Hos 5/76 (7%) lyckades inte HVPG mätas<br />
även om det bildmässigt var god effekt. Hos<br />
3/76 (3%) uppnåddes inte adekvat trycksänkning.<br />
Tekniskt utfall värderades som lyckat<br />
i 109/119 (92%), respektive misslyckade i<br />
10/119 (8%). Bland de 76 med TIPS misslyckades<br />
bara en patient. Hos gruppen med<br />
tromboser som genes till intervention var<br />
34/44 (77%) bedömda som lyckade och<br />
10/44 misslyckade (23%). Av patienter med<br />
TIPS kunde hepatisk encefalopathi (HE)<br />
värderas i 72 fall varav 12 utvecklade HE<br />
(17%). Vi har inte haft någon mortalitet<br />
direkt i samband med intervention men<br />
två dödsfall hos patienter med svåra varicerblödningar<br />
gick ad mortem två respektive<br />
tre dagar efter TIPS i multiorgansvikt. En<br />
tredje patient var på väntelista för levertransplantation<br />
och utvecklade hjärtinfarkt<br />
tre dygn efter initialt lyckad TIPS. Patienten<br />
avled efter ett par veckor till följd av hjärtsvikt.<br />
Tekniskt utfall är skilt från det kliniska<br />
utfallet och vi bedömde att av alla patienter<br />
som fått TIPS kunde vi utvärdera det kliniska<br />
utfallet som lyckat i 57/74 (77%), som<br />
tveksamt i 12/74 (16%) och som klart misslyckat<br />
i 5/74 (7%) (varav de tre som avled<br />
tidigt efter TIPS ovan ingår).<br />
Uppsalaslutsatser: Interventionsradiologiska<br />
ingrepp för patienter med leverchirros<br />
och levernära tromboser har blivit en<br />
möjlig utväg vid svåra sjukdomsyttringar<br />
kopplade till den portala hypertensionen.<br />
Vanligaste ingreppet är TIPS, som är ett<br />
säkert och effektivt ingrepp vid svåra akuta<br />
varicerblödningar/reblödningar och förbättrar<br />
vanligen refraktär ascites. Vid levernära<br />
tromboser är komplikationsfrekvensen<br />
högre men merparten är tekniskt och kliniskt<br />
framgångsrika. Hepatisk encefalopathi<br />
är fortfarande metodens svaghet och<br />
varje patient måste värderas individuellt<br />
före ingreppet. Verksamhetens utveckling<br />
bygger på ett nära samarbete mellan skilda<br />
kompetenser i teamet och att vi kan erbjuda<br />
dygnet runt service.<br />
