Intended Audience<br />
This conference is designed for a multidisciplinary audience of<br />
healthcare professionals including those who practice anesthesia and<br />
surgery as well as primary care providers.<br />
Purpose/Goal<br />
To provide knowledge, increase competency and performance, and<br />
provide networking opportunities.<br />

The Rasmus Center is on the <strong>Gundersen</strong> Lutheran La Crosse Campus.<br />
When you arrive on campus, please park in the clinic parking ramp<br />
located off 7th street. Our facility meets or exceeds ADA standards.<br />
Registration<br />
To register, please complete the attached registration form and return<br />
by mail or fax by Friday, March 11. There is no charge for <strong>Gundersen</strong><br />
Lutheran employees. The seminar fee includes tuition, course syllabus,<br />
continental breakfast, break refreshments and lunch.<br />
Accreditation<br />
<strong>Gundersen</strong> Lutheran Medical Foundation is accredited by the<br />
Wisconsin Medical Society to provide continuing medical education<br />
for physicians. <strong>Gundersen</strong> Lutheran Medical Foundation designates<br />
this educational activity for a maximum of 7 AMA PRA Category I<br />
Credit(s) TM . Physicians should only claim credit commensurate with<br />
the extent of their participation.<br />

Cancellation Policy<br />
If notice of cancellation is received in our office 72 hours prior to the<br />
beginning of the conference, you will receive a full refund. With less<br />
than 72 hours of notice, 80 percent of the registration fee will be<br />
returned. No refund will be made after the conference begins.<br />
