Lubo’s Place<br />
After a yearlong renovation of the<br />
beaches along the Rickenbacker<br />
Causeway in <strong>Miami</strong>, the beaches<br />
have reopened. Re-nourished<br />
beaches, newly planted palm trees<br />
and spacious parking now greet visitors.<br />
Gone are the “let’s park our<br />
truck on the beach and drink beer<br />
crowd”.<br />
The beaches have been returned to<br />
the crowd that enjoys being active<br />
in the outdoors. Catamaran sailing,<br />
windsurfing, stand up paddleboarding<br />
and cycling opportunities are<br />
available from several vendors<br />
located on the beaches between the<br />
tollbooth and the <strong>Miami</strong> Seaquarium.<br />
We recently stopped by to visit a<br />
friend of MSM. Lubo has a SUP<br />
and bike rental concession located<br />
just before the <strong>Miami</strong> Seaquarium.<br />
Actually “rental concession” doesn’t<br />
do it justice. It is more like a miniresort.<br />
With lounges, umbrellas and<br />
white sand beaches awaiting those<br />
who want to chill, and three trailers<br />
stocked with sports toys for those<br />
who prefer to get their sport on- Lubo’s<br />
Place is your own ClubMed.<br />
On our visit we joined in with a<br />
SUP group celebrating a birthday.<br />
On our return Lubo popped some<br />
chilled bubbly. The perfect postsport<br />
recovery drink!<br />

91-acre lake at Amelia Earhart<br />
Park is an ideal place to enjoy<br />
a day of water sports. The park<br />
also offers champion coaching<br />
for wakeboarding, waterskiing,<br />
wake surfing, knee boarding<br />

to learn a new sport. This is<br />
<strong>Miami</strong> Dade County’s Extreme<br />

Amelia Earhart is home to one<br />
of South Florida’s top bike trails<br />
for riders of all levels. The 7 to<br />
8-mile trails include both smooth,<br />
well-maintained single-tracks<br />
and fire road trails with climbs<br />
and down hills complete with<br />
banked corners for speed freaks.<br />
An advanced section of the trail<br />
has BMX-style jumps for more<br />

Park is the home of a historic<br />
lighthouse built in 1825 and<br />
reconstructed in 1846, and is<br />
the oldest standing structure in<br />
<strong>Miami</strong>-Dade County. This park<br />
boasts more than one mile of<br />
sandy Atlantic beachfront making<br />
it the perfect place for kayaking,<br />
SUP or just soaking up the sun.<br />
The park has a paved path<br />
approximately 1.5-miles long and<br />
unpaved service roads for easy<br />
biking/hiking. Visitors can easily<br />
ride/hike a few miles in a loop<br />
by routing through the various<br />
paths and dirt roads. Some of the<br />
trails lead you beneath the trees<br />
and through mangrove wetlands<br />
while others offer magnificent<br />
views of Biscayne Bay. Eagle<br />
ray and manatee sightings are not<br />

This park is named after the late<br />
football star Brian Piccolo who<br />
made a name for himself with<br />
his sports prowess at a local<br />
Broward High School. He later<br />
played college football and was<br />

football with the Chicago Bears.<br />
However, Brian only played four<br />
seasons before falling victim to<br />
cancer. Today, the 180-acre park<br />
bearing his name is one of the<br />

complete with a 333.3-meter<br />
Velodrome cycling track and<br />

Indian tribes, soldiers and pirates.<br />

spot the tropical birds and rare<br />

The Reluctant Kayaker<br />

Overcoming Adversity<br />
As I am sure you have surmised<br />
from my previous articles, I really<br />
don’t consider myself an athlete.<br />
I’m certainly no marathon runner<br />
or triathlete. Like most people,<br />
I engage in activities until I am<br />
tired or something hurts. Then it’s<br />
Miller and TV time! The “playing<br />
through the pain” concept is<br />
nothing I have ever embraced.<br />
Sometimes, however you have<br />
no choice. I first dealt with that<br />
concept in the Army when I went<br />
through basic infantry training. Of<br />
course, I was 21 years old and my<br />
only recourse for not dealing with<br />
it was court martial or suicide. So<br />
I dealt with it. In many ways it<br />
was enlightening as prior to this I<br />
thought “second wind” referred to<br />
an orchestra seating chart. Boy, did<br />
I find out differently.<br />
Another reason a person has to<br />
push beyond their self-imposed<br />
limits of fatigue and pain is if you<br />
have a serious injury that involves<br />
a lot of rehab therapy to regain<br />
your former physical status. This<br />
was my second bout with “playing<br />
through the pain” except now I<br />
am 68 years old. In many ways it<br />
was as enlightening as my Army<br />
experience mixed with liberal<br />
does of depression. I will briefly<br />
describe what happened to me.<br />
In Oct. of ’09 I had an accident<br />
which resulted in both of my<br />
quadriceps muscles tearing away<br />
from my knees. These muscles,<br />
as most of you know are the<br />

large muscles in front of your<br />
legs between the knee and the<br />
hip. Without these muscles, you<br />
cannot walk. I was admitted to<br />
the hospital, had MRI’s done, and<br />
seen by my orthopedic surgeon the<br />
following day. The prognosis: I<br />
probably wouldn’t walk again and<br />
even if my some miracle I did, I<br />
would always need a cane. How’s<br />
that for a life changing event! I<br />
said to the Doctor “You give me<br />
the wheels and I’ll drive the car”.<br />
Trust me, I sounded more positive<br />
than I actually felt. But, like the<br />
Army, I was going to focus and<br />
give it 110%. I was operated on<br />
2 days later and the torn muscles<br />
were reattached through holes<br />
drilled in my Patella’s. The<br />
muscles were also shortened<br />
because they were so badly torn.<br />
When I woke up after surgery,<br />
both knees were tightly bandaged<br />
and each leg was in a steel cast<br />
that extended from my hips to<br />
my ankle. This was, obviously,<br />
to prevent any movement during<br />
the expected 8 week healing<br />
process. The surgery went well but<br />
obviously, the proof of the pudding<br />
would be when the wounds healed<br />
and they put me on my legs for the<br />
first time.<br />
Now, you have to understand that<br />
I had never been an inpatient in<br />
a hospital before. Sure, I would<br />
visit people in the hospital and<br />
watched a lot of hospital shows<br />
on TV. <strong>May</strong>be that’s where I got<br />
my expectation spotlessly clean<br />
rooms filled with state-of-the-art<br />
equipment manned by competent,<br />
caring care givers. Boy was I<br />
wrong. I found not particularly<br />
clean rooms with old equipment<br />
and a staff, with some exceptions,<br />
pretty much indifferent. Of course<br />
since I could not walk or use a<br />
wheelchair, I was totally dependent<br />
on the staff for everything. Food,<br />
water, bathroom, etc. Just as an<br />

of hospital food. There’s nothing<br />
funny about it. I did discover,<br />
however, that the Kosher food<br />
was better than the regular fare.<br />
Of course, I had to hum Yiddish<br />
tunes my Grandmother taught me<br />
each time I was served but it sure<br />
beat the alternative. Thank God for<br />
supplements from my wife! After<br />
about 4 days I was transferred to<br />
an in-patient rehab center for the<br />
next phase of my ordeal. I am out<br />
of space for this column but will<br />
continue in the next issue. This is<br />
when the hard part starts.<br />

survival tips’from hank<br />
Chlorine<br />
Contaminated drinking water is real concern and can cause major<br />
complications to the unwary. Although there are many very good methods<br />
available out there to treat and filter water one very simple, tried and true<br />
method, is to use chlorine. That’s standard, unscented household bleach. It<br />
only takes 2 drops to treat a quart of clear water. For murky or discolored<br />
water a safe bet is to double that to 4 drops. You simply add the recommended<br />
drops, mix it by slowly rotating the container back and forth and wait at<br />
least 30 minutes before drinking. Be sure to sniff the water and make sure<br />
you can detect a slight odor of chlorine. If you don’t smell chlorine the safe<br />
thing to do is add another dose and try the sniff test again before drinking.<br />
Magnesium Fire Starter Bars<br />
Do not count on always being able to easily shave and collect that little<br />
pile of magnesium shavings. This is especially true when you find<br />
yourself in dire straights, cold and wet in a freezing rain, the wind is<br />
howling and you desperately need that fire. My advise, and practice by<br />
the way, is to always have a small pile of magnesium shavings stowed<br />
in my Personal Survival Kit, ready for immediate use. I carry mine in<br />
a small 2 inch x 2 inch zip lock bag so that I can see the contents plus<br />
the sealed bag keeps the shavings dry. I’d recommend carrying at least<br />
a tablespoon of shavings ready to use. When I use up that pile, the very<br />
first chance I get, I’ll refill it and have it ready for the next – “got to get<br />
that fire going now” situation. Remember folks – being prepared is the<br />
key to survival.<br />
Magnifying Lens<br />
Being somewhat on the “ always prepared” side I like to carry something<br />
I can use to start a fire with me at all times. One item that I’ve found,<br />
and started several fires with, is one of those plastic credit card size<br />
magnifying lens. You can get them at any Wal-Mart for about 6 bucks. I<br />
just stick it my wallet along with my credit cards and I’ve got a fire starter<br />
with me. The down side of course is that the sun must be shining. It has<br />
other uses obviously: reading fine print, picking splinters and spines out<br />
of your skin, threading a needle etc.<br />
Paracord<br />
Carry a minimum of 20 feet of paracord whenever you head outdoors.<br />
Some different ways to make sure you always have paracord at hand are<br />
to replace your shoelaces with 550 cord, wrap your knife handle with<br />
cord, wrap your water bottle or whatever you normally carry. Or – you<br />
could just carry a 20 foot hank in your backpack, ready for immediate<br />
use.<br />
Multi-use Tool<br />
I’m particularly fond of and always looking for survival tools that serve<br />
more than one purpose. One of the best I’ve found, and the one I’ve<br />
been carrying for a number of years now, is the Survivo II, 5 in 1. This<br />
simple 1 oz. tool has a Compass, Signal Mirror, Waterproof Match Case,<br />
Whistle, and Fire Starting Striker all in a pocket size package. It’s made<br />
of bright orange/red plastic so you can’t misplace it and comes with a<br />
lanyard to wear around your neck ( extra small line). It’s available from<br />
the Campmor catalog or their website for about $4.00 and can sometimes<br />
be found at <strong>Sports</strong> Authority, Gander Mountain, Bass Pro etc. Well worth<br />
the money folks.<br />
Tender<br />
Just like that rule of carrying a minimum of 3 fire starting devices make<br />
sure you have at least 3 very reliable tinders. I highly recommend these 3<br />
items: 1- cotton balls soaked in petroleum jelly, 2- alcohol swaps in blister<br />
packs, and 3- WetFire ( or similar) fire starting tinder. Each of these items<br />
has multiple uses, they work even when wet, they pack very small and<br />
they will work with either of your three fire starting tools.<br />
Stormproof Matches<br />
If any of you are carrying and depending on standard strike anywhere<br />
matches as one of your three fire starting options I encourage you to<br />
reconsider. Whether waterproofed or not these matches are VERY<br />
undependable and much more likely to just crumple than light. As a<br />
demonstration I routinely have my students try to strike one of these<br />
matches on something found in the field,: on a rock, on their knife, on a<br />
tree stump, anything commonly found in the woods, and to date, not one<br />
has worked. I’m not sure where that strike anywhere idea came from<br />
but –don’t bet your life on it. The only matches I now recommend are<br />
the “Stormproof Matches” made by Industrial Revolution. They are not<br />
strike anywhere but each box comes with two extra strikers in addition to<br />
the ones on each side of the box.. When ignited the tip burns for a full 15<br />
seconds before it reaches the wood part of the match, that’s compared to<br />
maybe 2 seconds with a regular match and they are darn near windproof(<br />
the best I’ve ever found) and very waterproof. I bought some at<br />
Gander Mountain in the camping section but I’m sure they are available<br />
elsewhere.<br />
Remember to always: Hope for the Best but Prepare for the Worst!<br />

A PERFECT DAY<br />
Sometimes, you just know! You<br />
plan a spearfishing trip and all the<br />
intangibles fall into place. The<br />
weather behaves, the sea is cement<br />
flat, and the visibility is 50<br />
to 60 feet.<br />
The plan was simple. Four<br />
members of the Broward<br />
Longfins Spearfishing<br />
Club were meeting at the<br />
Coconut Grove boat ramp<br />
at 0700 hrs on a beautiful<br />
Saturday in March. Our<br />
destination: the reefs South<br />
of Fowey Lighthouse and<br />
East of Elliot Key.<br />
I left my house and drove<br />
to the Hollywood Blvd.<br />
Tri-Rail parking lot, where<br />
Roberto Ramos awaited<br />
my arrival. Roberto is a<br />
Long fins club member that<br />
has qualified for the <strong>2011</strong><br />
U.S. National spearfishing<br />
competition. We quickly<br />
loaded his gear in the back<br />
of my Chevy truck. The<br />
drive to the marina is filled with<br />
anticipation and promise. The<br />
familiar routine is played out<br />
among all spearfishermen. What<br />
gun are you using? Float or<br />
reel? You think it’s too warm<br />
for the wetsuit? I don’t care if I<br />
shoot anything I’m just glad to be<br />
diving! We arrived at the boat<br />
ramp to find Richi Davalos and<br />
Deivy Fernandez launching the<br />
21ft Chris Craft.<br />
Mishka’s c.d. “ Above the Bones”<br />
played in the background as we<br />
navigated the maze of moored<br />
yachts and sailboats in the<br />
channel. The cool air was still<br />
and the ocean a vibrant royal blue.<br />
We arrived at the first reef which<br />
was 30 ft. deep with extensive<br />
structures: deep caves and large<br />
outcropping supported a plethora<br />
of marine life. I prefer diving<br />
these shallower reefs first to allow<br />
for the mammalian dive reflex.<br />
Once I am warmed up I proceed to<br />
the deeper reefs.<br />
It didn’t take long for Richi<br />
SUBSURFACE<br />
to surface with two nice male<br />
Hogfish. Before he could get<br />
back in the water Roberto surfaced<br />
with a large Mutton snapper and<br />
Triggerfish. Deivy surfaced with<br />
a Hogfish and a lobster. On my<br />
dive I grabbed a large lobster from<br />
underneath a ledge. I spotted a<br />
large male green turtle gliding<br />
effortlessly as he scanned the<br />
reef. I swam alongside and<br />
watched its grace and power.<br />
Whenever I spot one of theses<br />
majestic creatures, I always have a<br />
spectacular day like a premonition<br />
or a sign.<br />
The reefs looked healthy and alive<br />
with marine life. The large rock<br />
structures were covered with large<br />
sea fans and vegetation. The sand<br />
gaps between the rocks create<br />
channels that hold hogfish and<br />
other snappers. In one of these<br />
channels I spotted a large Goliath<br />
grouper lying listlessly against a<br />

ledge. I swam towards him and<br />
within a body length distance it<br />
released several strong guttural<br />
soundings that vibrated the<br />
water. This defense mechanism<br />
was repeated several times and<br />
with increasing intensity. The<br />
shock waves were powerful<br />
and effective. It calmly swam<br />
away confident I had received its<br />
warnings.<br />
With several fish<br />
harvested, we decided<br />
to move South, in an<br />
effort to avoid excessive<br />
pressure and to find<br />
deeper water. We agreed<br />
to dive in a 60 to 70 foot<br />
range, with the hope of<br />
finding bigger fish. Due<br />
to the strong current, a<br />
drift dive was our only<br />
option. Richi agreed to<br />
stay with the boat while<br />
Deivy, Roberto and I<br />
dove. I spent several<br />
minutes on the surface<br />
breathing up and slowing<br />
my heart rate. On my<br />
first approach I spotted<br />
3 large Mutton snappers<br />
at 40 feet. I followed<br />
the largest one along the<br />
bottom of the reef in 65<br />
feet of water. It came to a<br />
stop at the edge of the reef where<br />
the sand started. I braced myself<br />
against a rock with my left hand<br />
while I eased my gun forward<br />
with my right. I could feel the<br />
current trying to push me forward.<br />
I flattened my fins against the<br />
bottom and allowed the currents<br />
pressure to wedge me into the reef.<br />
The Mutton was 10 feet ahead. I<br />
had to be patient and wait for a<br />
perfect lateral shot.<br />

It sensed my presence and turned<br />
its body slightly to look for me. I<br />
fired my Mako 110cm spear-gun<br />
and watched the spear hit its mark<br />
and the fish was dispatched with<br />
one shot. The only struggle on<br />
the ascent was the weight of the<br />
fish.<br />
Once on the boat I marked my fish<br />
with a slit to the tail and placed<br />
it in the cooler with the rest. It is<br />
common practice to distinguish<br />
one’s catch from the others with<br />
such a mark. Meanwhile Roberto<br />
found two large Mutton and Deivy<br />
another.<br />
The sun was nearing the horizon<br />
and lost its intensity. As the<br />
orange hue faded in the distance<br />
we headed north for shallower<br />

water and set ourselves up for<br />
another drift dive with Deivy<br />
at the helm. We drifted North<br />
towards Fowey Lighthouse.<br />
The reef was 40 feet below. I<br />
dispatched two Hogs and another<br />
lobster while Richi boated a<br />
large hog and mangrove snapper.<br />
Roberto came in with another<br />
Mutton and two lobsters. Deivy<br />
switched with Richi and quickly<br />
found another Hogfish on a sandy<br />
patch below a large outcropping.<br />
I spotted a large silhouette in<br />
the distance approaching me.<br />
As it neared, I recognized its<br />
unmistakable shape. The 8<br />
foot hammerhead shark circled<br />
me with a long and purposeful<br />
pass. Suddenly the predator had<br />
become the prey. It is a humbling<br />
experience to be stalked by a large<br />

predator. It seems once we leave<br />
the concrete jungle we relinquish<br />
control. On the second pass, it<br />
tilted its odd-shaped head to look<br />
me over. I kept my loaded gun<br />
pointed at him and unimpressed,<br />
it swam away. Grateful, I swam<br />
what seemed the longest 100 feet<br />
ever to the boat.<br />
As I boarded, Richi was hard<br />
at working making his fathers<br />
famous Peruvian ceviche.<br />
He prepared this Latin delicacy<br />
with fresh snapper, onions,<br />
cilantro, lemon juice, and a few<br />
secret ingredients I promised<br />
not to disclose. The four of us<br />
ate ceviche and drank ice cold<br />
beverages while the boat aimlessly<br />
drifted with the current.<br />
A perfect day: we had excellent<br />

conditions, good friends, good<br />
food and a bountiful harvest.<br />
Not all days come together like<br />
this one did. That’s a good<br />
thing because life would lose its<br />
wonder and surprise if it did. An<br />
adventure exists because we don’t<br />
know its outcome. I was grateful<br />
that my premonition had come<br />
true.<br />
That evening I made a vegetable<br />
Mutton snapper soup for my<br />
family. My son Dylan had two<br />
huge servings. As we said grace<br />
I gave thanks for many things:<br />
family, friends and health. It was<br />
a perfect day.<br />

An obstacle course race is designed<br />
to test your resilience,<br />
strength, stamina, quick decision<br />
making skills, and ability to laugh<br />
in the face of adversity.<br />
Down and Dirty<br />
The Merryl -Down and Dirty<br />
Mud Event was held on <strong>May</strong> 1st<br />
at the Zoo <strong>Miami</strong>. Participants<br />
raced a 5k or 10k obstacle course<br />
alongside local Military personnel,<br />
Public Safety Officers and Police<br />
Academy trainees through military-style<br />
obstacles, commanding<br />
cargo climbs, wild water crossings,<br />
mud, mud, and more mud!<br />
The 10K overall winners were Tim<br />
Nichols of Cooper City, FL coming<br />
in at a fast 41:41 and the fastest<br />
female was Lourdes Hernandez<br />
47:47. The Race ended with food,<br />
music and an awesome Tech Tee.<br />
Super Spartan<br />
The Super Spartan Race <strong>Miami</strong><br />
was held at Oleta River State<br />
Park on April 23rd <strong>2011</strong>. It was<br />
a grueling 8+ mile course with<br />
15+ obstacles to test the physical<br />
strength and mental resolve of it’s<br />
participants. The race began at<br />
8:30 am with groups taking off every<br />
30 minutes in waves. Eighty-<br />
Five warriors ran the course which<br />
included hanging cargo nets, water<br />
crossings, mud runs, double and<br />
single tracks, barbed wire, a rope<br />
wall, steep slopes and running<br />
the gauntlet past burly Gladiators<br />
before a reaching the finish line.<br />
Matthew Cone of Yulee, FL came<br />
in first place in the men’s division<br />
at 48:56.7 with Maria Morales of<br />
Wilton Manors, Fl finishing first in<br />
the women’s division at 1:08:13.1.<br />
After they completed the painful<br />
journey, there was live music and a<br />

downwinders<br />

I first entered the world of<br />
kiteboarding over ten years<br />
ago with several lessons on the<br />
beaches, and eventually in the<br />
waters of Crandon Park, <strong>Miami</strong>,<br />
Florida.<br />
The relatively new sport appealed<br />
to me for the perceived sense of<br />
freedom I imagined it offered. I<br />
had visions of throwing my gear in<br />
the car and heading out for wind<br />
powered journeys − island hoping,<br />
and backpacking/kiteboarding<br />
expeditions.<br />
At the time, directional boards,<br />
which had evolved from the<br />
sport’s early use of modified<br />
surfboards were still common.<br />
In fact, the first time I attempted<br />
to actually ride, I began from<br />
a sandbar, powered up the kite<br />
and stepped into the straps of the<br />
surfboard style board and took<br />
off! I was pumped! The easy to<br />
plane and optional straps made the<br />
whole board thing a piece of cake.<br />
Then things took a turn for the<br />
worse.<br />
I decided to invest in my own<br />
gear. Following the advise of<br />
friends and instructors, I purchased<br />
a Cabrinha 14 and a twin tip<br />
board. The unidirectional boards<br />
were falling out of fashion and<br />
considered old school- apparently.<br />
I didn’t know it at the time but I<br />
had entered into the sport when<br />
wakeboarding was heavily<br />
influencing everything from<br />
boards to bindings and even riding<br />
style.<br />

By the time I got back out with<br />
my new gear the scene on the<br />
beach looked like a wakeboarder<br />
convention without boats.<br />
I tried to embrace the sport and<br />
master upwind techniques and<br />
tricks, but after a few overly<br />
successful air launches followed<br />
by far less successful landings,<br />
I was loosing interest. I was<br />
no closer to my wind powered<br />
expeditions than months before. I<br />
questioned everyone about boats,<br />
island hopping and “kitepacking”.<br />
I was met with a myriad of reasons<br />
as to why I couldn’t do such<br />
things. Several just looked at me<br />
with dumb expression when I<br />
mentioned “kitepacking”.<br />
I wasn’t interested in pulling tricks<br />
for spectators on the beach to see.<br />
That seemed more like “skate<br />
park” or BMX action. I was<br />
looking for more of an endurance,<br />
adventure, expedition sort of thing<br />
(what is called “downwinders”<br />
today). I found no one to embrace<br />
the idea. So I abandoned the sport.<br />
In the ten plus years that have<br />
elapsed since my foray into the<br />
sport it seems there have been a<br />
growing number of like-minded<br />
individuals that want to get their<br />
kiteboarding on, without the need<br />
for precision aeronautical stunts.<br />
The big attraction to these<br />
downwinder excursions is that<br />
newbies can actually get some<br />
kiteboarding done, before they<br />
fully master the sport.<br />

To accommodate these wise<br />
individuals, area kiteboarding<br />
shops are arranging downwinders<br />
in the form of water taxies and<br />
even kiteboarding safaris.<br />
<strong>Miami</strong> Kiteboarding offers water<br />
taxi service on any day with wind.<br />
The most common trip is to launch<br />
from Crandon Park and get picked<br />
up in Stiltsville or Soldier Key.<br />
These guys are full on committed<br />
to the concept with “Kite Safaris”<br />
going to Caribbean locations.<br />
When I shared my ideas of<br />
“kitepacking” with owner<br />
Christophe, he suprised me by<br />
informing me of some “out and<br />
back” trips to Boca Chita he<br />
has in the works. These trips<br />
are supported by boat, but he<br />
seemed down with the idea of an<br />
unsupported, self contained trip.<br />

downwinder / water taxi service<br />
from their <strong>Miami</strong> Beach location.<br />
Kiters start at X-treme kite’s<br />
beach side location at 7330 Ocean<br />
Terrace and head to South Beach,<br />
usually taking out at around 10th<br />
Street. These guys can set up taxi<br />
services anywhere in the <strong>Miami</strong><br />
area.<br />
Most area shops also offer similar<br />
services. Give them a call and<br />
see what they can set up for you,<br />
just be careful when metioning<br />
“kitepacking”.<br />

room for Platelet Rich Plasma<br />

Spend enough hours on a road<br />
bike or logging miles preparing<br />
for the next marathon and<br />
chances are at some point you<br />
will encounter a chronic overuse<br />
injury. If the usual remedies<br />
such as ice and Advil don’t<br />
get the job done, Platelet Rich<br />
Plasma (PRP) injections may get<br />
you back into the game faster.<br />
Developed in Spain in 2002, PRP<br />
is growing in popularity with<br />
athletes to treat a wide range of<br />
injuries and ailments.<br />
How it Works<br />
The body has an amazing capacity<br />
to heal itself. When the body<br />
becomes injured, a natural healing<br />
process occurs to repair the<br />
damaged tissue. The body signals<br />
platelets and other components<br />
in our blood supply to migrate to<br />
the site of injury. Under normal<br />
conditions, these platelets release<br />
a variety of factors that initiate<br />
and subsequently promote healing.<br />
A relatively new technique has<br />
been developed to harness and<br />
concentrate these platelets to be<br />
precisely introduced to the injury<br />
site in an injectable form. The<br />
implantation of these platelets<br />
from a small amount of the patient’s<br />
own blood has the poten-<br />
tial to “supercharge” the body’s<br />
capacity for healing.<br />
How its’ Done<br />
A small amount of blood is<br />
drawn from the patient into a<br />
sterile tube in the exact same<br />
manner as a standard blood sample.<br />
The tube containing the patient’s<br />
blood<br />
is placed into<br />
a centrifuge<br />
and spun to<br />
separate the<br />
platelets from<br />
the other<br />
blood components.<br />
After a<br />
few minutes,<br />
the concentrated<br />
platelets<br />
are removed<br />
from the same<br />
tube and reintroduced<br />
(injected) into<br />
the patient at<br />
the site of the<br />
injury. Using<br />
a sterile<br />
needle, the<br />
PRP is injected<br />
in and<br />
around the<br />
injury site. This is all accomplished<br />
without using any foreign<br />
material, although some manufactures<br />
utilize an anti-coagulate<br />
“pre-loaded” in the vials.<br />

We recently “looked on” as our<br />
contributing Sport’s Medicine<br />
Physician, Dr. Kalbac, administerd<br />
a PRP treatment to a patient.<br />
The entire process was completed<br />
in about ten minutes.<br />
Six of these minutes were the<br />
“spin time” required to isolate<br />
the plasma. The only time the<br />
patient “felt a small sting” was<br />
during the initial numbing injection<br />
which is administered before<br />
the plasma is re-introduced. This<br />
allows the physician to place the<br />
PRP exactly<br />

without discomfort<br />
to<br />
the patient.<br />
Dr. Kalbac<br />
began utilizing<br />
PRP two<br />
years ago<br />
and said “the<br />
use of PRP is<br />
an exciting<br />
new technology<br />
we have<br />
for treating<br />
sports<br />
injuries, both<br />
operative<br />
and nonoperative.”<br />
He considers<br />
it so beneficial<br />
that he<br />
utilizes the<br />
technique for all of his surgery<br />
patients. For this application,<br />
blood is drawn pre-op and the<br />
PRP is injected post surgery.<br />

The Science Behind PRP.<br />
Most of the studies we reviewed<br />
for the article where related to<br />
chronic overuse injuries common<br />
to athletes. The studies evaluated<br />
the use of Platelet-Rich Plasma<br />
(PRP) to address these conditions.<br />
A double blind, randomized<br />
controlled trial evaluated<br />
the use of PRP for chronic lateral<br />
epicondylitis (tennis elbow)<br />
which resulted in reduced pain<br />
and increased function, exceeding<br />
the effect of the group treated<br />
with a corticosteroid injection.<br />
Another study evaluated the<br />
use of a single PRP injection in<br />
patients with painful medial or<br />
lateral epicondylitis for at least 6<br />
months that were unresponsive<br />
to other nonsurgical treatments,<br />
including a corticosteroid injection.<br />
The results suggest that a single<br />
PRP injection can improve pain<br />
and function in patients who<br />
failed other types of nonsurgical<br />
treatment, thus avoiding surgery.<br />

Is it Safe?<br />
For over twenty years, PRP has<br />
been used in many different<br />
fields of medicine including: cardiac<br />
surgery, oral surgery, dentistry<br />
and periodontal implants,<br />
orthopaedics, wound care, sports<br />
medicine, neurosurgery, general<br />
surgery and cosmetics. 1 The research<br />
and clinical data we found<br />
mostly<br />
supported<br />
the manufacturer’s<br />
safety<br />
claims. 2,3<br />
Because<br />
the platelets<br />
are<br />
produced<br />
from<br />
your own<br />
blood,<br />
there is<br />
no risk of<br />
rejection or<br />
disease transmission. There does<br />
seem to be a small risk of infection,<br />
however it is very rare and<br />
no greater than any other type of<br />

What would the Tour de<br />
France say about this one?<br />
It seems to be safe and effective,<br />
but is it considered “blood<br />
doping”? Blood doping is basically<br />
the exact opposite of this<br />
procedure. The purpose of blood<br />
doping is to isolate, store and<br />
re-introduce red bloods cells to<br />
enhance oxygen carrying capacity<br />
of an athlete’s blood. We<br />
didn’t have enough time/space to<br />
follow this angle in depth, but we<br />
promise explore it more in the<br />
near future.<br />
How fast does it Work?<br />
In two different studies evaluating<br />
the use of PRP for the treatment<br />
of<br />
tennis elbow,<br />
both<br />
showed<br />
a gradual<br />
improvement<br />
at one<br />
month,<br />
three<br />
months<br />
and six<br />
months.<br />
These<br />
studies<br />
along with<br />
other studies treating different<br />
conditions suggest a reduction in<br />
pain and improvement in function.<br />
PRP delivered via<br />
injection directly to<br />
the problem area.<br />
The “spin” on most websites<br />
claim you may notice improvements<br />
after one injection but,<br />
“additional injections may be<br />
necessary.”<br />
What say we?<br />
This seems to be a better option<br />
than the overused, go to<br />
treatment of choice for the last<br />
several decades- cortisone. It is<br />
certainly worth exploring before<br />
surgery, but don’t take our word<br />
for it, check out the following<br />

Although this starts out as a<br />
basketball story, it pertains to any<br />
athlete that runs in their sport<br />
or runs when they train. A few<br />
years ago I had a pro-basketball<br />
center that presented to my<br />
office with complaints of lateral<br />
right knee pain. He described a<br />
motion where he is passed the<br />
ball, plants his right foot and<br />
hooks around as he jumps to<br />
shoot the ball. He does this again<br />
and again throughout practice<br />
and during a game. The history<br />
certainly pointed to a potential<br />
knee injury. However, after a<br />
more in-depth history and an<br />
examination of the right knee<br />
(and examination of the left knee<br />
for comparison), I determined<br />
that his knee pain wasn’t really<br />
caused by a problem with his<br />
knee. Next on up the kinetic<br />
chain was the hip, sacroiliac and<br />
lower back. Everything in the<br />
hip & lower back were normal<br />
but it was the sacroiliac joints<br />
where I eventually discovered the<br />
problem.<br />
Upon examining the knee there<br />
was some pain in the distal aspect<br />
of his lateral knee (the outside of<br />
the knee). Yes it was tender and<br />
sore, but the orthopedic exam of<br />
the knee was absolutely normal<br />
– as was the examination of his<br />
hips and lower back. When I<br />
examined the sacroiliac joints,<br />
all standard orthopedic tests were<br />
normal and didn’t cause any pain<br />
in the back, hip or knee. Then I<br />
did a technique quite common to<br />
chiropractors, known as motion<br />
palpation. This is where we apply<br />
a little repetitive springy motion<br />
into a joint to see if it has the<br />
normal springy end feel that a<br />
properly functioning joint should.<br />
When I applied this examination<br />
technique to the sacroiliac joint,<br />
the joints had a hard or locked<br />
end feel father than the springy<br />
motion of a normally functioning<br />
joint. Although there are several<br />
causes for I.T.-Band Syndrome,<br />
it’s typically thought to be from<br />
overuse, misuse, overtraining<br />
syndrome (not giving the body<br />
enough time to recover between<br />
training sessions), returning<br />
from an injury too soon or faulty<br />
biomechanics.<br />
Although the sacroiliac (S.I.)<br />
joints don’t move much, they<br />
are supposed to move a little at<br />
the end range of your walking<br />
and running gate. Both S.I.<br />
joints should flex and extend<br />

the S.I. joint locks up, the TFL<br />
and ITB pull with increased force<br />

A Case Of Lateral Knee Pain In An Athlete Without A “Knee Problem”<br />
overstressed and ultimately<br />
inflamed.<br />
It’s my theory that when the<br />
S.I. joints lock up it prevents the<br />
normal pendulum like swing of<br />
your legs and thus the TFL/ITB<br />
will help move it laterally, pulling<br />
slightly on the lateral structure of<br />
the knee, ultimately causing the<br />
inflamed distal I.T. band. Sounds<br />
good, but again, that’s my theory.<br />
In either case, the problem is<br />
occurring at the sacroiliac joints<br />
(part of your lower back) and<br />
not at the knee. Remember what<br />
I said initially, both the knee<br />
exam and the low back exam<br />
were virtually normal. Any<br />
physician would have examined<br />
it and found it normal. This is<br />
why it’s important to take a good<br />
history (know the sport and how<br />
the injury occurred) and do a<br />
good physical exam that includes<br />
classic orthopedic test but also<br />
examines the soft tissues AND<br />
the intrinsic motion within a joint<br />
complex. Without this last part,<br />
this problem would never have<br />
been properly identified.<br />
This Iliotibial band friction<br />
syndrome (I.T. Band Syndrome)<br />
is commonly seen in runners,<br />
triathletes and weight lifters that<br />
include a lot of squatting in their<br />
routines. Everyone that comes in<br />
with this swears they have a torn<br />
meniscus “or something” wrong<br />
in their knee. But buyer beware,<br />
this problem can present itself as<br />
low back pain, hip pain, upper<br />
thigh pain or lateral knee pain.<br />
What’s the best way to tell the<br />
difference? Get an examination<br />
by a competent sports medicine<br />
physician - AND always ask<br />
for a detailed explanation as to<br />
why you are in pain. If they<br />

can explain the mechanism of<br />
the problem to you clearly, that<br />
means they most likely have<br />
a good understanding of your<br />
problem and you’re in the right<br />
place.<br />
Remember…pain is merely a<br />
symptom that alerts you to the<br />
existence of a problem. It doesn’t<br />
necessarily tell you what the<br />
problem is or where the problem<br />
is. Most of you are aware that<br />
a heart attack can present itself<br />
as pain in the left arm or left<br />
shoulder blade and not in your<br />
chest. In this case we saw<br />
lateral knee pain being caused<br />
by something other than a knee<br />
problem.<br />
So let me finish with this, while<br />
the internet can provide you with<br />
some background on your aches<br />
and pain, nothing replaces a<br />
good ole’ fashion medical history<br />
and physical examination. If<br />
you continue training and you<br />
continue to experience pain, don’t<br />
wait too long to get it looked<br />
at by a trained sports medicine<br />
professional. The faster you find<br />
the right answer, the faster you’ll<br />
find the right solution.<br />

As a SUP or kayaker: more<br />

Start Position:<br />
Begin in a push-up position<br />
so that the lower third of your<br />
shins and the top of your feet are<br />
resting on the physio-ball.<br />
Motion 1:<br />
Lift your right foot off the<br />
physio-ball, bring your knee<br />
forward, drop your right hip and<br />
bring your right foot under your<br />
stomach so that it is pointing<br />
to your left shoulder. Return it<br />
back to the start position on the<br />
physio-ball.<br />
Motion 2:<br />
With both feet back on the ball<br />
(stat position) lift your hips up<br />
into the air while bringing your<br />
feet closer to your hands until<br />
your wrist, shoulders, and hips<br />
are aligned vertically. Then<br />
return to the start position.<br />
Now repeat Motion 1 lifting<br />
the left leg off the physio-ball.<br />
Complete Motion #1 then move<br />
on to Motion 2. Keep alternating<br />
legs for increased core activation.<br />
Perform 2 sets of 5 each leg<br />
prior to your sport to activate<br />
and recruit the necessary neural<br />
patterns. You should notice more<br />
stability and precision with your<br />
movements.<br />

month it is all about flexibility.<br />
Part III Keeping it<br />
Flexible..<br />
Can you lie on your back and<br />
lift your leg straight up in the<br />
air? Probably not, if you’ve been<br />
following the program. That’s<br />
because building endurance will<br />
make your hamstrings tight,<br />
truncate your hip flexors and bow<br />
your upper body, depending on<br />
your activity of choice. What you<br />
are, Gertrude, is literally bent out<br />
of shape by the demands of your<br />
sport.<br />
See, training gets you out of shape<br />
more than in as those untold repetitions<br />
start limiting your range<br />
of motion and impede your ability<br />
to access all the strength you’re<br />
developing. Cardiovascular conditioning<br />
in no way obviates being<br />
the equivalent of ‘muscle-bound’.<br />
So, to get strong, be flexible. And<br />
you don’t worry about your performance<br />
suffering when you’re<br />
undoing all that tightness, because<br />
flexibility allows you to push the<br />
envelope further.<br />
If you’re diligently following<br />
this program then strength- and<br />
endurance-training have made you<br />
pretty inflexible. Counter that with<br />
a stretching program that incorporates<br />
techniques and poses for<br />
building eccentric strength—in<br />
weight-room parlance, your negative<br />
reps. You’ll also need to train<br />
core muscles with multiple-joint<br />
movements that utilize front-toback,<br />
side-to-side, and rotational<br />
planes of motion.<br />
And remember to always warm<br />
up not by stretching but by doing.<br />
You can stretch later. (During is<br />
OK, if you’re strength-training.)<br />
Boosting your athleticism may<br />
mean taking a page from yoga to<br />
help your flexibility, breathing and<br />
balance. Increase these areas and<br />
you’ll find yourself able to extend<br />
and pull back repeatedly without<br />
getting hurt. The end result is more<br />
positions you can be powerful in!<br />
Use controlled breathing that<br />
involves inhaling deeply, filling<br />
your rib cage, and allowing you<br />
to slowly exhale through the nose<br />
(constraining and pressurizing<br />
airflow) to generate internal heat;<br />
this allows reaching deeper when<br />
you stretch. It slows the heart<br />
rate, decreases tension and blood<br />
pressure—even increases V02<br />
max, the standard gauge of cardio<br />
efficiency—and increases mental<br />
clarity, too! Breathing this way<br />
when you stretch bestows a calmness<br />
helpful for dealing with pain<br />
when, say, you’re pushing through<br />
the wall. You’ll stay more relaxed<br />
in stressful situations, in or out of<br />
sports.<br />
More lean muscle means the<br />
more bending becomes difficult.<br />
Without turning yourself into a<br />
pretzel—stretching isn’t a competition—follow<br />
these stretches (best<br />
done after a warm-up) to help keep<br />
you balanced and strong.<br />

Always remember to breathe.<br />
Over time, this becomes subconscious,<br />
part of what you do. Inner<br />
focus like this translates over to<br />
sports, and getting into this breathflow<br />
zone will center you.<br />
Follow the series below of flexibility<br />
stretches as a cooling-down<br />
adjunct to your endurance work<br />
three days a week. Following<br />
periodization protocol, functionaltraining<br />
takes a back seat to allow<br />
time to rest for next month’s speed<br />
and power work. For that you’ll<br />
need your ligaments and tendons<br />
really flexible to maximize muscle<br />
performance.<br />
To help you out, we’ve scavenged<br />
the yoga bin for some of their best<br />
stuff:<br />
Forward Fold/Good Mornings<br />
A great warm-up that stretches<br />
hamstrings, calves and hips and<br />
strengthens legs and knees.<br />
Keep your feet hip-width apart<br />
while hinging hips forward and<br />
lowering your torso towards the<br />
floor. Bend knees while grasping<br />
your elbows with opposite hands.<br />
Breathe deeply, and relax; release<br />
tension. Hold for one minute, before<br />
returning to standing.<br />
Downward-Facing Dog/Modified<br />
Plank Releases tightness in your<br />
lower back and stretches the feet,<br />
shoulders, hamstrings and calves<br />
while strengthening upper body<br />
and core. Torches abdominal fat<br />
while strengthening your thighs<br />
and legs.<br />
Assume a hands-and-knees<br />
position and place your hands<br />
shoulder-width apart with fin-<br />

gers spread. Lift your knees and<br />
straighten your legs. Lengthen<br />
pose by walking your hands forward<br />
and feet backward. Squeeze<br />
your thighs and press them back<br />
towards the wall. Pressing your<br />
heels back and down, gaze between<br />
your feet. Suck in your abs,<br />
while engaging your core. Breathe;<br />
hold 3 minutes. Repeat.<br />
Chair/Squat Stretches shoulders<br />
and chest; strengthens thighs,<br />
calves, spine and ankles.<br />
Stand with feet together, and<br />
inhale while raising your arms,<br />
palms facing, parallel to head and<br />
neck. Exhale, bending knees while<br />
sitting down and back. Keep your<br />
shoulders back, and prevent knees<br />
from extending past toes. Continue<br />
to exhale while ‘sitting’ deeper until<br />
your thighs are parallel with the<br />
floor. Keep core muscles engaged<br />
while keeping knees and thighs<br />
together. Hold for 30 seconds.<br />
Crescent Lunge A pre-workout<br />
routine for loosening and opening<br />
the hips; stretches groin; strengthens<br />
your arms and legs.<br />
Assume a hands-and-knees position.<br />
Step right foot between hands<br />
while keeping your right knee<br />
directly over your heel. Inhale,<br />
slowly lifting back knee off the<br />
floor. Press your heel back towards<br />
the wall while straightening the<br />
back leg. Keep your spine elongated;<br />
hold and breathe. Letting<br />
your hips fall forward, continue to<br />
press your back heel behind. Hold<br />
one minute, repeat on other side.<br />
Do each side twice.<br />

Perform one-leg forward lunges and side<br />
lunges two times a week, and add in<br />
upright rows, Swiss-ball flies, bent-over<br />

your sport to activate and recruit the<br />
necessary neural patterns. You should<br />
notice more stability and precision with<br />
your movements.<br />
