IL FOCUS SULLA DEPRESSIONE<br />
Il focus quasi esclusivo sulla<br />
depressione negli ultimi 50 anni<br />
e l’uso eccessivo ed ubiquitario<br />
degli antidepressivi ha dannegiato<br />
i pazienti e la psichiatria stessa.
DEPRESSIONE<br />
Il termine Depressione, a parte la sua<br />
imprecisione semantica e nosologica,<br />
comporta un grave pregiudizio teorico:<br />
che si tratti sempre di stati di ridotta<br />
attività del sistema nervoso e delle<br />
funzioni neuropsichiche e quindi<br />
richieda sempre dei trattamenti attivanti.
ECCITAMENTO<br />
• Le potenzialità patogenetiche<br />
dell’eccitamento sopratuto nelle sue forme<br />
più leggere sono state sottovalutate.<br />
• Genera depressione, stati misti, stati<br />
psicotici e instabilità a lungo termine.<br />
• Dovremmo riconsiderare l’esteso,<br />
prolungato, e spesso totalmente non<br />
necessario uso degli antidepressivi.

La destabilizzazione<br />
Troppi pazienti bipolari o potenzialmente<br />
bipolari subiscono un peggioramento del<br />
decorso della loro malattia sotto l’azione<br />
degli antidepressivi. Tale effetto viene<br />
solo parzialmente contrastato dagli<br />
stabilizzatori dell’umore.

Prevenzione della depressione con<br />

Inaspettati effetti di stabilizzazione<br />

durante gli ultimi 50 anni, con agenti<br />
antimaniacali come il litio, gli<br />
antiepilettici, i neurolettici tipici e<br />
atipici, che sono diventati trattamenti<br />
correnti nella prevenzione sia della<br />
mania sia della depressione.

Conclusioni<br />
La psichiatria clinica dei disturbi<br />
dell’umore dopo il brillante sviluppo per<br />
opera, sopratutto di Griesinger, Falret,<br />
Kahlbaum e Kraepelin, ha subito una<br />
regressione durante la seconda metà del<br />
XX°secolo al punto di approfittare solo in<br />
parte delle nuove tecniche e terapie<br />
scoperte nello stesso secolo.
Le Cause<br />
• La dicotomia della malattia maniacodepressiva<br />
in forme unipolari e bipolari<br />
e la sottovalutazione delle forme<br />
attenuate di eccitamento e delle forme<br />
miste<br />
• Il prolungato uso di antidepressivi<br />
senza tenere conto del decorso<br />
successivo<br />
• I limiti e gli errori del DSM
