Form<br />

Non-Latex Gloves<br />

A supply of medium and large non-latex gloves should be kept onsite <strong>for</strong> physicians<br />

per<strong>for</strong>ming examinations. Non-latex gloves are preferred because of the possibility<br />

of latex allergies. These gloves will be changed after every examination. It is there<strong>for</strong>e<br />

recommended that an ample supply of gloves be kept on site during the event.<br />

Referral Notes<br />

Approximately 96 to 99% of all athletes screened at a <strong>MedFest</strong> will be cleared, without<br />

qualification, <strong>for</strong> sports participation. Some athletes however will require additional<br />

professional evaluation which cannot be rendered at the <strong>MedFest</strong> event. A standardized<br />

referral note (Appendix C) should be kept in each examination station.<br />

Pipe and Drape<br />

Privacy <strong>for</strong> athletes is a necessity during the screening. Adequate pipe and draping must<br />

be supplied in order to create private rooms.<br />

Clip Boards and Pens<br />

Having a large number of clip boards and pens will help the check-in process to go<br />

smoothly. Arranging the athlete medical <strong>for</strong>ms in either alphabetical order or scheduled<br />

arrival time will facilitate faster processing time at the check-in area.
