POLICY FOR ALTERNATE PATHWAYS TO CERTIFICATION IN DIAGNOSTIC RADIOLOGY<br />

AND SUBSPECIALTY CERTIFICATION FOR INTERNATIONAL MEDICAL GRADUATES<br />

INTRODUCTION<br />

Physicians who have taken their radiology residency tra<strong>in</strong><strong>in</strong>g outside the United States or<br />

Canada are considered <strong>in</strong>ternational medical graduates (IMGs). If they have completed a<br />

cl<strong>in</strong>ical year and have been fully tra<strong>in</strong>ed and qualified <strong>to</strong> practice <strong>in</strong>dependent diagnostic<br />

radiology <strong>in</strong> their country of diagnostic radiology tra<strong>in</strong><strong>in</strong>g, they are eligible <strong>to</strong> pursue<br />

<strong>certification</strong> by the <strong>American</strong> <strong>Board</strong> of Radiology through either of two <strong>pathways</strong>: the<br />

Standard Pathway or the Alternate Pathway.<br />

STANDARD PATHWAYS<br />

Standard Pathway <strong>to</strong> Diagnostic Radiology Certification<br />

Successful completion of ACGME-accredited tra<strong>in</strong><strong>in</strong>g (or Royal College of Physicians and<br />

Surgeons of Canada equivalent) <strong>in</strong> diagnostic radiology, followed by a letter of attestation of<br />

the candidate’s read<strong>in</strong>ess from the tra<strong>in</strong><strong>in</strong>g program direc<strong>to</strong>r, is the standard by which<br />

candidates ga<strong>in</strong> entry <strong>to</strong> the ABR exam<strong>in</strong>ation process. ACGME-accredited tra<strong>in</strong><strong>in</strong>g <strong>in</strong> a<br />

diagnostic radiology residency may or may not <strong>in</strong>clude a fellowship (either ACGME-<br />

accredited or non-accredited).<br />

Standard Pathway <strong>to</strong> Subspecialty (“Certificate of Added Qualification, or CAQ”)<br />

Certification<br />

ABR diagnostic radiology tra<strong>in</strong><strong>in</strong>g, followed by successful completion of an ACGME-<br />

accredited fellowship program <strong>in</strong> the subspecialty, a letter of attestation from the tra<strong>in</strong><strong>in</strong>g<br />

program direc<strong>to</strong>r, and a year of cl<strong>in</strong>ical practice <strong>in</strong> all subspecialties except nuclear radiology,<br />

is the standard by which candidates ga<strong>in</strong> entry <strong>to</strong> the subspecialty exam<strong>in</strong>ation process.<br />

Note: A year of tra<strong>in</strong><strong>in</strong>g <strong>in</strong> an ACGME-accredited fellowship program that is<br />

completed be<strong>for</strong>e diagnostic radiology tra<strong>in</strong><strong>in</strong>g is not recognized by the ABR as<br />

satisfy<strong>in</strong>g this requirement <strong>for</strong> entry <strong>in</strong><strong>to</strong> the subspecialty certify<strong>in</strong>g exam via the<br />

standard pathway <strong>for</strong> nuclear radiology; however, a fellowship taken be<strong>for</strong>e<br />

diagnostic radiology tra<strong>in</strong><strong>in</strong>g does satisfy this requirement.


ALTERNATE PATHWAYS<br />

<strong>The</strong> Alternate pathway is available at <strong>in</strong>stitutions with an ACGME-accredited residency<br />

tra<strong>in</strong><strong>in</strong>g program and requires the sponsorship of the department chair of the <strong>in</strong>stitution <strong>in</strong><br />

which the IMG is a faculty member or fellow. <strong>The</strong> department chair establishes a<br />

prospective four-year plan with the <strong>in</strong>dividual, attests <strong>to</strong> the plan <strong>in</strong> writ<strong>in</strong>g <strong>to</strong> the ABR, and<br />

moni<strong>to</strong>rs and reports <strong>to</strong> the ABR on a yearly basis the <strong>in</strong>dividual IMG’s progress on the path<br />

<strong>to</strong> <strong>certification</strong>. <strong>The</strong> required admission process <strong>to</strong> the <strong>alternate</strong> pathway <strong>to</strong> ABR<br />

<strong>certification</strong> is (1) documentation of tra<strong>in</strong><strong>in</strong>g and <strong>certification</strong> obta<strong>in</strong>ed <strong>in</strong>ternationally, as<br />

described on the ABR website at http://theabr.org/ic/ic_dr/ic_dr_imedgrad.html; and (2)<br />

submission of the completed and signed <strong>for</strong>m, “Sponsor<strong>in</strong>g Department Agreement: IMG<br />

Alternate Pathway <strong>for</strong> Diagnostic Radiology Certification” [l<strong>in</strong>k <strong>to</strong> <strong>for</strong>m]. An application fee is<br />

required.<br />

<strong>The</strong> ABR reta<strong>in</strong>s the right and responsibility <strong>to</strong> exercise <strong>Board</strong> discretion concern<strong>in</strong>g<br />

the request of any <strong>in</strong>dividual <strong>to</strong> enter the exam<strong>in</strong>ation process.<br />

Alternate Pathway <strong>to</strong> Diagnostic Radiology Certification<br />

<strong>The</strong> IMG must serve four cont<strong>in</strong>uous years as a full-time member of the department <strong>in</strong> any<br />

comb<strong>in</strong>ation of the follow<strong>in</strong>g categories: resident, ACGME-accredited fellowship, nonaccredited<br />

fellowship, or faculty.<br />

Alternate Pathway <strong>to</strong> Subspecialty Certification (<strong>for</strong>merly “Certificate of Added<br />

Qualifications, of CAQ”) <strong>in</strong> neuroradiology, nuclear radiology, pediatric radiology,<br />

and vascular and <strong>in</strong>terventional radiology <strong>for</strong> <strong>in</strong>ternational medical graduates<br />

Alternate Pathway <strong>to</strong> Subspecialty (“CAQ”) Certification<br />

ABR diagnostic radiology <strong>certification</strong>, followed by a two-year faculty appo<strong>in</strong>tment with full-<br />

time cl<strong>in</strong>ical responsibility <strong>in</strong> the subspecialty area, at a s<strong>in</strong>gle <strong>in</strong>stitution with an ACGME-<br />

accredited fellowship tra<strong>in</strong><strong>in</strong>g program <strong>in</strong> the area of subspecialty <strong>certification</strong> desired<br />

provides <strong>for</strong> entry <strong>to</strong> the ABR subspecialty <strong>certification</strong> exam<strong>in</strong>ation. If the cl<strong>in</strong>ical<br />

responsibility is less than full time, the years <strong>to</strong> <strong>certification</strong> may, at the ABR’s discretion, be<br />

modified accord<strong>in</strong>gly.<br />

Note: U.S. graduates with ABR diagnostic radiology <strong>certification</strong> are also eligible<br />

<strong>for</strong> the <strong>alternate</strong> pathway <strong>to</strong> subspecialty <strong>certification</strong>.
