CTA Community Supports<br />

Weber counties. Community Treatment Alternatives<br />

(CTA) is a non-profit organization operating in Utah since<br />

1985. We specialize in the treatment of individuals with<br />

autism spectrum disorders. It is our philosophy to<br />

maintain the highest standards of treatment available to<br />

HELP: The Home Electric Lifeline Program, in<br />

cooperation with Rocky Mountain Power, provides their<br />

HEAT-eligible customers with a monthly discount off<br />

their electric bill. UTAP: The Utah Telephone Assistance<br />

(Lifeline/Link-up) Program, provides low-income<br />

households with discounts for initial landline telephone<br />

installation and their monthly phone bills. UMP: Protects<br />

eligible households from having their heat being shut-off<br />

during the winter. The SEAL mission is to “identify and<br />

lowest-income with the highest heating costs especially<br />

“The Medicaid Work Incentive was designed for working<br />

people with disabilities who do not qualify for a "free"<br />

Medicaid services because their income and/or assets<br />

are too high. The MWI allows a higher income and asset<br />

limit and gives you the option to pay a premium for a<br />

Medicaid services. The MWI is not the same as the<br />

Medicaid Spenddown because you will pay a premium.<br />

permanent kidney failure. It provides basic protection<br />

against the cost of health care. Prescription drug<br />

discount cards are available. It provides basic health<br />

care coverage and may include prescription drug<br />

coverage. It does not cover all medical expenses or the<br />

cost of most long-term care. A portion of the Federal<br />

Insurance Contributions Act (FICA) taxes, or payroll taxes<br />

Primary Care Network<br />

The Utah Primary Care Network (PCN) is offered through<br />

the Utah Department of Health. It is for uninsured<br />

adults whose income is less than 150% of the poverty<br />

level. There is no asset limit for eligibility. PCN provides<br />

primary care services, basic dental and vision care,<br />

emergency care, birth control, and 4 prescription<br />

medications per month. Annual enrollment costs are<br />

$25-$50 and co-payments are required for some<br />

services. You may only enroll during periodic “open<br />

required to obtain the pass. Acceptable documentation<br />

includes: statement by a licensed physician; document<br />
