Chairman’s Desk
I am happy to see that Human Resource Department is publishing the
employee newsletter from September 2019, which is the rst time in the
history of BMH. This will give lots of opportunities for the employees in BMH
to express their ideas and showcase their extracurricular activities and
achievements. It helps to build a very strong interpersonal relationship
among our employees that leads to a very good team work in order to achieve
our ultimate goal “Quality Care”.
I appreciate the effort taken by the entire team to make this
successful.Special thanks to team HR, the editorial board members and all
the BMH family members.

CEO’s Desk
I am extremely glad to know that our HR department has planned to initiate a
monthly E Journal of our employees from September 2019 onwards. Surely it
will be an ideal platform for our employees to exhibit their talents, knowledge
sharing and increasing the internal bonds.
BMH is well known for its proximity to its employees and we have bagged the
AHPI Awards for “The best hospital to work”, last year. The monthly
communication meetings, the GEM awards and long service awards are few to
be highlighted as the major employee appreciation activities undertaken by
the management. Our beloved Chairman Dr. K.G. Alexander is the only one in
the state of Kerala who was kind enough to help our employees who were the
victims of ood last year by way of constructing 2 houses, repairing few houses
and providing nancial assistance for about 69 employees for their losses.
BMH is one step ahead of any other healthcare institutions in the state for
imparting continuous training to its employees thus achieving employee
empowerment. We are technically so advance that many organizations send
their representative to us for observational visits and learning our practices.
Every external accreditation bodies appreciated the initiatives taken by our
Quality, HR and IT departments.
Wishing all our employees bundles of Health, Happiness and Prosperity.

I feel privileged to write these few lines in the rst issue of newsletter.
Congratulations to those creative minds who have come up with innovative
idea of a news letter. It’s envisaged to serve as a motivational force and
growth opportunity for talented staff and students of BMH family. While

to the market. I could not control my mind when I saw the beautiful

rupees notes. Looking at the notes, the manager said, ‘‘This currency
won’t be taken here..!’’
I was surprised to hear this..! I went to God and complained. God smiled
and said: ‘‘As a trader you don’t know all this? Your currency is not valid

Basics of Hematology
Hematology is the study of blood in health and disease. The
word "heme" comes from the Greek for blood. It includes
problems with the red blood cells, white blood cells, platelets,
blood vessels, bone marrow, lymph nodes, spleen, and the
proteins involved in bleeding and clotting (hemostasis and
thrombosis).
Anatomy of blood
Blood is made up of several component cells and includes red
blood cells, white blood cells, platelets, all which are
produced in the bone marrow. Red blood cells, which make
up about 45% of whole blood, carry oxygen from the lungs to
the body's tissue and also carry carbon dioxide back to the
lungs to be exhaled. White blood cells help fight infection.
Platelets are small, colorless fragments that stick together and
interact with clotting proteins to stop or prevent bleeding.
Plasma is the fluid part of the blood.Composed of 92% water,
it also contains vital proteins, mineral salts, sugars, fats,
hormones and vitamins.
Areas of study
Major areas of study within hematology include disorders of
red cells, white cells, platelets and clotting proteins
manifesting as hematological malignancies, anemia and
hemoglobinopathies, neutropenias, platelet disorders and
coagulopathy.
A hematologist is a medical doctor who applies this
specialized knowledge to treat patients with blood conditions.
Hematologists focus largely on lymphatic organs and bone
marrow and diagnose blood count irregularities or platelet
irregularities. Hematologists also treat organs that are fed by
blood cells, including the lymph nodes, spleen, thymus and
lymphoid tissue.In the medical field, hematology includes the
treatment of blood disorders and malignancies, including
types of haemophilia (a disease that prevents your blood from
clotting), leukemia (a cancer that affects blood cells),
lymphoma (a cancer that affects the lymph nodes), myeloma
(a cancer of plasma cells which affects bones and kidneys),
anemias (condition in which there aren't enough red blood
cells in your body) and thrombosis (condition in which blood
clots form inside your veins).
So, the Department of Clinical Hematology and
Hematooncology looks after blood and blood related
disorders, including nutritional anemias, haemolytic anemias,
bone marrow failure syndromes, platelet disorders,
myelodysplasias, bleeding and clotting disorders
(haemophilia and others), leukemias, lymphomas, myeloma
and other rare hematological malignant and non-malignant
disorders and consultative hematology. Diagnostic facilities
for basic investigations, coagulation and bone marrow
aspiration/biopsy are also available in department.

and a bone bit is biopsied to be analyzed under a

Mobile Apps: An Emerging
Tool to Improve Acute Stroke Care
Along with my colleague Dr Fabith Moideen in emergency
medicine dept. we decided to try a mobile app based approach
to solve this challenge of acute stroke care.
Stroke is a medical emergency caused by a sudden defect in
blood supply to the brain. In most cases, it is due to a clot in
one of the arteries supplying the brain, and timely
administration of clot dissolving medication, or extraction by
a device in case of large clots, can reverse the damage, and is
now the treatment of choice.
However, there is a catch - the treating team has to coordinate
a complex care plan efficiently, within a limited time. A scan
has to be done without delay, and this has to be interpreted
correctly, in proper context of the patient's symptoms. This
makes the consistent delivery of high-quality care a
formidable challenge.
The patient may for example, arrive at midnight, when only
an inexperienced doctor is on duty, making it difficult to
provide the expected standard of care. Or the expert may be
off site for some reason, and the phone number may not be
readily available to the person at the side of the patient.
In fact, there international guidelines recommending that the
so called “door to needle time” – the interval between arrival
of the patient at the hospital door and starting the clot busting
injection - should not exceed one hour. However, a review
conducted in 2011 noted that less than 1/3rd of patients
receive treatment within this stipulated time. There is thus a
lot of room for improvement in the delivery of acute stroke
care.
I personally had been interested in technology since my
school days and have been involved in various software
projects, including the Indian Train Status app, released in
2011 and has garnered over 25 million downloads.
The story of how the app was developed goes like this: for the
several years, since around 2011, we had been struggling to
keep the door to needle time for intravenous thrombolysis
under the recommended 1 hour. Our center, Baby Memorial
Hospital is spread over three blocks with the emergency
department and imaging centers located in separate buildings.
We had tried several measures like announcing a stroke code,
several training sessions, but the results were not consistent.
Similar approaches to reducing the door to needle time
including improving patient movement times, administering the
drug in the imaging room have been tried at several centers,
with varying success. An example is the “Helsinki model”
which was reported to improve door to needle times by 25
minutes in Melbourne, Australia. This included 1) ambulance
prenotification with patient details alerting the stroke team to
meet the patient on arrival; 2) patients transferred directly onto
the scan table on the ambulance stretcher; and 3) drug delivered
in scan room immediately after imaging. These approaches are
however, limited by availability of suitable manpower and
resources, and are yet to overcome logistical difficulties in all
settings.
In this context, we thought that the current wide spread
availability of smart phones and connected devices provided us
an opportunity to exploit their unique capabilities for
improving these aspects and optimize the execution of acute
care of stroke.
And that is how we came to think of making a mobile app,
which would enable the doctors in the emergency department
to enter the patient details, checklists and start a timer, which
was immediately notified to all team members including the
Stroke Neurologist, Radiologist, who could then open the app
from their location and follow the progress of the patient.
As the patient was shifted to scanning and results were available they could also be instantly communicated to the entire
team.
Thereby the team members were kept updated in real time regarding the patient's progress and could plan their timely
entry into the care pathway.
Development of the app took around 1 year to complete and finally we were able to deploy it successfully by April, 2017.
And the results were gratifying.
Over the next 1 ½ years after deploying the app we had an improvement of average door to needle time from 57 to 41
minutes, that is 16 minutes, and the percentage thrombolysed under 1 hour improved to 89% from 67%.
In stroke it has been estimated that every minute 2 million neurons – nerve cells – are lost. So, an average improvement
of 16 minutes in door to needle time which was achieved, which would amount to around 32 million nerve cells saved.
Thus, the app helped to improve team performance and identify causes of delay. One thing that surprised us was that
increased visibility of the performance was by itself a motivator and gave an additional boost to the facilitation provided
by technology. So, after all, it was not just technology - there was a human dimension to the benefit derived.
The app also automatically maintains a record of past cases and this can be used to audit the team's performance anytime.
There is another interesting possibility - once sufficient records are collected, we are able to use artificial intelligence and
machine learning to predict delays, and warn the team if a patient has potential for delay based on previous records from
that center. This has already been achieved at Baby Memorial Hospital, where we have over 200 historical records
available.
The app has been subsequently installed in 5 other hospitals around Kerala and they are reporting similar benefit. It now
available for download and trial from mcareapps.com
To conclude, mobile apps are an emerging tool to improve acute stroke treatment in all settings. There have been other
reports of mobile apps being successfully deployed with similar results, from USA and Europe.
Stroke care is unique in that a considerable amount of evaluation over separate facilities is required in a relatively narrow
window, thereby making such a mobile app-based
strategy especially valuable.
Similar app-based solutions are already available
for myocardial infarction, trauma and sepsis. We
envisage extended usage of similar apps in many
other subspecialties of medicine.
