PIERWOTNA ŻÓŁCIOWA MARSKOŚĆ WĄTROBY (PŻMW) – NAJCZĘŚCIEJ WYSTĘPUJĄCA CHOROBA AUTOIMMUNIZACYJNA WĄTROBY 51
jak również UDCA, prednizolonu i kolchicyny oraz
UDCA z metotreksatem i kolchicyną.
Osteoporozę i osteopenię współistniejącą z PŻMW
leczy się w zależności od stopnia zaawansowania
choroby według powszechnie przyjętych procedur
medycznych.
W schyłkowym okresie PŻMW leczeniem z wyboru
jest przeszczep wątroby. Rok po transplantacji
przeżywa ok. 80% chorych, a 5 lat ponad 60%.
Transplantację wątroby wymuszają: pogarszająca się
jakość życia, narastająca cholestaza i pogarszająca się
zdolność syntetyzująca wątroby. Wskazania do transplantacji
wątroby opracowane w Klinice Mayo opierają
się na monitorowaniu stężenia bilirubiny, albumin,
długości czasu protrombinowego, obecności lub braku
obwodowych obrzęków oraz wieku chorego[35].
Najważniejszym wskaźnikiem prognostyczno-rokowniczym
u chorych z PŻMW jest stężenie bilirubiny
w surowicy krwi [36] [37]. Narastanie jej stężenia jest
sygnałem do przygotowania chorego do kwalifikacji
do przeszczepu wątroby.
