Section 1 – Introduction<br />

This directory provides a list of <strong>Care</strong> <strong>Improvement</strong> <strong>Plus</strong>’s network providers. To get detailed<br />

information about your health care coverage, please see your Evidence of Coverage.<br />

<strong>Care</strong> <strong>Improvement</strong> <strong>Plus</strong> has a network of doctors, specialists, hospitals and other health care<br />

providers. However, we are an open access network, which means in addition to the<br />

providers listed in this directory, our members may also use any Medicare provider who<br />

will accept payment from <strong>Care</strong> <strong>Improvement</strong> <strong>Plus</strong>, with no referrals required for<br />

Medicare-covered services.<br />

Your Primary <strong>Care</strong> Physician/Provider (PCP) is the doctor who manages your general health<br />

care, meets state requirements, and is trained to give you basic medical care.<br />

As a member of <strong>Care</strong> <strong>Improvement</strong> <strong>Plus</strong>, you are encouraged to select a PCP. You may select<br />

your PCP from within our Plan network, or you may select an out-of-network provider as your<br />

PCP; however the provider must accept Medicare, and must also be willing to accept payment<br />

from our Plan. If you don’t have a PCP or if you wish to find a new one, you may call Member<br />

Services, and they will assist you in finding one. For the most current listing of Plan providers,<br />

you can visit www.careimprovementplus.com or contact Member Services.<br />

You may change your PCP for any reason, at any time. It is not necessary to notify the plan;<br />

however, if you want us to update your membership record, you may contact Member Services<br />

and we will update your information.<br />

The “network providers” listed in this directory have agreed to provide you with your healthcare,<br />

vision and dental coverage. This directory has been created for you based on your permanent<br />

mailing address. You may go to any of our network providers listed in this directory or for a<br />

complete list of network providers please visit our website at www.careimprovementplus.com or<br />

call Member Services at 1-800-204-1002.<br />

If you have been going to one network provider, you are not required to continue going to that<br />

same provider. In some cases, you may get covered services from non-network providers. <strong>Care</strong><br />

<strong>Improvement</strong> <strong>Plus</strong> is has an open access network; however, some services may require<br />

authorization. Hospital admissions, Skilled Nursing Facility (SNF), LTAC, Home Health, select<br />

Durable Medical Equipment (DME), and certain Part B drugs administered in a provider’s office<br />

may require authorization. For a complete list and detailed information on the services which<br />

require authorization, please see your Evidence of Coverage. If you have been going to one<br />

network provider, you are not required to continue going to that same provider. In some cases,<br />

you may get covered services from non-network providers that do not have a signed contract<br />

with our plan, so long as they agree to accept our terms and conditions of payment, as described<br />

at section1.2 of Chapter 3 in your Evidence of Coverage.<br />

It is important to remember we are an open access network, which means in addition to the<br />

providers listed in this directory, our members may also use any Medicare provider who<br />
