Names of all the authors of this article in the order they should appear on the published article. Include<br />

credentials (PA-C, etc) for each author. Identify the corresponding author.<br />

Work affi liations for each author as these should be listed on the published article. Include, as appropriate,<br />

position title, institution or practice, city, and state. If an author does not have a formal title, describe what<br />

the person does: eg, John Smith practices family medicine at….. (As much as possible, the work affi liations<br />

should make clear to the reader each author’s experience/knowledge in the topic area.)<br />

• Images must be jpeg, tiff, or eps format.<br />

• If the pathology on the image will not be clearly obvious to readers who don’t work in your specialty, submit<br />

a duplicate copy of each image with arrows or other markings to highlight the area of interest. These<br />

duplicates may be PowerPoint fi les or images embedded in a Word fi le.<br />

• Submit large fi les in individual emails, contact the editor for information on uploading to our ftp site, or<br />

mail them on a CD.<br />
