EMPOWERING LIVES IN KENYA: THE CHEBAIYWA CLINIC<br />

The Cl<strong>in</strong>ic tried to ma<strong>in</strong>ta<strong>in</strong> a relatively affordable fee structure for its patients, but Tarus knew that it<br />

cont<strong>in</strong>ued to suffer from the perception <strong>of</strong> some that as a “missionary” entity the Cl<strong>in</strong>ic should never<br />

charge for services. But Chebaiywa was still significantly cheaper than the private cl<strong>in</strong>ics—perhaps it<br />

could ga<strong>in</strong> marg<strong>in</strong>s by rais<strong>in</strong>g prices across the board.<br />

Another possibility was to somehow charge more to those with a higher will<strong>in</strong>gness or ability to pay<br />

for services. A small portion <strong>of</strong> the local population was relatively wealthy and could afford to pay a<br />

great deal <strong>in</strong> medical costs; maybe the Cl<strong>in</strong>ic could <strong>of</strong>fer house-call services at a premium price, or<br />

guarantee “special access” to the medical staff upon walk<strong>in</strong>g <strong>in</strong>to the Cl<strong>in</strong>ic. It seemed such services<br />

would be pr<strong>of</strong>itable, but it was unclear what impact they would have on the Cl<strong>in</strong>ic’s ability to serve<br />

the rest <strong>of</strong> the non-premium patients. Perhaps a more simple solution would be to merely assess a<br />

family’s wealth and charge on a slid<strong>in</strong>g scale.<br />

Market<strong>in</strong>g<br />

Simply gett<strong>in</strong>g more patients <strong>in</strong> the door would help to close the gap—the gross marg<strong>in</strong> was positive<br />

on every s<strong>in</strong>gle Cl<strong>in</strong>ic transaction. But there rema<strong>in</strong>ed the question <strong>of</strong> how many more patients the<br />

Cl<strong>in</strong>ic could handle, and how to attract them. Tarus believed word <strong>of</strong> mouth was the most powerful<br />

advertis<strong>in</strong>g mechanism. As a respected community leader, he was certa<strong>in</strong>ly <strong>in</strong> a position to drive word<br />

<strong>of</strong> mouth—but what should the message be, and where and to whom should it be delivered?<br />

Tarus was also <strong>in</strong>trigued by the possibility <strong>of</strong> comb<strong>in</strong><strong>in</strong>g pric<strong>in</strong>g with market<strong>in</strong>g by issu<strong>in</strong>g customer<br />

loyalty cards similar to those he had seen <strong>in</strong> U.S. supermarkets. The Cl<strong>in</strong>ic could <strong>of</strong>fer a discount with<br />

the use <strong>of</strong> a free card, <strong>in</strong> turn encourag<strong>in</strong>g loyalty from customers.<br />

New Services<br />

Another avenue to consider was new services beyond those currently <strong>of</strong>fered by the Cl<strong>in</strong>ic. For<br />

example, several members <strong>of</strong> the community had compla<strong>in</strong>ed that they had to go far away to get<br />

<strong>in</strong>sul<strong>in</strong> for their diabetes treatments, and asked why the Cl<strong>in</strong>ic couldn’t <strong>of</strong>fer diabetes care.<br />

Tarus’ ambitions to transform the dispensary <strong>in</strong>to a health center would allow the Cl<strong>in</strong>ic to <strong>of</strong>fer<br />

<strong>in</strong>patient care and non-emergency maternity services. Across the ma<strong>in</strong> road <strong>in</strong> the next district,<br />

Lumakanda district hospital delivered over 500 <strong>in</strong>fants every year. Could some <strong>of</strong> those deliveries<br />

take place <strong>in</strong> Chebaiywa? At a price <strong>of</strong> 1,000 Ksh per delivery, maternity services could have a big<br />

impact on the Cl<strong>in</strong>ic’s <strong>in</strong>come.<br />

Likewise, the lab technician <strong>in</strong> the Cl<strong>in</strong>ic argued strongly for acquir<strong>in</strong>g a CD-4 counter to conduct<br />

blood cell counts and enable direct monitor<strong>in</strong>g <strong>of</strong> HIV patients. Currently the Cl<strong>in</strong>ic transported its<br />

HIV patients every week to an AMPATH satellite cl<strong>in</strong>ic <strong>in</strong> Turbo, about 10 kilometers down mostly<br />

unpaved roads. HIV care also had the advantage <strong>of</strong> be<strong>in</strong>g funded by the government and NGOs.<br />
