Hon. Dr. Fenton Ferguson<br />

<strong>HIV</strong> <strong>and</strong> AIDS continue to be a major priority in<br />

Jamaica. The disease is not solely a health concern<br />

but has developmental implications for the country.<br />

The Ministry of Health continues to provide treatment,<br />

care <strong>and</strong> support to persons living with the disease.<br />

A part of this support is access to information <strong>and</strong><br />

personnel from whom help can be sought.<br />

We acknowledge that more needs to be done if<br />

we hope to halt <strong>and</strong> reverse the spread of <strong>HIV</strong> <strong>and</strong><br />

AIDS as outlined in our development agenda. We<br />

are moving beyond talk <strong>and</strong> have demonstrated in tangible ways our commitment<br />

to the fundamental human rights of our citizens <strong>and</strong> the importance of universal <strong>and</strong><br />

easy access to <strong>HIV</strong> resources. This directory is a part of our commitment to remove<br />

the impediments to access to social services, to allow infected <strong>and</strong> affected persons<br />

to participate <strong>and</strong> to have a voice.<br />

The directory will allow anyone who so desires to access trained human resource<br />

to identify <strong>and</strong> address the peculiar issues related to unbalanced gender power<br />

relations; as well as locate agencies to support work to address such issues. It will<br />

also allow us to keep track of the persons that we have trained <strong>and</strong> to use this<br />

resource in the best way possible to support the national programme.<br />

This is a first for us <strong>and</strong> this Registry will assist us to reach our goals of ensuring that<br />

all persons have safe, easy <strong>and</strong> universal access to care, treatment <strong>and</strong> support<br />

services through gender equality mainstreaming in the national <strong>HIV</strong> response.<br />

The National <strong>HIV</strong>/STI<br />

Programme, Jamaica<br />

Jamaica’s <strong>HIV</strong> l<strong>and</strong>scape includes features of both a generalized <strong>and</strong> concentrated<br />

epidemic. This accounts for a low in <strong>HIV</strong> prevalence among the adult population of<br />

1.7% to a high among men who have sex with men (MSM) of 32%. Currently, there<br />

is an estimated 32,000 persons living with <strong>HIV</strong> (PL<strong>HIV</strong>), half of which is unaware of<br />

their status. Since January 1982 <strong>and</strong> December 2010, twenty seven thous<strong>and</strong> two<br />

hundred <strong>and</strong> seventy two cases (27,272) of <strong>HIV</strong> were reported to the Ministry of<br />

Health (AIDS Report, 2010).<br />

High risk behaviour such as early sexual debut, multiple sexual partners, high levels<br />

of transactional sex, gender inequalities, inadequate condom use <strong>and</strong> homophobia<br />

continue to fuel the transmission of <strong>HIV</strong> <strong>and</strong> STIs, (MoH, 2010). The <strong>HIV</strong> epidemic<br />

is also closely tied to poverty <strong>and</strong> related development issues, including the slow<br />

rate of economic growth, high levels of unemployment, low educational attainment<br />

especially among males, <strong>and</strong> crime <strong>and</strong> violence. Moreover, the epidemic threatens<br />

national productivity because the majority of cases occur in the reproductive <strong>and</strong><br />

working age groups.<br />

As such, the national <strong>HIV</strong> response focuses equally on preventing the further spread<br />

of the epidemic, supporting better care for those infected <strong>and</strong> affected by <strong>HIV</strong>, <strong>and</strong><br />

building capacity <strong>and</strong> resilience to withst<strong>and</strong> the impact of AIDS through several<br />

priority areas. These include:<br />

1. Expansion of prevention programmes especially among most at risk<br />

populations (MARPs) including direct <strong>and</strong> targetted risk reduction messages,<br />

access to testing <strong>and</strong> peer counselling.<br />

2. Strengthening of positive health dignity <strong>and</strong> prevention programmes<br />

particularly among most at risk populations including increased care <strong>and</strong><br />

support for adolescents living with <strong>HIV</strong> <strong>and</strong> AIDS.<br />

3. Scaling up of <strong>HIV</strong> testing, particularly among most at risk populations <strong>and</strong><br />

hospital admissions in order to identify persons most in need of treatment <strong>and</strong><br />

delay progression to AIDS.<br />

4. Scaling up <strong>HIV</strong> management <strong>and</strong> emphasis on care <strong>and</strong> support.<br />

5. Improved management of STIs.<br />

6. Continued capacity building for stakeholders on monitoring <strong>and</strong> evaluation<br />

through widespread training <strong>and</strong> use of technical assistance.<br />

7. Increased dissemination <strong>and</strong> utilization of data for programme planning <strong>and</strong><br />

underst<strong>and</strong>ing the epidemic.<br />

8. Improved underst<strong>and</strong>ing of the context of stigma <strong>and</strong> discrimination through<br />

national surveys; expansion of the system for reporting <strong>and</strong> redress to reduce<br />

stigma <strong>and</strong> discrimination in all settings.<br />

9. Increased advocacy among high level leadership for improved socioeconomic<br />

policy initiatives <strong>and</strong> legislative changes to reduce vulnerability to <strong>HIV</strong><br />

including policy development in various sectors; <strong>and</strong><br />

10. Strengthen partnerships <strong>and</strong> communication mechanisms with key<br />

stakeholders in the national response.<br />

Jamaica, like other countries, is concerned about the potential impact of the global<br />

financial crisis on the sustainability of its national response to <strong>HIV</strong>. In particular, there<br />

are concerns about the ability of the Ministry of Health to meet its responsibilities as<br />

part of the national <strong>HIV</strong> response to not only achieve the universal access targets,<br />

but also to sustain gains already achieved <strong>and</strong> maximize the efficiency <strong>and</strong> benefits<br />

of current <strong>and</strong> future investments. Currently, the National <strong>HIV</strong>/STI Programme (NHP)<br />

is largely funded by a US$40 million grant from the Global Fund supported project<br />

2008 – 2013. Other funding sources include the Government of Jamaica, a World<br />

Bank Loan <strong>and</strong> a USAID Grant. Funding has also been made available from the<br />

European Union (EU)/ UN Women, CHAI, Pan American Health Organization/<br />

World Health Organization (PAHO/WHO), the Caribbean Epidemiology Centre<br />

(CAREC), UNAIDS, UNFPA <strong>and</strong> UNICEF.<br />

The Work of the Jamaica<br />

National AIDS Committee (NAC)<br />

The NAC is an organization which functions in such a unique way that no<br />

organization working in <strong>HIV</strong> <strong>and</strong> AIDS in the world functions in quite the same<br />

way. Although its name is ‘National’ AIDS Committee <strong>and</strong> it was created by the<br />

Government of Jamaica in 1988, the NAC is actually nongovernmental. The NAC<br />

began with 18 members <strong>and</strong> now has over 100 member organizations.<br />

The Mission Statement of the NAC is:<br />

‘To facilitate multisectoral collaboration to reduce the negative impact of <strong>HIV</strong> <strong>and</strong><br />

AIDS on the Jamaican society <strong>and</strong> promote an informed, supportive <strong>and</strong> caring<br />

environment that empowers persons living with <strong>and</strong> affected by <strong>HIV</strong> <strong>and</strong> AIDS.’<br />

The NAC’s main functions are:<br />

• To advise the Minister of Health on policy issues relevant to <strong>HIV</strong> & AIDS/STIs<br />

• To involve all sectors of society in efforts to prevent <strong>and</strong> control <strong>HIV</strong> & AIDS/<br />

STIs<br />

• To act as a central body where ideas, experiences <strong>and</strong> questions about <strong>HIV</strong> &<br />

AIDS/STIs in Jamaica can be shared, discussed, addressed<br />

• To provide a sustainable means of supporting the initiatives of the NAC <strong>and</strong><br />

member organizations by eliciting funds from fundraising activities, public <strong>and</strong><br />

private sector participation.<br />

The NAC partners with the National <strong>HIV</strong>/STI Control Programme (NHP),<br />

representatives from both public <strong>and</strong> private sector organizations, non-governmental<br />

organizations (NGOs) <strong>and</strong> community based organizations (CBOs).<br />

The NAC is facilitated by an Executive Committee, Sub-committees <strong>and</strong> Parish AIDS<br />

Associations (PAAs). In addition there are elected members from both the public <strong>and</strong><br />

private sectors. The Executive committee is the final authority <strong>and</strong> the management<br />

arbiter. The NAC, acting in its capacity as an umbrella organization, reflects the<br />

needs voiced by agencies in the structure of its Sub-committees.<br />

These needs are therefore reflected in the choice of Sub-committees. The Subcommittees<br />

of the NAC are Information & Communication, Legal <strong>and</strong> Ethical,<br />

<strong>Resource</strong> Mobilisation, Advocacy <strong>and</strong> International Development Partners.<br />

In addition to the Sub-committees, the National AIDS Committee decentralizes<br />

the <strong>HIV</strong>/AIDS response through the work of PAAs across the Isl<strong>and</strong>. These parish<br />

associations reflect the structure of the NAC <strong>and</strong> provide a multi - disciplinary<br />

community based response to the needs of persons infected <strong>and</strong> affected by <strong>HIV</strong> <strong>and</strong><br />

AIDS. The PAAs also act as local representative bodies that inform <strong>and</strong> contribute<br />

to translating ideas <strong>and</strong> building policy enabling the work of the NAC to be<br />

more realistic.<br />

The UN Women-EC Project<br />

The Jamaica National <strong>HIV</strong>/STI Programme (NHP) through the Ministry of Health, is<br />

a recipient of funds from the UN Women -EC project “Supporting <strong>Gender</strong> <strong>Equality</strong> in<br />

the Context of <strong>HIV</strong> <strong>and</strong> AIDS”. UN WOMEN, is the arm of the United Nations which<br />

provides technical <strong>and</strong> financial assistance to innovative programmes <strong>and</strong> strategies<br />

to foster women’s empowerment <strong>and</strong> gender equality. The overall objective of this<br />

programme being implemented in 5 countries including Jamaica, is to ensure the<br />

mainstreaming of gender equality <strong>and</strong> human rights into key policies, programmes,<br />

actions <strong>and</strong> budgets to address <strong>HIV</strong> <strong>and</strong> AIDS at the national level.<br />

Mainstreaming gender in National Strategic Plans can enhance prevention,<br />

treatment <strong>and</strong> control of the p<strong>and</strong>emic, <strong>and</strong> promote gender equality consistent with<br />

international human rights commitments including Convention on the Elimination of<br />

Discrimination against Women <strong>and</strong> Girls (CEDAW) <strong>and</strong> the Millennium Development<br />

Goals. Underst<strong>and</strong>ing how gender impacts the roles, rights, attitudes <strong>and</strong> behaviours<br />

of males <strong>and</strong> females of all ages <strong>and</strong> classes, can guide interventions aimed at<br />

prevention, treatment <strong>and</strong> care as well as advocacy. Bringing gender awareness to<br />

<strong>HIV</strong> helps to identify how gender differentials <strong>and</strong> inequalities make males <strong>and</strong> females<br />

vulnerable to <strong>HIV</strong>.<br />

The specific objective of the NHP project component is to develop core capacities of the<br />

national AIDS coordinating mechanism to promote human rights <strong>and</strong> gender equality<br />

in the <strong>HIV</strong> <strong>and</strong> AIDS response in Jamaica.<br />

A St<strong>and</strong>ards Guide<br />

for the <strong>Gender</strong> <strong>Equality</strong> <strong>and</strong> <strong>HIV</strong><br />

<strong>Resource</strong> <strong>Persons</strong> Registry (Jamaica)<br />

PURPOSE<br />

To establish a registry for Jamaican based resource persons <strong>and</strong> agencies in the<br />

area of gender equality <strong>and</strong> <strong>HIV</strong> & AIDS. This is a deliverable of the National <strong>HIV</strong>/<br />

STI Programme (NHP) project in partnership with UN WOMEN-EC. The objective<br />

is to build the capacity of the national <strong>HIV</strong> mechanism to address gender equality<br />

mainstreaming in the national <strong>HIV</strong> <strong>and</strong> AIDS response through the NHP, the National<br />

AIDS Committee (NAC) <strong>and</strong> Civil Society.<br />

The Registry is intended to contribute to the sustainability of the mainstreaming<br />

process, beyond the project life, as it will provide a resource pool for the national<br />

<strong>HIV</strong> response (government, civil society <strong>and</strong> other partners) to draw on or partner<br />

with, towards incorporating gender issues within a human rights frame, in the goal<br />

to halt <strong>and</strong> reverse <strong>HIV</strong>.<br />

It will be a directory of persons in Jamaica with knowledge <strong>and</strong> skills to link gender<br />

issues with <strong>HIV</strong> <strong>and</strong> address gender equality within a human rights frame, in the<br />

prevention <strong>and</strong> control of the epidemic. The target was fifty resource persons in<br />

commemoration of Jamaica 50—The Golden Jubilee 1962-2012. This was however<br />

exceeded, as training increased the numbers. It will also include agencies with a<br />

specific focus on gender equality mainstreaming in <strong>HIV</strong> which can provide expertise<br />

as well as partnership opportunities.<br />

The Registry is intended for use by any agency, individual, programme or project<br />

in the national <strong>HIV</strong> response requiring the skills, expertise <strong>and</strong> support described.<br />

It may also be used by persons within the Caribbean region or internationally<br />

requiring expertise <strong>and</strong> knowledge from or about the region. This is a deliverable<br />

of the gender equality mainstreaming Project of the National <strong>HIV</strong>/ STI Programme<br />

(NHP) of the Ministry of Health (MoH).<br />

RATIONALE<br />

Across the world, there has been a changing pattern of male/female infections. Early<br />

cases in many countries were concentrated in male homosexuals <strong>and</strong> intravenous<br />

drug users, but as the epidemic has spread there has been a progressive shift<br />

towards heterosexual transmission <strong>and</strong> increasing infection rates in females. The<br />

reality today is that, globally, more women than men are now dying of <strong>HIV</strong> <strong>and</strong><br />

AIDS with significantly different age patterns of infection.<br />

Beyond the statistics of sex-based differences in infection rates, there are profound<br />

differences in the underlying causes <strong>and</strong> consequences of <strong>HIV</strong> <strong>and</strong> AIDS infections<br />

in males <strong>and</strong> females. This reflects differences in biology, sexual behaviour, social<br />

attitudes <strong>and</strong> pressures, economic power <strong>and</strong> vulnerability. Often, the inequity that<br />

women <strong>and</strong> girls experience as a result of <strong>HIV</strong> <strong>and</strong> AIDS is a measurement of<br />

their (generally) different <strong>and</strong> lower status in society in relation to men <strong>and</strong> boys<br />

<strong>and</strong> the discrimination they encounter in many spheres, including health, education<br />

<strong>and</strong> employment.<br />

Based on these reasons, <strong>HIV</strong> <strong>and</strong> AIDS are regarded by development specialists as<br />

inherently a gender-based issue. They argue that the epidemic needs to be seen in<br />

this light if it is to be addressed effectively. It is so significant that it is posited that<br />

<strong>HIV</strong> <strong>and</strong> AIDS will only be conquered when the effort to achieve gender equality<br />

is successful.<br />

PROCESS FOR INCLUSION IN THE REGISTRY<br />

By invitation <strong>and</strong> referral for the experts in gender <strong>and</strong> <strong>HIV</strong> <strong>and</strong> by dint of being<br />

trained by the Project. The Registry therefore also provides a record of those<br />

benefiting from training in <strong>Gender</strong> <strong>and</strong> <strong>HIV</strong>.<br />
