Completing the circle of care
Gundersen Lutheran is committed to enhancing community-based healthcare
by providing patients with access to the best care possible, as close to home as
possible. Our growing network of regional clinics provides comprehensive
care and specialty services to residents of all ages in communities throughout
our 19-county service region. From urgent care needs to complete physicals,
our experts are continually innovating to address rural healthcare issues in a patient-centric
manner. Mobile mammography projects, remote fetal monitoring
systems, and integrated electronic medical records that can be accessed by
healthcare personnel throughout the Gundersen Lutheran system are the latest
initiatives that enhance community-based patient care in the Tri-state Region.
Gundersen Lutheran’s new ExpressCare clinics in Holmen, Onalaska and
La Crosse make it possible for people to receive a quick diagnosis and
treatment of common ailments in about 15 minutes with no appointment.
ExpressCare nurse practitioners and physician assistants treat adults and
children 2 years of age and older for a flat fee, no matter where they receive
their primary care. Gundersen Lutheran ExpressCare patients benefit from
our infrastructure and physician referral network through fully integrated
electronic outpatient health records, direct access to primary care appointments,
and internal rotation of regular clinic staff.
After-hours medical care is available through three Gundersen Lutheran Urgent
Care clinics, where patients can be seen for unexpected, non life-threatening
illness or injuries. To help patients choose the care that best fits their needs, our
Telephone Nurse Advisors can be called around the clock, even on evenings,
weekends, and holidays, for guidance on medical questions and sudden health
problems requiring medical attention.
42 | CIRCLE OF CARE
Convenient access to health
and preventive care services
in both traditional and
non-traditional settings is
central to Gundersen
Lutheran’s commitment to
community-based healthcare.
an integrated delivery system supports the health of our communities
43
As a Level II trauma center, Gundersen Lutheran
meets national benchmarks established by the
American College of Surgeons for demonstrated
commitment to superior trauma care. Immediate,
specialized, multidisciplinary care is delivered
24 hours a day, 365 days a year, to treat critical
injuries and life-threatening events.
The Trauma & Emergency Center at Gundersen
Lutheran includes all components identified with
optimal trauma care: access, acute hospital care, rehabilitation,
research activities and prevention. Our responsibility
to provide a continuum of care extends beyond the
10,000 square feet of our actual Trauma & Emergency
Center; we provide disaster preparedness, emergency
medical and surgical patient care, education, and coordination
point services spanning 20,000 square miles and
more than 30,000 patients per year.
Ancillary Services
Providing the highest level of trauma care for our region
requires a rapid response to emergency calls and transportation
to care facilities. Gundersen Lutheran offers the
most advanced pre-hospital patient care available through
its wholly owned subsidiary, Tri-State Ambulance.
Highly trained response teams are equipped with the latest
time-saving technology, making response times faster
and accurate patient information available to emergency
medical technicians in the field. Wireless laptop systems
allow paramedics to interact in real time with the dispatch
center, and to call up database records before the
ambulance arrives on the scene. Only 10% of EMS agencies
in the country use this state-of-the-art technology.
44 | CIRCLE OF CARE
Tri-State Ambulance recently launched a new Cardiac Alert protocol
for use in the field by paramedics to complement the Priority One
Heart Attack Program already in place at Gundersen Lutheran. When
a patient meets specific symptoms for a heart attack, ambulance
personnel signal a Cardiac Alert, activating a specialized Cardiac
Catheterization team at Gundersen Lutheran. The team saves
precious time by preparing for the heart attack patient even during
transport to the hospital. The goal of the protocol is to reduce
the time from symptom onset to intervention, another step
in continuing to improve our nationally recognized cardiac
care in the communities served by Gundersen Lutheran.
Residents of western Wisconsin, northeast Iowa and
southeast Minnesota have free access to health and
human services information and referrals through
Great Rivers 2-1-1, a preventive program operated
by Gundersen Lutheran to promote the health
and well-being of people throughout the region.
The toll-free phone lines are open 24 hours a day,
7 days a week, to provide callers with a central
access point for complete, current information on
over 3,500 community and human services
programs, as well as telephone counseling and
crisis intervention. By providing the critical
connection that makes services easier to access,
Great Rivers 2-1-1 (www.greatrivers211.org)
benefits nearly 40,000 callers each year.
5.4%
14.4%
According to the American Heart
Association, 5.4% of patients nationwide
who suffer a sudden cardiac arrest
outside a hospital setting survive to be
discharged from the hospital. We have
achieved a survival rate nearly three
times (14.4%) the national average in
the region served by Tri-State Ambulance.
