SECTION II – AFFILIATIONS INFORMATION<br />

Disclose any relationship the applicant has, or has had, with any U.S. or<br />

foreign motor carrier, broker, or freight forwarder registered with the former ICC,<br />

FHWA, Office of <strong>Motor</strong> <strong>Carrier</strong> <strong>Safety</strong>, or <strong>Federal</strong> <strong>Motor</strong> <strong>Carrier</strong> <strong>Safety</strong><br />

Administration within the past 3 years. For example, this relationship could be<br />

through a percentage of stock ownership, a loan, a management position, a<br />

wholly-owned subsidiary, or other arrangement.<br />

If this requirement applies to applicant, provide the name of the affiliated<br />

company, the latter’s MC or MX number, its U.S. DOT Number, if any, and the<br />

company’s latest U.S. DOT safety rating. Applicant must indicate whether these<br />

entities have been disqualified from operating commercial motor vehicles<br />

anywhere in the United States pursuant to Section 219 of the <strong>Motor</strong> <strong>Carrier</strong><br />

<strong>Safety</strong> Improvement Act of 1999 (Pub. L. 106-159, 113 Stat. 1748)(MCSIA).<br />
