In order to be eligible to receive the free 45118 Mini Thermo-Anemometer, you must complete the following steps:
a) Purchase Extech HD350 Pitot Tube Anemometer & Manometer between 10/1/2011 and 12/31/2011.
b) Complete this form and sign it in the space provided.
c) Mail this form together with a copy of your itemized invoice or sales receipt showing your purchase of the qualifying Extech product during the Qualifying Period to the
address listed in Section 3 below. Please keep a copy of your invoice or receipt for your records.
2. WITH RESPECT TO THE ABOVE PROMOTION, YOUR COMPLETED FORM AND ITEMIZED INVOICE OR SALES RECEIPT WILL MAKE UP YOUR CLAIM. YOUR
