This list comprises the Level II HCPCS codes, with the exception of those beginning
with the letter D. It is derived from file 01anweb.xls, the 2001 alpha-numeric
HCPCS file, downloadable from http://hcfa.gov/stats/pufiles.htm#alphanu.
These codes are revised annually. Discontinued codes are retained for four years.
Since the data in Medicare Data Utilities are typically more than four years old,
some records include codes not listed below. Such codes might be found in the 1999
and 2000 HCPCS files, downloadable from the same location.
Level II codes beginning D are copyrighted by the American Dental Association. They
may be purchased as part of the alpha-numeric HCPCS file from the National Technical
Information Service (703-487-4650) or the Government Printing Office (202-512-1800).
Level I codes are copyrighted by the American Medical Association. They may be
