D.J.’s Hero Awards
The D.J.’s Hero Award scholarship is unique as it honors the “unsung hero” who has overcome
challenges, demonstrated leadership and commitment to their community.
On a separate sheet of paper, please complete the following questions, using detailed and very
specific examples (list questions numerically). Failure to answer the questions in complete detail
will disqualify the nominee:
1. Describe in detail how the nominee showed courage by acting independently and/or
responsibly when faced with a life challenge or adversity.
2. Please list personal goals of nominee, their biggest achievement in life and how the nominee
expresses enthusiasm for life.
3. Demonstrate the nominee’s commitment to others and the community (Please avoid resume-like
listings of volunteer projects ).
The nomination detail should not exceed two typed pages (must use at least an 11 point size).
Specific details are important. You may nominate more than one youth for this award. The form
may be duplicated for additional entries. The nominator cannot be a relative of the nominee.

The Salvation Army is now accepting nominations
for the 2009 D.J.’s Hero Awards. Given in memory
of D.J. Sokol, the award recognizes Nebraska youths
ages 17-19, who exemplify his qualities of
dedication to faith, selfless acts for others and
commitment to community. Honorees receive a
$5,000 scholarship toward their selected university
or college.
D.J. Sokol had many obstacles to overcome.
However, he focused his energy on his family and
friends. With a passion for life, he delighted everyone
around him and with an engaging sense of humor, he
profoundly impacted his community. D.J.’s positive
outlook on life, and extraordinary dedication to
helping others made him a hero.
Through the D.J.’s Hero Awards, D.J.’s memory lives
on in the recipients who share his same qualities
and dedication.

Help Recognize Nebraska’s Outstanding Youths
You can help The Salvation Army recognize
exceptional young Nebraskans by submitting your
nomination today. (Entries must be postmarked
by March 6, 2009.) Winners will be notified by
April 1. The presentation of the D.J.’s Hero Awards
will be at The Salvation Army D.J.’s Hero Awards
Luncheon on May 4, 2009 at Qwest Center Omaha.
Attendance is required by all honorees.
