Free Stage Entertainment Application

The Charlotte Black Gay Pride (CBGP) is thrilled to provide an opportunity for Charlotte talent to perform in front of a
diverse audience including members of the LGBT community. This performance will provide exposure and
experience rather than a financial exchange.
The stage at the CBGP Extravaganza will be equipped with equipment for CD players/iPod players, a sound system,
chairs, (no music stands) and a stage crew. Performance times are 11:00 am, 12:00 pm, and 2:00 pm; these are 30
minute sets and you must finish on time and break down 15 minutes before the next hour. This will allow the show to
begin as scheduled in the CBGP Event Guide.
Please remember when planning your event that time is limited and dressing rooms are not available. The Extravaganza
will provide signage for your performance.
Because of the number of applications, final scheduling may take until the 1 st of July. We try
to honor the times you requested but sometimes, because of other scheduled events, this
becomes impossible. Please fill out all three choices. If you do not write three choices down
you will be placed into the “anytime” file and will not be able to change your performance
once it is scheduled. The date of the 2013 CBGP Extravaganza is Saturday, July 20 th .
If you are selected to perform, you will receive a confirmation email telling of your performance time. At this point, you will
need to send an email or letter confirming that you will perform. You will be asked to park in Satellite parking with
vendors. You will be allowed to park near the stage right before, during, and right after your performance only. Due to
space constraints, only one vehicle is allowed per act. Please remember this when planning your performance.
Please complete and return this entertainment application and a promotional packet to the CBGP Extravaganza, at the
below address by June 1, 2013. Please complete the entire application! Incomplete packets and packets submitted
after the deadline will not be considered. Packets can be mailed or emailed.
CBGP Extravaganza’s policy prohibits performers from soliciting contributions from the audience; however, you are
allowed to sell merchandise pertaining to your performance (cds, DVDs or shirts) with a $25.00 donation toward the Music
Booth; no merchandise fee being paid back to the CBGP. You are allowed to sell during your performance only.

Entertainment Waiver Form

This is only an application for an opportunity to perform. Submission of this application
does not guarantee the performer a booking at the Extravaganza, nor does it constitute any
agreement with CBGP to provide space, services or compensation.
The parties agree that the participant, and any agents and employees of participant are
acting in an independent capacity and not as officers, employees, or agents of the CBGP
Extravaganza or its affiliates.
CBGP Extravaganza or its affiliates are not responsible for loss or damage to property of
participant.
Participant hereby waives all claims and recourse against the State CBGP Extravaganza or
its affiliates including the right to contribution for loss or damage to persons or property in
any way connected to this Agreement, and shall indemnify and hold harmless the CBGP
Extravaganza or its affiliates, from all claims due to any acts or omissions.
Participant shall indemnify and hold harmless the CBGP Board of Directors, CBGP
Extravaganza or its affiliates, from any and all claims, suits or actions of every name, kind
and description brought forth from, or on account of, injuries to or death of any person
including, but not limited to, workers and the public, or damage to property resulting from
the performance at the CBGP Extravaganza.
