Helps those in need of food assistance-People can get information on food assistance in<br />

Colorado. Callers can receive guidance in English or Spanish and hotline operators,<br />

meals, congregate meal program at the community center, In Home Homemaking and<br />

Personal Care assistance for 6 weeks while recovering from an illness, accident, surgery,<br />

etc. (the client must have an expected recovery by their physician to qualify for the<br />

assistance), eyeglasses, dental assistance, care giver support group, caregiver respite<br />

relief to caregivers providing 24/7 to someone. Each fall the agency offers a furnace start<br />

up program. A contractor checks the furnace for safety, turns it on, cleans it and changes<br />

the filter if needed. Those qualifying also receive a carbon monoxide monitor and smoke<br />

detector installed free of charge.<br />

An incentivized goal-based program that assists families who are committed to attaining<br />

self-sufficiency. Also assists community and families in finding needed support and<br />

resource. Clients work with Sarah to set up goals. Once goals are complete, incentives<br />

are paid directly to client. Goal amounts range from $25-$300 per goal achieve. Very<br />

limited immediate needs dollars are available, but only to current clients enrolled in the<br />

program. Families can apply for services if they make under $75,000.00/year, have some<br />

care control over a dependent child (up to 18 yrs), and have a lawful presence.<br />

You can participate in WIC if you: Live in Colorado. Are pregnant or breastfeeding, and<br />

household size of 3, monthly income less than $2,823). If you currently get TANF, Food<br />

Chaffee County Mammography Fund provides funds to pay for or defer the cost of a<br />

mammogram or alternative diagnostic procedure for the medically underserved,<br />

uninsurable, and underinsured people of Chaffee, Lake, Northern Fremont and Saguache<br />

Counties. Appointments for eligibility can be made with Patient financial <strong>Services</strong>, (719)<br />

539-6661. The applicant must have a prescription for the mammogram, which may be<br />

obtained from a physician or Chaffee People’s Clinic in Salida (719)539-4144 or in<br />

<strong>Buena</strong> <strong>Vista</strong> (719)395-3238. Once the requirements have been met, a CCMF voucher is<br />

issued and a mammogram can be done at the HRRMC.<br />
