D.J.’s <strong>Hero</strong> <strong>Awards</strong><br />

The D.J.’s <strong>Hero</strong> Award scholarship is unique as it honors the “unsung hero” who has overcome<br />

challenges, demonstrated leadership and commitment to their community.<br />

On a separate sheet of paper, please complete the following questions, using detailed and very<br />

specific examples (list questions numerically). Failure to answer the questions in complete detail<br />

will disqualify the nominee:<br />

1. Describe in detail how the nominee showed courage by acting independently and/or<br />

responsibly when faced with a life challenge or adversity.<br />

2. Please list personal goals of nominee, their biggest achievement in life and how the nominee<br />

expresses enthusiasm for life.<br />

3. Demonstrate the nominee’s commitment to others and the community (Please avoid resume-like<br />

listings of volunteer projects ).<br />

The nomination detail should not exceed two typed pages (must use at least an 11 point size).<br />

Specific details are important. You may nominate more than one youth for this award. The form<br />

may be duplicated for additional entries. The nominator cannot be a relative of the nominee.<br />

The <strong>Salvation</strong> <strong>Army</strong> is now accepting nominations<br />

for the 2009 D.J.’s <strong>Hero</strong> <strong>Awards</strong>. Given in memory<br />

of D.J. Sokol, the award recognizes Nebraska youths<br />

ages 17-19, who exemplify his qualities of<br />

dedication to faith, selfless acts for others and<br />

commitment to community. Honorees receive a<br />

$5,000 scholarship toward their selected university<br />

or college.<br />

D.J. Sokol had many obstacles to overcome.<br />

However, he focused his energy on his family and<br />

friends. With a passion for life, he delighted everyone<br />

around him and with an engaging sense of humor, he<br />

profoundly impacted his community. D.J.’s positive<br />

outlook on life, and extraordinary dedication to<br />

helping others made him a hero.<br />

Through the D.J.’s <strong>Hero</strong> <strong>Awards</strong>, D.J.’s memory lives<br />

on in the recipients who share his same qualities<br />

and dedication.<br />

Help Recognize Nebraska’s Outstanding Youths<br />

You can help The <strong>Salvation</strong> <strong>Army</strong> recognize<br />

exceptional young Nebraskans by submitting your<br />

nomination today. (Entries must be postmarked<br />

by March 6, 2009.) Winners will be notified by<br />

April 1. The presentation of the D.J.’s <strong>Hero</strong> <strong>Awards</strong><br />

will be at The <strong>Salvation</strong> <strong>Army</strong> D.J.’s <strong>Hero</strong> <strong>Awards</strong><br />

Luncheon on May 4, 2009 at Qwest Center Omaha.<br />

Attendance is required by all honorees.<br />
