TGF-β is released by platelets, macrophages and neutrophils which are present <strong>in</strong> the <strong>in</strong>itial<br />
phases of the repair process (298-300). The growth stimulatory action of TGF-β appears to be<br />
mediated via an <strong>in</strong>direct mechanism <strong>in</strong>volv<strong>in</strong>g autocr<strong>in</strong>e growth factors such as PDGF-A and –<br />
B, basic FGF or CTGF (287, 301-303). TGF-β is a potent stimulatory signal for fibrosis, and<br />
elevated TGF-β mRNA or prote<strong>in</strong> levels have been documented <strong>in</strong> several different fibrotic<br />
diseases and dur<strong>in</strong>g wound repair (287, 304). The stimulation of fibroplasia <strong>in</strong> vivo has been<br />
attributed to several documented <strong>in</strong> vitro activities of TGF-β <strong>in</strong>clud<strong>in</strong>g stimulation of fibroblast<br />
proliferation, stimulation of the syn<strong>thesis</strong> of extracellular matrix components <strong>in</strong>clud<strong>in</strong>g<br />
fibronect<strong>in</strong>, type I collagen and <strong>in</strong>tegr<strong>in</strong>s, and stimulation of the syn<strong>thesis</strong> of protease <strong>in</strong>hibitors<br />
and gelat<strong>in</strong>ases and suppression of stromelys<strong>in</strong> gene expression (305, 306). In epithelial cells,<br />
TGF-β arrests the cell cycle <strong>in</strong> late G1 (307). The growth <strong>in</strong>hibitory response of epithelial and<br />
endothelial cells to TGF-β lies on the transcriptional control of key regulators of the cell cycle<br />
by the <strong>in</strong>com<strong>in</strong>g TGF-β signal (308) (309).<br />
TGF-β1 is the most abundant isoform <strong>in</strong> all tissues and <strong>in</strong> wound fluid most of the TGF-β is<br />
the type 1 isoform (6, 310). However, <strong>in</strong> vitro studies have suggested that the different TGF-β<br />
isoforms may play both dist<strong>in</strong>ct and non-redundant functions dur<strong>in</strong>g wound heal<strong>in</strong>g (311). All<br />
three genes of TGF-β isoforms share a similar <strong>in</strong>tron/exon structure with a total of seven exons<br />
(Table II) (93). The TGF-β1 promoter does not conta<strong>in</strong> TATA or CAAT elements, <strong>in</strong>cludes<br />
several response elements important <strong>in</strong> wound<strong>in</strong>g (312) (Fig. 8.). Expression of TGF-β1 is<br />
<strong>in</strong>duced <strong>in</strong> response to various mediators and by auto<strong>in</strong>duction which is mediated through AP-<br />
1 sites <strong>in</strong> the TGF-β1 promoter (312).<br />
TGF-β is secreted from cells <strong>in</strong> a latent, <strong>in</strong>active complex conta<strong>in</strong><strong>in</strong>g two prote<strong>in</strong>s: active TGF-<br />
β and its prodoma<strong>in</strong>, TGF-β latency-associated prote<strong>in</strong> (LAP). Active TGF-β is cleaved from<br />
its propeptide, but it rema<strong>in</strong>s associated with TGF-β by non-covalent <strong>in</strong>teractions, conferr<strong>in</strong>g<br />
latency to the complex (313). Two cha<strong>in</strong>s of pro-TGF-β associate to form a disulfide bonded<br />
dimer. LAP and TGF-β together form the small latent TGF-β complex. Most cell l<strong>in</strong>es,<br />
however, secrete also large latent TGF-β complexes, conta<strong>in</strong><strong>in</strong>g additional high molecular<br />
weight prote<strong>in</strong>s that associate with LAP. Best characterized of these are latent TGF-β b<strong>in</strong>d<strong>in</strong>g<br />
prote<strong>in</strong>s (LTBP’s), which can b<strong>in</strong>d to LAP via a disulfide bond(s) (Fig. 9. C). LTBP appears to<br />
<strong>in</strong>crease the efficiency of secretion of TGF-β from cells and promotes the association of TGF-<br />
β to matrix and facilitates its activation (305). Several ECM prote<strong>in</strong>s have been suggested to<br />
b<strong>in</strong>d the active form of TGF-β1. These <strong>in</strong>clude type IV collagen, fibronect<strong>in</strong>, thrombospond<strong>in</strong>,<br />
