Narrative Report<br />

As a medical anthropologist trained in public health, I analyze how social inequalities are embodied as differential<br />

risk for pathologies common among the poor and how health policies may alter the course <strong>of</strong> epidemic disease and<br />

other pathologies afflicting populations living in poverty. I work mostly in the areas <strong>of</strong> infectious disease—more<br />

specifically HIV and syphilis—and reproductive health in Latin America and the Caribbean (LAC). I dedicate 80<br />

percent <strong>of</strong> my time to the areas <strong>of</strong> investigation and development <strong>of</strong> practice guidelines and 20 percent to teaching<br />

and mentoring <strong>of</strong> predominantly medical students and residents.<br />

My largest current research project is <strong>The</strong> Latin American and Caribbean Initiative for the Integration <strong>of</strong> Prenatal<br />

Care with the Testing and Treatment <strong>of</strong> HIV and Syphilis (ILAP), which I initiated in 2007. ILAP is a collaborative<br />

project between HMS, eight national AIDS programs (Brazil, Colombia, Cuba, Dominican Republic, Nicaragua,<br />

Paraguay, Peru, and Uruguay), UNICEF, UNAIDS, and the Pan American Health Organization (PAHO). By<br />

conducting innovative health services research that combines medical anthropology, epidemiology, and health<br />

policy analysis, we plan to delineate a detailed implementation strategy for each country aimed at universalizing<br />

routine testing and treatment <strong>of</strong> HIV and syphilis during pregnancy while we measure the impact <strong>of</strong> the<br />

interventions. Its implications for clinical practice and public health are enormous: ILAP’s objectives are to<br />

contribute to the early diagnosis and timely treatment <strong>of</strong> HIV and syphilis in women, decrease the numbers <strong>of</strong><br />

mother-to-child transmissions (MTCT) <strong>of</strong> HIV, congenital syphilis, and other STIs, and provide better care for<br />

large numbers <strong>of</strong> women and children in LAC—while providing a framework for national and international largescale<br />

policy change aimed at strengthening health systems. As director <strong>of</strong> ILAP, I’m responsible for designing<br />

research protocols for all countries, collecting data or training local teams to collect data, leading data analysis,<br />

facilitating the translation <strong>of</strong> research results into implementation strategies, and mobilizing political support and<br />

funding resources. I’m training and mentoring several medical students and residents who are involved in ILAP in<br />

different research and clinical capacities, both in Boston and in the field.<br />

In recognition <strong>of</strong> this effort, since 2009 I serve as consultant for UNICEF, UNAIDS, and PAHO in the area <strong>of</strong><br />

prevention <strong>of</strong> mother-to-child transmission <strong>of</strong> HIV and congenital syphilis in LAC. For a joint<br />

UNICEF/UNAIDS/PAHO policy document on the greatest challenges posed by the HIV epidemic in LAC, I have<br />

written one <strong>of</strong> the six chapters and I serve as editor <strong>of</strong> the document, which will be published in June 2009. I’m also<br />

involved in the Latin American and Caribbean Initiative for the Elimination <strong>of</strong> MTCT <strong>of</strong> HIV and Congenital<br />

Syphilis, led by PAHO and UNICEF and initiated in 2008.<br />

My contribution to the field <strong>of</strong> AIDS research stems from the work I have conducted in several LAC countries and<br />

particularly in Cuba, where I lead a large research project <strong>of</strong> a randomized, statistically representative sample <strong>of</strong><br />

people who receive antiretroviral therapy (ART). In collaboration with the Institute <strong>of</strong> Tropical Medicine in<br />

Havana, we are preparing manuscripts for publication on the social and clinical impact <strong>of</strong> ART based on the<br />

quantitative and qualitative analyses <strong>of</strong> the life histories <strong>of</strong> 354 people with AIDS.<br />

I’m currently preparing a postdoctoral fellowship in social medicine and global health aimed at Latin American<br />

scholars.<br />
