Offer good on purchase of listed Patriot memory product from Newegg.ca or Newegg.com between 3/3/2009 and 3/31/2009. You must include this
completely intact and unedited rebate form, copy of sales receipt/invoice clearly listing the purchase date and product number, and the original UPC
found on the memory product packaging. Your envelope must be postmarked within 21 days of the purchase date, not delivery date. The following
quantity and submission restrictions apply, all submissions that do not adhere to these rules will be refused. The maximum number of times you can
submit a rebate for a purchased product is one (1) per rebate period. Each household, address, full name and receipt/invoice is eligible to receive
one (1) rebate and only one (1) rebate, per product during the eligible period above. Furthermore, only one (1) each of the products listed is eligible
for rebate during the period above. This program is valid in the United States of America and Canada, void where taxed or restricted by law, and void
with any other user-end offer. Requests from P.O. boxes, groups, or organizations will not be honored or acknowledged. Offer not valid with any
other offer. Patriot, Newegg.ca, Newegg.com, or their fulfillment companies are not responsible for lost, mutilated, misdirected or postage-due
requests. Usage of fictitious names or addresses, or fraudulent submissions of multiple or altered requests could result in federal prosecution for
U.S. and Canadian Mail Fraud. Photocopies or any form of reproduction of the UPC will not be accepted.
Make a photocopy of all materials for your personal records, as Patriot requires the original UPC and will not return submitted materials. A copy of
the receipt/invoice is acceptable. Rebates are paid in United States funds for the amount listed on the program and should arrive within 10 weeks
from the date of the envelope's postmark. Patriot is not responsible for requests will illegible or missing information. Patriot has the right to reject any
