HPAC APPLICATION INSTRUCTIONS<br />

<strong>To</strong> <strong>obtain</strong> a <strong>committee</strong> <strong>interview</strong> <strong>and</strong> <strong>letter</strong>, <strong>you</strong> <strong>must</strong> <strong>submit</strong> <strong>all</strong> materials to Interfolio by midnight on Friday,<br />

February 22, 2013. Please see the Interfolio Instructions document on the Health Professions website for detailed<br />

instructions.<br />

APPLICATION COMPONENTS<br />

I. WRITTEN MATERIALS<br />

The following should be uploaded to Interfolio as a single Word document:<br />

A. Applicant Data Form <strong>and</strong> Signature Page<br />

You <strong>must</strong> include an AMCAS, AADSAS, AACOMAS, or TMDSAS ID on the data form. These IDs<br />

are assigned to <strong>you</strong> when <strong>you</strong> open an account with the appropriate health profession’s application<br />

II. TRANSCRIPT(S)<br />

After the f<strong>all</strong> 2012 grades have posted, <strong>obtain</strong> an unofficial copy of <strong>you</strong>r transcript from the Registrar <strong>and</strong><br />

<strong>submit</strong> it to Interfolio. We will not accept transcripts printed from GWeb. Also <strong>obtain</strong> copies of<br />

unofficial transcripts from other schools where <strong>you</strong> have taken courses <strong>and</strong> have these transcripts <strong>submit</strong>ted<br />

III. LETTERS OF RECOMMENDATION (LOR)<br />

You need at least three <strong>letter</strong>s of recommendation, including one from a member of the science faculty at<br />

GWU <strong>and</strong> one from a member of the faculty in <strong>you</strong>r major at GW. The total number of<br />

recommendations should not exceed five. You should waive <strong>you</strong>r rights of access to <strong>all</strong> of <strong>you</strong>r <strong>letter</strong>s of<br />

recommendation. A confidential <strong>letter</strong> is taken more seriously <strong>and</strong> given more credence by its reader.<br />

INTERVIEW PROCESS<br />

You will be notified via email (by the end of February) with instructions on how to schedule <strong>you</strong>r HPAC<br />

<strong>interview</strong>. Interviews will only be scheduled upon <strong>you</strong>r submission of a completed HPAC application. Interviews<br />

take place between March <strong>and</strong> June. Each <strong>interview</strong> will be 15-20 minutes in duration. You will need to provide 4<br />

copies of <strong>you</strong>r current resume at the time of <strong>you</strong>r <strong>interview</strong>.<br />

COMMITTEE LETTER PACKET<br />

The Committee <strong>letter</strong> of evaluation will be written by early July for <strong>all</strong> <strong>interview</strong>ed students. Once the <strong>letter</strong> is<br />

completed <strong>and</strong> released, it will be maintained in <strong>you</strong>r Interfolio account. The <strong>committee</strong> evaluation packet will<br />

consist of a cover <strong>letter</strong> describing the GW HPAC process, an individual evaluation <strong>letter</strong> written by the<br />

<strong>committee</strong>, <strong>and</strong> <strong>you</strong>r <strong>letter</strong>s of reference <strong>submit</strong>ted as a part of this application.<br />
