Names of all the authors of this article in the order they should appear on the published article. Include
credentials (PA-C, etc) for each author. Identify the corresponding author.
Work affi liations for each author as these should be listed on the published article. Include, as appropriate,
position title, institution or practice, city, and state. If an author does not have a formal title, describe what
the person does: eg, John Smith practices family medicine at….. (As much as possible, the work affi liations
should make clear to the reader each author’s experience/knowledge in the topic area.)
Contact information for corresponding author. At minimum, an email address and a telephone number
where we can reach you directly are required.

duplicates may be PowerPoint fi les or images embedded in a Word fi le.
• Submit large fi les in individual emails, contact the editor for information on uploading to our ftp site, or
mail them on a CD.
