Order Forms: See Order Forms Section in front of book and the Order Forms at the back of this section
Use MYL Hardware Order Form for all Hardware & Hard Window Treatment products.
Kirsch Cubicle Track and Architrac/Cut-to-Measure Order Forms should be used for those products.
Call to order Somfy products.
• Unit specified on the Hardware Order must correspond to the units listed in the Hardware Vendor Price Books.
• It is acceptable to order Hardware along with Window Treatment Orders by attaching the Hardware Order to the main treatment Order.
Listing hardware on the same Order as the Window Treatment is also acceptable and will be assumed a Hardware Order if
items are listed.
• Custom Made Hardware is available – please order the Custom Hardware directly from the Hardware Department so that we can get the
item(s) ordered as soon as possible.
• Please specify if Order is to go directly to customer, store, installer or other workroom.
Must provide address and phone number where delivery will be made
• To assure next day delivery, the Order must be placed by noon on the previous day.
Orders placed after 3:00 pm the day before delivery may receive a 25% Priority Charge.

• Complete the Hardware Return for Credit Authorization Form located in this section and in the Order Forms section.
• Please have Invoice number available for the items you are returning.
Must be in original package with all parts - No Returns on Open Packages or Custom Rods
Must have been purchased from MYL within the last 6 months
A restocking fee of 25% for Kirsch will be charged & Confirmation of Return Authorization will be faxed to you
If older than 6 months, 50% restocking fee for Kirsch
A copy of the Hardware Return for Credit Authorization Form should be attached to hardware when returned.
• NOTE: Custom Kirsch Hardware is not returnable
• NOTE: Special Order Kirsch Hardware is not returnable
Returns: Select and House Parts Hardware
No Returns on Open Packages or Custom Rods a restocking fee of 35 % for Select and House Parts will be charged - Confirmation
