Evidence that training has been provided to social work staff regarding Right 7(1),<br />

<strong>and</strong> that the legal guide on ―Caring for Patients with Diminished Competence‖ has<br />

been circulated to all social work staff, by 30 November 2010.<br />

An update on the arrangements with <strong>Taikura</strong> <strong>Trust</strong> for patients referred from<br />

<strong>Auckl<strong>and</strong></strong> City Hospital to that service, by 31 January 2011.<br />

A copy of the guidelines being developed in relation to discharge planning, <strong>and</strong><br />

the role of social work, for complex patients, by 31 January 2011.<br />

254. I recommend that <strong>Taikura</strong> <strong>Trust</strong> provide to HDC by 31 January 2011:<br />

Evidence of the training provided to staff in relation to the PPPR Act <strong>and</strong> the HDC<br />

Code of Rights, <strong>and</strong> a copy of the staff guide;<br />

An update on the arrangements with ADHB for clients referred to <strong>Taikura</strong> <strong>Trust</strong><br />

from <strong>Auckl<strong>and</strong></strong> City Hospital, including feedback on the testing of the hospital ―in<br />

reach‖ position, how it has worked, <strong>and</strong> whether it will be rolled out further.<br />

A review of the requirements for staff to keep clients, their representatives <strong>and</strong><br />

other key people informed in relation to time frames, <strong>and</strong> the outcomes of<br />

assessments/reassessments.<br />

A copy of the ―Documentation St<strong>and</strong>ard‖ currently being developed, <strong>and</strong><br />

confirmation of the proposed audit process/schedule.<br />

255. I recommend that <strong>Aranui</strong> <strong>Home</strong> <strong>and</strong> Hospital Ltd t/a Oak Park Dementia Unit provide<br />

to HDC by 30 November 2010:<br />

A written apology to Ms A’s family for breaching the Code <strong>and</strong> failing to protect<br />

<strong>and</strong> uphold Ms A’s rights.<br />

A review of its policies <strong>and</strong> procedures for residents subject to a compulsory<br />

court order.<br />

Evidence of further training provided to staff specifically in relation to informed<br />

consent <strong>and</strong> the requirements of the PPPR Act.<br />
