□ By Credit Card 信 用 卡 付 款 I hereby authorize Hong Kong Broadband Network Ltd. (“HKBN”) to debit all amounts due for services registered
by me, including but not limited to Internet access service / IDDservice / telephone service / television service / all telecommunications services /
all related value added services incidental to the aforesaid services, under the above HKBN account from the following credit card account. If
subscribers elect credit card payment as the designated payment method, HKBN will charge those subscribers for the related monthly service fee
via the relevant subscriber's registered credit card account. For whatever reasons, if any subscriber fails to settle the monthly service fees via his
designated registered credit card account. HKBN will charge the relevant subscriber HK$30 per month as administration fee. 本 人 現 授 權 予 香

作 為 指 定 的 付 款 方 式 , 香 港 寬 頻 將 以 客 戶 登 記 之 信 用 卡 收 取 有 關 服 務 計 劃 之 費 用 。 不 論 任 何 原 因 , 若 任 何 客 戶 未 能 以 已 登 記 的 信 用 卡

Declaration 聲 明
1. I agree and authorize HKBN to continue to keep the credit card (“the said Credit Card”) and/or the autopay account (“the said Autopay Account”)
information, if applicable, currently used to settle the service charges of my HKBN account for credit reference. In case the new credit card
information provided by me in this form fails to settle the service charges under the above HKBN account for whatever reasons, HKBN is authorized
to use the said Credit Card and/or the said Autopay Account information, if applicable, to settle the services charges under the above HKBN account
until further written notice. 2. I acknowledge that I have not provided any credit card copy to HKBN for verifying the credit card information
provided by me above herein and the credit card information is used by HKBN on such basis. I understand that it is a criminal offence to appropriate
the credit card information of third party and I shall be responsible for all liabilities in respect thereof. 3. I understand that this request will be
effective within 7 days upon HKBN’s receipt of this form. 4. I acknowledge and confirm that (i) I have read all the above terms and conditions of
this form and hereby agree to be bound by them once this form has been accepted by HKBN; and (ii) all information provided by me is true and
correct. 5. I agree to receive the HKBN monthly billing and any promotional messages of HKBN through the above selected channel. 6. I agree and
accept that from time to time Hong Kong Broadband Network Ltd. will send me service invoice and other service related correspondence via the
above selected and assigned address and those correspondences are treated as official notice. I acknowledge and agree that from time to time Hong
Kong Broadband Network Ltd. will send or email to me information of promotion and related services provided by Hong Kong Broadband Network
Ltd. or any third party.

至 另 行 書 面 通 知 。2. 本 人 確 認 「 香 港 寬 頻 」 乃 在 本 人 沒 有 提 供 信 用 卡 副 本 作 核 實 的 情 況 下 而 使 用 本 人 上 述 所 提 供 之 信 用 卡 資 料 。 本 人 明 白
如 本 人 盜 取 他 人 之 信 用 卡 資 料 使 用 , 乃 屬 刑 事 罪 行 , 本 人 須 負 上 一 切 之 法 律 責 任 。3. 本 人 明 白 此 更 改 服 務 將 於 「 香 港 寬 頻 」 收 到 本 表 格 後
七 天 內 生 效 。4. 本 人 承 諾 及 確 認 (i) 本 人 已 詳 閱 此 表 格 所 有 有 關 條 款 及 細 則 , 並 謹 此 同 意 當 「 香 港 寬 頻 」 接 納 此 表 格 時 , 願 意 受 其 約 束 及
(ii) 本 人 所 提 供 的 全 部 資 料 均 屬 正 確 無 誤 。5. 本 人 同 意 透 過 上 述 方 式 收 取 「 香 港 寬 頻 」 月 結 單 及 任 何 有 關 本 公 司 之 推 廣 優 惠 資 料 。6. 本 人 同
意 及 接 受 「 香 港 寬 頻 」 將 不 時 透 過 以 上 所 安 排 及 指 定 之 電 子 郵 箱 地 址 向 本 人 發 出 服 務 賬 單 及 其 他 服 務 有 關 之 通 信 而 該 等 函 件 將 視 作 正 式
通 知 。 本 人 知 悉 及 同 意 「 香 港 寬 頻 」 將 不 時 透 過 郵 遞 或 電 子 郵 件 向 本 人 發 出 有 關 「 香 港 寬 頻 」 或 其 他 公 司 所 提 供 之 服 務 宣 傳 品 。

In submitting this form, I declare the following:
1. All information given in this registration form is true and correct.
2. I agree that all personal data collected and held by HKBN may be used by it for: (a) processing of above application; (b) conducting identity
verification and/or credit checks at the time of above application; (c) processing of payment instructions or collection of amounts outstanding
from me in relation to above application; (d) marketing of HKBN services; (e) designing services for me; (f) conducting market research or
customers’ surveys; (g) provisioning of customer services; (h) handling complaints and account enquiries; (j) compliance with applicable laws,
rules and regulations; and (j) any other purposes directly related to the purpose for which the personal data were originally collected.
3. I agree that all personal data collected and held by HKBN will be kept confidential, but where disclosure is necessary for HKBN to comply with
statutory obligations or requirements, or for it to provide the services to me or to carry out the original purpose, or a directly related purpose, for
which the personal data were collected, those data may be provided to (whether within or outside Hong Kong): (a) the Telecommunications
Authority or other governmental or statutory authorities, institutions or organizations; (b) HKBN’s associated companies, contractors, agents or
other telecommunications service operators who are involved in the sale, administration or provision of HKBN services; and (c) banks, financial
institutions, credit card issuing companies or debt collection agencies.
4. I will send a written request to HKBN Personal Data Officer, P.O. Box No. 73910, Kowloon Central Post Office, if I wish to access or correct my
personal data held by HKBN or do not wish to receive marketing material or information from HKBN.

所 需 的 服 務 ;(f) 進 行 市 場 研 究 或 顧 客 意 見 調 查 ;(g) 為 提 供 客 戶 服 務 ;(h) 處 理 投 訴 及 賬 戶 查 詢 ;(i) 遵 守 適 用 的 法 例 、 規 則 或 規 例 ; 及
(j) 任 何 與 收 集 個 人 資 料 的 原 來 目 的 直 接 有 關 之 其 他 用 途 。
3. 同 意 「 香 港 寬 頻 」 收 集 及 持 有 關 於 本 人 的 個 人 資 料 均 會 被 保 密 處 理 ; 但 若 需 要 按 法 例 的 規 定 或 要 求 而 履 行 法 律 責 任 , 或 為 本 人 提 供 上
述 服 務 , 或 執 行 收 集 個 人 資 料 的 原 來 目 的 或 直 接 有 關 之 目 的 ,「 香 港 寬 頻 」 可 能 會 向 上 述 人 士 披 露 該 等 資 料 ( 不 論 在 香 港 境 內 或 境
外 ):(a) 電 訊 管 理 局 或 其 他 政 府 部 門 、 法 定 機 構 或 組 織 ;(b)「 香 港 寬 頻 」 之 聯 繫 公 司 、 參 與 該 服 務 銷 售 或 行 政 , 或 提 供 「 香 港 寬 頻 」
服 務 的 承 辦 商 、 代 理 或 其 他 電 訊 服 務 供 應 商 ; 及 (c) 銀 行 、 金 融 機 構 、 信 用 卡 發 行 公 司 或 追 收 欠 款 服 務 公 司 。
4. 如 欲 查 閱 或 更 正 個 人 資 料 , 或 不 欲 收 到 關 於 「 香 港 寬 頻 」 服 務 的 宣 傳 函 件 或 資 訊 , 本 人 會 以 書 面 方 式 通 知 「 香 港 寬 頻 」 私 隱 資 料 主
