Annexure –A
Undertaking by the Managing/Responsible officer of Spot Admission Form Collection
Centre (2011-14).
a. The Spot Admission Form Collection Centre will be required to provide the Hand Book of
Information Brochure for 2011-12 to the students on receipt of Rs. 200/- (Rs. 50/- for SC/BC
candidates of Haryana), the actual fee of Handbook of Information Brochure.
b. The Spot Admission Form Collection Centre shall send the duly completed Admission-
Cum-Examination forms either by hand or through speed post/registered
post/parcel/courier strictly within one week after the last date of receipt of applications.
In special circumstances the application can be accepted with special permission of the
Vice-Chancellor by the Director, University Centre for Distance Learning, Chaudhary
Devi Lal University, Sirsa-125055 (Haryana) failing which the Spot Admission Form
Collection Centre will be responsible for non-submission of Application Form of student.
University will not be responsible for the same.
c. The Spot Admission Form Collection Centre shall receive the complete application form along
with fee by way of Demand Draft drawn in favour of The Registrar, Chaudhary Devi Lal
University, Sirsa payable at Sirsa after satisfying itself by verifying the eligibility from original
mark sheets/certificates and shall attach the attested photocopies of required mark
sheets/certificates.
d. The application form must be sent to the University along with complete details of the name of
candidate/course/fees/demand draft number etc. after fixing stamp of Spot Admission Form
Collection Centre. And also ensure the candidate has written his/her name, course and
application form No. on the back of demand draft.
e. The share amount of 10% of the admission fee only (excluding examination fee) deposited
by the student of first year/second year/third year and/or lateral entry through the
concerned Spot Admission Form Collection Centre will be paid within 100 days after the
closing date of admission, only if student is admitted.
f. The local advertisement may be released by the centre at its own expenditure with the prior
approval of the Director, University Centre for Distance Learning, CDLU, Sirsa.
g. The Spot Admission Form Collection Centre will operate only at the approved place and at no
other place under any circumstances for the prescribed session only and no sub-centre.
h. No cash transaction will be made with the students by the Spot Admission Form Collection
Centres except sale of prospectus.

i. List of the approved supervisors be displayed on the University Website. However, in
exceptional cases (for the students of far-flung area only) the students may arrange their
supervisors from their neighboring places as per University rules.
j. The Spot Admission Form Collection Centre must ensure that the candidate has written his/her
name, course and application form No. on the back of Demand Draft.
k. The students will have to submit Admission-cum-Examination form at the same instance
along with the prescribed fee by the University. However eligible SC candidates of
Haryana are entitled for exemption of full tuition fee and remaining funds will be
charged in two installments except in case of courses through semester system. In case of
semester system the students will have to deposit first installment at the time of taking
admission and second installment will be deposited up to 31-03-2012. In case of semester
system, they will have to deposit the fee/funds (except tuition fee) along with Admission-
cum-Examination form at the time of taking admission in first semester of the course and
for the second semester of the course onwards, the students will have to deposit the
fee/funds as per schedule mentioned in HBI.
l. In case of any act or omission or deficiency on the part of the Spot Admission Form Collection
Centre, the University will be at liberty to cancel the Spot Admission Form Collection Centre
without assigning any reason. The Centre will also be required to attach the Annexure ‘B’ with
each form duly signed by the candidate or the guardian in case if the candidate is minor, and
also attested by centre’s authorized/responsible officer.
m. The Spot Admission Form Collection Centre will be black listed under intimation to the
UGC/DEC, New Delhi, in case of breach of rules/regulations and norms laid down relating to
the Academic and Financial matters. Further, the University reserves the right to forfeit in full
or part, the service charge/share payable by the University to the Spot Admission Form
Collection Centres, in case of any act or omission in violation of the rules and to take action
through the state machinery.
n. In case there is any deficiency/fraud/violation of rules etc. by the centre, the University has
right to cancel the centre without assigning any reason and can also forfeit the share amount
payable to centre in full or part as decided by the Director, UCDL, CDLU, Sirsa and further to
take action as per law.
o. In case any financial liability is incurred or imposed on the University on account of any act or
omission of the centre, in that case, the centre will be liable to repay and compensate the
University alongwith costs, interest etc.

p. The decision of the University in all the above matters shall be final. However in case there is
any dispute, the matter will be referred to the Vice-Chancellor, being sole arbitrator and the
decision of the Vice-Chancellor will be final.
q. The jurisdiction to hear the disputes or cases in this regard will be of the courts at Sirsa,

Dear Sir/Madam,
With reference to your application, your centre/organization is being recognized as the Spot
Admission Form Collection Centre to operate at (Name of place)

You are required to furnish and perform the following:
