Case-Carrying CSW Responsibilities<br />

1. If the child is returned home or the court orders legal guardianship:<br />

a) Retrieve the HEP Binder from the caregiver.<br />

b) Photocopy all medical, dental, <strong>and</strong> educational materials, place the photocopies<br />

in the Psychological/Medical/Dental/School Records folder <strong>and</strong> provide the<br />

original documents to the parent(s) or legal guardian(s).<br />

c) Provide the parent(s) or legal guardian(s) with a copy <strong>of</strong> the most recent HEP.<br />

d) For <strong>of</strong>fices who have implemented Concurrent Planning Redesign: Place a<br />

copy <strong>of</strong> the most recent HEP <strong>and</strong> the original DCFS FB forms in the<br />

Psychological/ Medical/Dental/School Records folder.<br />

For <strong>of</strong>fices who have not implemented Concurrent Planning Redesign:<br />

Place a copy <strong>of</strong> the most recent HEP <strong>and</strong> the original DCFS 4344 III in the<br />

Psychological/ Medical/Dental/School Records folder.<br />

e) Return the HEP Binder to the EW for recycling.<br />

2. If the youth becomes a ward <strong>of</strong> the court <strong>and</strong> is placed under the supervision <strong>of</strong> the<br />

Probation <strong>Department</strong>, then the youth is removed from DCFS supervision <strong>and</strong> his or<br />

her dependency jurisdiction is terminated. The following steps should be followed if<br />

the Probation Officer requests written reports on the child’s medical, mental health<br />

<strong>and</strong> educational status:<br />

a) Retrieve the HEP Binder from the caregiver.<br />

NOTE: Do not give the HEP Binder or its contents directly to the Probation<br />

Officer.<br />

b) Photocopy all medical, dental, <strong>and</strong> educational materials, place the photocopies<br />

in the Psychological/Medical/Dental/School Records folder <strong>and</strong> provide the<br />

original documents to the youth’s Probation Officer.<br />

c) Provide the Probation Officer with a copy <strong>of</strong> the most recent HEP.<br />

d) For <strong>of</strong>fices who have not implemented Concurrent Planning Redesign:<br />

Place a copy <strong>of</strong> the most recent HEP <strong>and</strong> the original DCFS 4344 III in the<br />

Psychological/ Medical/Dental/School Records folder.<br />
