information system (LIS) is obsolete and will no longer be supported by Cerner after 2010. The selection and design of a LIS is a long and complicated process, which requires the undivided attention of a medical informatics specialist (i.e., a pathologist with formal fellowship training in laboratory medicine informatics). This is critical because our LIS underlies almost every report that we generate. Such a unit could also help diminish inappropriate or duplicate lab test ordering and help prevent incidents. Additional important issues for 2007 will include short-term renovations of the sixth and eleventh floor laboratories at Foothills Medical Centre (as well as planning for the move to the new west tower), planning for laboratory needs resulting from other CHR capital projects, implementation of LEAN Sigma at all CLS sites The Calgary Health Region has achieved a landmark accomplishment through implementation of Sunrise Clinical Manager (SCM), the acute care component of our e_record system. Activated at Rockyview General Hospital, Peter Lougheed Centre and Foothills Medical Centre over the past year, this feat reflects positively on region staff and the collective contributions made to e_record advancement. While the decision to implement the acute e_record component was partly influenced by the need to replace TDS – the legacy system – motivation for this evolution links directly to the value we place on what is best for our patients and their families. As a region, we aim to promote optimal health and longevity, while providing efficient and effective care. Safety is paramount given the vulnerability of hospitalized patients and the risks inherent to the interventions that illness often dictates. To minimize risk, we must minimize errors. The region’s e_record system addresses a prime area of safety concern - medication administration. Experience has taught the region that 50 per cent of errors in drug management occur in the process of ordering medications and transmitting the orders to pharmacy. Substantial safety improvements have now been made to the medication order process thanks to computerized provider/physician order entry (CPOE) into the acute e_record system (SCM). CPOE has dramatically reduced issues such as writing misinterpretation, misinterpretation of verbal orders, and missing data (dose, route, frequency etc.). Additionally, the e_record supplements health providers with electronic guidance as to the most appropriate choices of drug, dose and route. and implementation of new diagnostic modalities. The department will continue to be proactive and consult with other regional departments to learn of new programs and program expansions so that we can be better positioned to provide state-of-the-art laboratory support. The new regional workforce plan being developed in conjunction with regional clinical departments will help with this process. In conclusion, our regional integrated model for pathology and laboratory medicine is unique in Canada. With a coherent organizational structure and strong leadership throughout the organization, this model will allow us to develop a dynamic, productive department, leading nationally and internationally in clinical service, teaching, and research. Breaking records and nearing goals: regional computerized physician order entry By Dr. Andrew Pattullo, medical director, clinical informatics The extent to which these safety benefits are experienced by our patients directly corresponds to the frequency of provider order entry into the e_record system. Previously, CPOE rates at Foothills and Peter Lougheed were fair, but certainly not optimal. Though this was in part due to the lack of ease afforded by the legacy system (TDS), the launch of the acute e_record system (SCM) at the three acute sites was accompanied by a mandate to achieve 100 per cent direct CPOE for all possible orders. Not even six months after implementation of the acute e_record system, our rate of CPOE for medication orders was already nearing this goal. During March and April 2007, 80 per cent of all medication orders entered into the acute e_record system were entered directly by providers. Residents achieved CPOE rates of nearly 90 per cent. Noting that this figure represents only the CPOE rate of medication orders, and that many orders are appropriate indirect orders, I’m confident stating that we will achieve our goal of 100 per cent CPOE. Few North American health centers ever achieve 80 per cent CPOE. In fact, most don’t come anywhere close. This accomplishment is one that our clinical community should proudly share. It is not enough to believe we’ve made things better - we have a duty to measure and confirm the desired outcomes. Work continues to tailor the system to local and clinical needs, resulting in tools that clinicians own, value and use routinely. The most immediate measure of our ongoing success is the rate of CPOE in our Region. Thank you for your ongoing role in the CPOE success story. Vital Signs June 2007 Page 15
