krwi, Columbia CNA + krew,<br />
TSA, TSA + krew, CPS i Baird Parker.<br />

SPOSÓB WYKONANIA<br />
1. Przed uyciem doprowadzi odczynniki do<br />
temperatury pokojowej (18-25°C).<br />
2. Wymiesza dokadnie odczynniki. Usun pcherzyki<br />
z kroplomierza.<br />
3. Na jednorazow kart lub czyste szkieko<br />
podstawowe, nanie:<br />
-1 kropl R1 (odczynnik do wykrywania<br />
Staphylococcus aureus);<br />
-1 kropl R2 (ujemna kontrola).<br />
Nakrapiajc odczynnik trzyma butelk w pozycji<br />
pionowej.<br />
4. Do kadej kropli doda ostronie 1-2 podejrzanych<br />
kolonii o redniej wielkoci z agaru niewybiórczego lub<br />
3-6 bardzo maych kolonii z podoa wybiórczego (np.<br />
agar Champana). Miesza dokadnie ez<br />
bakteriologiczn lub pipet pasteurowask przez<br />
10 sekund.<br />
5. Przez 20 sekund delikatnie obraca kart i odczyta<br />
reakcj w normalnym wietle, bez uywania szka<br />
powikszajcego.<br />
ODCZYT I INTERPRETACJA<br />
Wynik dodatni to aglutynacja z odczynnikiem R1 po 30<br />
sekundach (czas przeznaczony na wymieszanie kolonii<br />
i odczynnika oraz obracania kart) uczulonych krwinek<br />
czerwonych (kolor czerwony), czsteczek lateksu (kolor<br />
biay), lub obu jednoczenie (kolor czerwonopomaraczowy).<br />
Wynik ujemny to brak aglutynacji z odczynnikami R1<br />
i R2.<br />
Test nie do zinterpretowania jeli wystpi aglutynacja<br />
z odczynnikiem kontrolnym (R2).<br />
Aglutynacja uczulonych krwinek czerwonych wskazuje<br />
na obecno clumping factor.<br />
Aglutynacja uczulonych czsteczek lateksu wskazuje na<br />
obecno biaka A lub specyficznych antygenów,<br />
przeciwko którym s skierowane przeciwciaa<br />
monoklonalne.<br />
W przypadku bardzo sabej aglutynacji z odczynnikiem<br />
do wykrywania Staphylococcus aureus, lub jeli<br />
zachodzi aglutynacja z odczynnikiem kontrolnym, lub<br />
powstan grudki, dalsz identyfikacj szczepu naley<br />
przeprowadzi testem na koagulaz probówkow lub<br />
testami biochemicznymi.<br />
KONTROLA JAKOCI<br />
Zawsze przy otwieraniu nowego zestawu, naley<br />
przeprowadza kontrol jakoci, uywajc nastpujcych<br />

si za wzorcowy dla wyniku ujemnego;<br />
- wystpuje autoaglutynacja odczynników w butelce;<br />
- odczynniki s zanieczyszczone, shemolizowane, lub<br />
wyschnite.<br />
Uwaga:<br />
Obowizkiem uytkownika jest prowadzenie kontroli<br />
jakoci zgodnie z lokalnymi przepisami.<br />
OGRANICZENIA TESTU<br />
Niektóre szczepy s trudne do zawieszenia i mog<br />
wykazywa z odczynnikiem obecno wókienek lub<br />
grudek. Nie powinno si myli tego z prawdziw<br />
aglutynacj. Pomimo wszystko, prawdziwa aglutynacja<br />
moe by widoczna pomidzy grudkami.<br />
Wyniki faszywie ujemne mog si zdarzy, jeli:<br />
- do testu uyje si niewystarczajcej liczby kolonii.<br />
W tym przypadku naley namnoy izolat<br />
i przeprowadzi test jeszcze raz z odpowiednim<br />
inokulum;<br />
- szczep nie wytwarza clumping factor, biaka A (agar<br />
krwawy);<br />
- szczep nie wytwarza specyficznych antygenów,<br />
przeciwko którym s skierowane przeciwciaa<br />
monoklonalne.<br />
W takiej sytuacji, naley wykona inne testy (6).<br />
Moliwe jest zaobserwowanie pozytywnych reakcji ze<br />
szczepami innych gatunków Staphylococcus, które<br />
posiadaj clumping factor i/lub koagulaz:<br />
S. schleiferi, S. lugdunensis, S. intermedius, S. delphini,<br />
S. hyicus (1, 7).<br />
Faszywie pozytywne reakcje mog zaj z niektórymi<br />
szczepami paciorkowców (szczególnie grup A, C i G),<br />
które posiadaj receptory dla immunoglobulin (8)<br />
i fibrynogenu (9).<br />
Reakcje nie do zinterpretowania s obserwowane<br />
z niektórymi szczepami enterokoków i mikrokoków.<br />
Nie powinno si uywa SLIDEX ® Staph-Kit<br />
bezporednio do bada hodowli krwi. Mona otrzyma<br />
faszywe wyniki spowodowane oddziaywaniem<br />
fibrynogenu i/lub IgG obecnych we krwi.<br />
OCENA TESTU<br />
Ocen odczynników przeprowadzono przy uyciu<br />
wieych izolatów szpitalnych i szczepów zamroonych<br />
w kilku szpitalach. Wyniki bada wieloorodkowych<br />
zostay opublikowane (10).<br />
Identyfikacj wszystkich szczepów Staphylococcus<br />
potwierdzono barwieniem metod Grama, testem na<br />
katalaz, testem probówkowym na koagulaz,<br />
a wtpliwoci potwierdzono metodami biochemicznymi.<br />
Zbadano wraliwo wszystkich szczepów na meticylin.<br />
Dokonujc oceny przebadano 940 szczepy S. aureus<br />
i 441 innych gronkowców. Wyniki otrzymane w tecie<br />
SLIDEX Staph-Kit porównano równie z otrzymanymi przy<br />
uyciu dwóch innych odczynników dostpnych w handlu.<br />
SLIDEX Staph-Kit wykazywa wysz czuo dla<br />
szczepów meticylino-opornych (wyniki nie do<br />
zinterpretowania nie byy brane pod uwag).<br />
Czuo wynosia 95.4% dla szczepów MRSA i 100% dla<br />
