providers. If you get the care from network providers, your share of the costs will usually be<br />

lower than if you get the care from out-of-network providers.<br />

What is “urgently needed care”?<br />

“Urgently needed care” is a non-emergency situation when you need medical care right away<br />

because of an illness, injury, or condition that you did not expect or anticipate, but your health is<br />

not in serious danger.<br />

What if you are in the plan’s service area when you have an urgent need for<br />

care?<br />

In most situations, if you are in the plan’s service area and you use an out-of-network provider,<br />

you will pay a higher share of the costs for your care. If the circumstances are unusual or<br />

extraordinary, and network providers are temporarily unavailable or inaccessible, our plan will<br />

allow you to get covered services from an out-of-network provider at the lower in-network costsharing<br />

amount.<br />

What if you are outside the plan’s service area when you have an urgent need<br />

for care?<br />

Suppose that you are temporarily outside our plan’s service area, but still in the United States. If<br />

you have an urgent need for care, you probably will not be able to find or get to one of the<br />

providers in our plan’s network. In this situation (when you are outside the service area and<br />

cannot get care from a network provider), our plan will cover urgently needed care that you get<br />

from any provider at the lower in-network cost sharing amount.<br />

What if you are billed directly for the full cost of your covered services?<br />

You can ask the plan to pay our share of the cost of your covered services<br />

In limited instances, you may be asked to pay the full cost of the service. Other times, you may<br />

find that you have paid more than you expected under the coverage rules of the plan. In either<br />

case, you will want our plan to pay our share of the costs by reimbursing you for payments you<br />

have already made.<br />

There may also be times when you get a bill from a provider for the full cost of medical care<br />

you have received. In many cases, you should send this bill to us so that we can pay our share<br />

of the costs for your covered medical services.<br />

If you have paid more than your share for covered services, or if you have received a bill for the<br />

full cost of covered medical services, go to Chapter 7 (Asking the plan to pay its share of a bill<br />

you have received for medical services drugs) of your Evidence of Coverage for information<br />

about what to do.<br />

If services are not covered by our plan, you must pay the full cost<br />

<strong>Care</strong> <strong>Improvement</strong> <strong>Plus</strong> covers all medical services that are medically necessary, are covered<br />

under Medicare, and are obtained consistent with plan rules. You are responsible for paying the<br />
