Selecting the Right Size and How to Measure 

1. Compression Level 

A basic guide to compression levels is 

on page 7. Finding your correct support 

level is an important step that should be 

discussed with your doctor in order to 

maximize the benefits of compression 

therapy. Underlying medical conditions 

may prevent the use of a particular compression 

range. For example, it is generally 

understood that people with diabetes 

should not use compression higher 

than 30mmHg. Higher compression garments 

can be difficult to put on and may 

not be appropriate for people with severe 

arthritis or immobility. (We have donning 

aids on page 93 that can help.) 

2. Style and Brand 

FOR HOSIERY: Next, you decide on the 

style of hosiery: knee-high, thigh-high, 

pantyhose, leotard, or sock. Then select 

the brand. 

FOR LYMPHEDEMA ARMSLEEVES, 

GAUNTLETS, AND GLOVES: Lymphedema 

products have several style options: 

armsleeve, gauntlet, glove, or a 

combination. Decide on a brand after 

reviewing the lymphedema section, 

starting on page 66. 

Depending on the brand, armsleeves are 

made with a regular elastic or silicone 

top band. If you have a large upper arm, 

you may need an armsleeve with the silicone 

top band since it provides additional 

assistance in helping the armband to 

stay in place. Gauntlets cover part of the 

hand and thumb but not the other four 

fingers. Gloves cover the hand and part 

of all five fingers but not the fingertips. 


3. Take Your Measurements 

Since each brand measures and assigns 

sizes in different ways, it’s important 

that you measure based on the size 

chart for the brand you have selected. 

Each product listing will refer to a 

size chart nearby or at the end of that 

brand’s section. The chart will explain 

what part to measure and where to 

measure. Be sure to write this down as 

you measure. (Note: You’ll need to measure 

both limbs if they are visibly different 

sizes. This is particularly important 

when wearing thigh-high stockings.) 

4. When to Measure 

Measure when swelling is at a minimum—usually 

in the morning. 

5. Use the Correct Size Chart 

Each hosiery brand has unique size 

charts and size-measuring instructions. 

Use the correct chart for the product, 

style and brand you have selected. Don’t 

guess! If you wear a small or a large in 

other types of garments, don’t assume 

you’ll wear the same size in compression 

hosiery. This is a medical garment 

and you want it to fit well and perform 

as required.

FAQ: First-Time Compression Hosiery Buyers 

Q. Any advice for a first-time buyer of 

compression hosiery? What style 

should I buy? 

A. Each brand’s section will list the styles— 

which may include mid-calf socks, 

knee-high socks or stockings, thigh-high 

stockings, pantyhose, and leotards—and 

the compression levels available. Some 

of the knee-high and thigh-high stockings 

have silicone top bands to help 

them stay up. (The stockings with the 

silicone top bands are usually used by 

people with larger than average calves 

or thighs because the stockings may not 

stay in place with the regular elastic 

bands.) Not all types of stockings are 

available for each level of compression. 

KNEE-HIGH HOSIERY OR SOCKS 

If you have aching feet or swelling in 

your lower legs or ankles, you may need 

a knee-high stocking that provides compression 

from the ankle to just below 

the knee. 

THIGH-HIGH HOSIERY OR PANTYHOSE 

If you need compressive support 

throughout the entire length of the leg, 

thigh-high stockings, pantyhose, or a 

leotard should be appropriate. 

MID-CALF SOCKS 

People with diabetes, athletes, and 

frequent travelers needing a low level 

of compression can benefit from the 

various types of socks that rise to midcalf.

Q. How do I know what size to order? 

A. Great question! Wearing the right size is 

crucial to getting the best results from 

compression hosiery. Each brand has its 

own size charts at the end of each section. 

Each size chart requires you to take 

measurements. Before you measure, 

you’ll need to know four things: compression 

level, style of hosiery, hosiery 

brand, and brand line (examples of 

brand lines are Jobst UltraSheer or Jobst 

Opaque). For more information on how 

to take your measurements, please refer 

to pages 8–9. 

Q. What about insurance—does it 

usually cover compression hosiery? 

A. Many insurance plans don’t cover compression 

hosiery, but you should check 

to see if yours will reimburse you for the 

expense. Because of the administrative 

costs, we regret that we are unable to 

process insurance claims for you. You’ll 

need to contact your insurance company 

regarding coverage and how to file for 

reimbursement.

FAQ: Brands and Types of Products 

Q. Why are some brands so much more 

expensive than others? 

A. Because compression hosiery is a medical 

product that must meet strict standards 

of performance, the materials 

used, the fabric finish, and the techniques 

employed in weaving the hosiery 

all contribute to the cost. Premium 

brands made by large companies with 

established reputations are more expensive 

than private-label brands which 

have lower marketing and packaging 

costs. Other factors that affect compression 

hosiery costs are fabric quality, 

level of compression and ease of care for 

the products. Smoother, softer fabrics 

and higher levels of compression have 

higher production costs. Today, many 

brands allow machine washing and, for 

some products, machine drying. 

Q. I hear the terms support stockings, 

support hose, compression hosiery, 

and compression hose all used from 

time to time. Do they mean the same 

thing? 

A. Yes, they do. At BrightLife Direct we use 

the term compression hosiery most frequently 

but it means the same thing as 

the other terms. 

Q. Will I get the same compression 

from sheer fabric stockings as I 

will from the thicker opaque fabric 

stockings? 

A. Yes. Both fabrics provide the same quality 

and level of compression. The difference 

is usually that the thicker opaque 

fabrics are more durable than the thinner 

sheers.

Q. How long do compression stockings 

last? 

A. They should last between four and six 

months, depending on the type of fabric 

(sheer stocking fabrics are not as durable 

as opaque) and if they are cared 

for properly. Keep the following factors 

in mind when estimating how long your 

hosiery should last: 

• The most common reason for 

compression garments wearing out 

too quickly is careless washing and 

drying. Washing and drying must be 

done according to the manufacturer’s 

instructions that come with each 

pair. Be sure to use the correct kind 

of cleaning solution. Jolastic Washing 

Solution is recommended for cleaning 

compression hosiery. Woolite, 

however, can ruin the compressive 

ability of the garments. Don’t use 

high temperatures when washing or 

drying, and don’t machine dry unless 

the stocking instructions specifically 

allow for it. 

• Clean frequently enough to remove 

the natural perspiration, as well as 

the oils, acids, and flakes that the skin 

is always producing and shedding. 

Letting these substances remain in 

contact with the stocking fibers will 

weaken them and decrease the life 

span of the garment. 

• When donning and removing the 

hosiery, don’t snag the stockings on 

finger or toe nails, jewelry, or rough 

surfaces. Keep toenails trimmed, 

as long toenails can tear or cause 

increased wear in the toe area. 


• If worn with shoes that are too tight 

or that rub against the stocking fabric, 

the life span of the garment will be 

decreased. 

• Compression will gradually decrease 

over time, so mark your calendar 

with a reminder of when you need to 

replace your stockings. 

Q. What’s the difference between 

open-toe and closed-toe stockings? 

A. There is no difference in the medical 

efficacy between open-toe and closedtoe 

stockings. It’s a personal preference. 

An open-toe stocking is one that is 

woven so that the toes and part of the 

foot remain bare. With the popularity 

of sandals and open-toe shoes, there’s 

been a bigger demand for compression 

hosiery made in the open-toe style. 


Q. Does compression hosiery still have 

that “clinical look,” or do I now have 

more stylish options? 

A. Compression hosiery has come a long 

way since the days when support 

stockings all looked clinical. You can 

now select fashionable sheer or very 

sheer hose in an array of attractive 

colors while still benefiting from all 

compression levels. Men’s socks are now 

woven using techniques that make them 

indistinguishable from regular ribbed 

dress socks. Women’s trouser socks 

look like regular socks as well. Because 

of the advanced weaving technology 

now employed, most non-surgical types 

of compression hose do not have a 

therapeutic appearance. 

Q. What is the difference between 

anti-embolism stockings (TED) and 

graduated compression hosiery? 

A. Anti-embolism stockings, also called TED 

stockings, are used in hospitals for nonmobile 

patients. Doctors prescribe them 

for those patients who are confined to a 

bed and at risk of blood clot formation. 

TED stockings, in appearance, are made 

from white fabric and have an inspection 

opening just above the toes. TED 

stockings are usually a mild 8–15mmHg 

or 15–20mmHg compression, with the 

highest amount of compression at the 

ankle, diminishing as it goes up the leg. 

TED stockings are also appropriate for 

nighttime wear.

The Allegro Surgical Weight stocking is a unisex product 

and recommended by many of our referring physicians. 

Available in the higher compression levels, these are often 

used for relief from the more severe forms of venous 

deficiencies and disease. Like all Allegro stockings, they 

feature medically correct gradient compression. The 

Surgical Weight fabric is opaque and is designed to help 

conceal varicosities, blemishes, or scarring. Styles available 

include knee-high and thigh-high stockings, in both closed 

and open toe, and pantyhose, in closed toe only. 

Surgical Weight Knee-Highs are available in an open or closed toe, with a “balloon” toe pocket for added 

comfort. The stockings provide either 20–30mmHg or 30–40mmHg graduated compression. (30–40mmHg

fabric. It is a unisex garment made in regular and short lengths in five 

different sizes. Styles include knee-high or thigh-high in either closed or

What is Economy Class Syndrome? 

“Economy Class Syndrome,” referring to the cramped 

seating in economy sections of aircraft, is the common 

name for the medical condition called deep vein 

thrombosis (DVT). Sitting in a confined space for long 

periods of time, whether in an airplane, car, truck or even 

at your desk, can reduce blood circulation and allow blood 

clots to form deep in the legs. Medical studies have shown 

that these clots can be extremely dangerous. However, 

studies also show that gradient compression hosiery 

(support stockings) can improve circulation in the leg and 

reduce the chance of developing DVT. 

WHAT YOU CAN DO 

Stretch in your seat and flex your feet up and down or in 

a circular fashion. Get up and walk around, if possible, 

at least once an hour. Avoid crossing your legs and drink 

plenty of fluids. Wearing compression socks, thigh-highs 

or pantyhose is generally considered a very effective 

deterrent of DVT. 

WHICH STOCKINGS WORK BEST? 

15–20mmHg compression is generally recognized to be 

adequate for travelers without any significant underlying 

medical problems. Wear what is most comfortable for 

you. Most people choose a sock with some wicking 

properties to keep feet cool and fresh in addition to 

providing compression to enhance blood flow. Make sure 

your sock is knee-high so that it covers and compresses 

the entire calf. If you have any questions, contact your

What is a venous skin ulcer 

and how is it treated? 

A venous skin ulcer is an open wound that 

typically develops on the lower leg and can 

take months to heal. Wearing compression 

stockings is the single most effective way 

of controlling the painful fluid buildup 

and pooling of blood in the lower legs that 

cause venous skin ulcers. As a preventative 

measure, you can greatly improve your 

chances of remaining ulcer-free by wearing 

compression stockings on a daily basis. 

Most ulcer care systems come with an inner 

light-compression liner. It is donned first to 

cover and protect the bandaging on an active 

ulcer and also to prevent abrasions from the 

higher compression outer garment rubbing

Q. I am having breast cancer surgery soon. Is 

there any product that can help alleviate the 

post-surgical pain? 

A. Yes, many people find that a compression 

bra used immediately after surgery and for a 

period up to eight weeks thereafter will help 

reduce the pain. More importantly, gentle, 

even compression around the torso helps to 

evacuate post-surgical edema and assists to 

resolve fluid accumulation in the breast and 

chest wall area. 

Q. What’s the difference between a mastectomy 

bra and a torso compression garment? 

A. A good torso compression garment will facilitate 

lymph drainage from the breast and 

chest wall. Mastectomy bras are not designed 

to deal effectively with swelling, lymphedema, 

or post-surgical discomfort in the chest and 

breast area. Their primary function is cosmetic. 

They are designed to hold a prosthesis 

in place and give a women a natural shape. 

The important difference is that these bras 

often have smaller or shorter silhouettes, thin 

straps, rigid cups, limited adjustability, and 

underwires that may trap fluid in the breast 

area. The cup of a fashion bra is there to accept 

the volume of one breast and improve it’s 

cosmetic appearance and shape. But rigid or 

tight elastic or wires may increase pain by digging 

into tender tissue and may impede flow 

of postoperative or congested lymphatic fluid. 

Q. I had a lumpectomy several years ago. I am 

finding my operated breast feeling different 

than my non-operated breast. Do I need a 

special bra? 

A. If your affected breast skin texture feels different, 

your bra strap leaves an indentation, or 


you have scar dimpling, this could be an indication 

that you have unresolved swelling and 

likely chest wall lymphedema. Lymph nodes 

are commonly removed during breast surgery, 

which impacts the body’s ability to drain interstitial 

fluid from the chest wall and breast. 

The body sometimes can not establish new 

fluid pathway’s quickly, so fluid may accumulate 

slowly over time. If fluid stays in an area 

for too long, then it grows scar tissue fibers 

that create a dense or full feel to the breast 

and chest wall. Surgical scars may contract or 

thicken over time and block fluid movement. 

Q. If I have swelling in my arm after breast 

cancer treatment, do I have lymphedema? 

A. When breast cancer is a diagnosis, surgery, 

chemotherapy, and/or radiation are the treatments 

of choice. When surgery is performed 

on the breast, the nearby lymph nodes are 

commonly removed, too. Those lymph nodes 

are part of the lymphatic system that drains 

that quadrant of the body, including the associated 

limb. Radiation also affects good cells 

and underlying tissue. Because the lymph 

vessels are so delicate, they become damaged 

during the treatment and thereby block the 

lymphatic vessels from draining and prevents 

it from returning to the circulatory system. 

Swelling can result when the remaining 

vessels can no longer assume the job of the 

blocked vessels. Lymphedema would then be 

a very logical diagnosis. But, it is important to 

have the swelling evaluated by a knowledgable 

physician or a certified lymph therapist. 

Q. I was told that I have stage two lymphedema. 

What does that mean? 

A. There are four recognized stages of lymphedema.

the condition is irreversible, swelling with pitting 

edema is present, and fibrosis formation 

is likely. Treatment is highly recommended to 

mobilize excess fluid, pathogens, extra cellular 

debris, and protein rich fluid back into 

the lymphatic system for filtration through 

the lymph nodes. The risk of infection is high. 

This stage can progress to stage three, which 

is elephantiasis. For further information about

Q. My arm isn’t swollen, but it feels heavy and 

achy. Is this lymphedema? 

A. Many people who have lymphedema have 

no symptoms of discomfort and are not even 

aware of the swelling. However, there are also 

many people who report that the limb feels 

different, heavy, achy, and even painful. We 

estimate that approximately 30% more fluid 

can exist in the tissue before it is detected by 

visual inspection. Symptoms without swelling 

are probably the very early signs of lymphedema, 

which is the latency stage or stage zero, 

and is best treated with manual lymph drainage 

(self-performed after instruction from a 

certified therapist) and a mild compression 

garment. If treated early, studies are showing 

that the lymphedema can be controlled and 

even reversed with regular treatment. 

Q. Why do doctors and therapists prescribe 

compression garments? 

A. Increasing tissue pressure around the cells 

where fluid (interstitial fluid) accumulates will 

increase the ability of the fluid to be pulled 

into the lymphatic vessels where it becomes 

lymph. When this fluid is left to accumulate, 

swelling becomes obvious, but more importantly, 

this fluid is filled with plasma proteins, 

pathogens, waste products from cells, and 

even cancer cells. It becomes the perfect medium 

for bacteria to grow, leaving the area at 

higher risk for infection, which can become a 

life threatening condition. 

It is important to understand the type of 

edema that is present and why it is occurring. 

There are some types of swelling that are

caused from venous insufficiency or, possibly, 

a problem with the heart or kidneys. Great 

caution must be taken when applying compression 

to cardiac related swelling as it can 

make the cardiac condition worse. 

Q. When I wear my compression stockings, I am 

more swollen in the evening than when I put 

them on in the morning. Are my stockings 

working correctly? 

A. In a perfect world, compression garments 

should keep you from swelling from the time 

you put them on in the morning to the time 

you take them off at night. However, there 

are many factors that may cause you to swell 

beyond the garment by the end of the day. You 

may need to go to a higher compression or 

different type of garment. Or you may benefit 

from increased muscle pump activities, which 

you can get if you walk or move your limbs 

more throughout the day. If you are swelling 

only a little bit by the end of the day, and it 

resolves after a night’s sleep, then your garments 

are probably working correctly. 

Q. My therapist told me that I have “protein rich 

edema,” or lymphedema. Why shouldn’t I just 

restrict my diet of protein? 

A. The protein that is trapped is not protein 

from food, but the protein that is found in the 

plasma of your blood. Restricting protein is 

not advised, as protein from food is the building 

blocks for all cell growth and repair. Protein 

restriction will actually make the swelling 

worse. 

Q. My therapist told me that I should wear a 

compression garment when I fly? Why? 

A. The less atmospheric pressure you encounter 

on the outside of your body, the less tissue 

pressure you have within your body. A fine 

balance exists when atmospheric pressure 

fluctuates and swelling often results. Under 

normal conditions, when the lymphatics are 

simply stressed and not damaged, the lymphatics 

accommodate and return to normal 

within a short period of time. But when the 


lymphatics have been damaged from a surgical 

or radiation procedure or traumatic injury, 

sometimes flying can be the the trigger for the 

onset or exacerbation of lymphedema. 

Q. My doctor didn’t tell me that I could be at risk 

for lymphedema. What is my risk of developing 

this condition? 

A. Lymphedema is a condition that often develops 

after cancer treatment. Whenever lymph 

nodes or the underlying lymph vessels are 

damaged, there is always risk to that system. 

The chance of developing lymphedema increases 

with the number of lymph nodes that 

are removed, the amount of radiation, the age 

of the body, previous injuries, general health, 

and general nutrition. External factors, such 

as insect bites, acute injuries to the affected 

limb, poorly fitting clothing, and/or the hot 

seasons of the year, have also been identified 

as factors that can influence the condition. 

Q. My mother and grandmother have always 

had heavy legs. I don’t have heavy legs, but 

my daughter is now a teenager and she is 

getting those heavy legs. What could be the 

problem? 

A. This may be an interesting condition called 

primary lymphedema, or lipodema. Primary 

lymphedema is inherited and is caused by inadequate 

functional lymph vessels or an inadequate 

number of vessels. It can express itself 

in infancy or as late as the mid-fifties. Patients 

sometimes have a triggering event and very 

suddenly have unexplained swelling, while others 

slowly develop the condition. Lipodema is 

not really a lymphedema condition, but, when 

it becomes severe, lymphedema can result. 

Q. My husband had a crushing injury to his upper 

leg when he was a teenager. He recently 

had a knee replacement on that same leg, but 

now has terrible swelling that won’t go away. 

Did the knee replacement surgery go wrong? 

A. Severe trauma to any body part often results 

in the development of scar tissue. Scar tissue 

develops to pull a wounded area together, 


and the lymph vessels get trapped and strangulated 

in this maze of fibrous tissue. When 

injury occurs in young people, the adjacent 

lymph vessels often take over the function of 

the damaged area. But as we age, the lymph 

vessels don’t function with as much vigor as 

they once did. So when additional trauma 

is encountered, the lymph system becomes 

overwhelmed and swelling results. This 

swelling may be temporary or it may become 

true lymphedema, where the lymph system 

can’t keep up with the demand and swelling 

remains for the remainder of one’s life. It is 

unlikely that the surgery was done incorrectly, 

but the pre-existing condition contributed to 

the onset of lymphedema. 

Q. Why shouldn’t I have regular massages when 

I have lymphedema? 

A. Regular massage is generally a technique that 

addresses muscles and deep tissue stimulation. 

Traditional massage increases blood 

flow, which is good for most people, but it 

also increases the fluid load on the lymphatics. 

If the lymphatics are compromised, then 

this overload can make lymphedema worse. 

The special techniques used in lymphedema 

management are very superficial (shallow), 

and generally not deep in the tissue. Regular 

massage to an unaffected body part is safe. 

Q. I want to know more about lymphedema. 

Where can I get reliable information about 

the condition? 

A. There are a number of places to find reliable 

information about lymphedema on the Internet. 

There are also lymphedema patient support 

organizations, and research efforts being 

made by dedicated scientists who are trying to 

unravel the causes, identify the best treatment 

practices, and recommend garments and 

devices that assist in management of lymphedema.
