Application
申 請 人 可 以 使 用 IA-P-R1 表 格 申 請 續 領 牌 照 , 以 便 繼 續 管 有 一 具 處 於 不 運 作 狀 態 的 輻 照 儀 器 。
You may use form IA-P-R1 to apply for the renewal of a licence that permits you to possess an irradiating apparatus in a
non-functional state.

申 請 費 用 為 $410。 當 輻 射 管 理 局 收 到 有 關 申 請 後 , 會 向 申 請 人 發 出 一 般 繳 款 單 , 申 請 人 請 依 照 一 般 繳 款 單
內 所 列 的 繳 款 辦 法 繳 付 所 需 費 用 。
假 如 申 請 最 終 被 否 決 , 已 繳 交 的 款 項 將 盡 快 全 數 退 還 。
The application fee is $4 10 . A f t er recei vi ng the ap pl ication, the Ra diation Boar d will issue a Ge ner al
Dem and Note to the ap plic ant. The ap plicant could m ake paym ent accordi ng t o the paym ent
instructions stated in the Gene ral Dem and Note.
Shoul d the appl ication be unsuccessful, the paid am ount will be ref und ed to t h e ap plicant as soo n as
possible
3. 按 情 況 需 要 連 同 申 請 書 一 併 遞 交 的 文 件
Documents to be submitted with the application (where applicable)
(i) A1 段 - 申 請 人 的 45x60( 毫 米 ) 近 照 兩 張 。
Section A1 - 2 recent photographs of size 45 x 60(mm) of the applicant.
(ii) A2 段 - 商 業 登 記 證 副 本 和 獲 授 權 人 士 身 分 證 / 證 明 文 件 的 副 本 ( 如 獲 授 權 人 士 有 所 更 改 )。
Section A2 - Copy of the Business Registration Certificate, and
Copy of Identity Card/Document of the authorized person
(only for the change of authorized person).
4. 表 格 中 所 列 的 所 有 有 關 牌 照 必 須 於 呈 交 本 申 請 書 時 仍 為 有 效 。 逾 期 管 有 輻 照 儀 器 乃 違 反 輻 射 條 例 第 7(1) 條 的 罪 行 , 一 經 定 罪 , 可 處
罰 款 $50,000 及 監 禁 2 年 。
Any current licence mentioned in this application must remain valid on the date of submission of this application. Possessing an
irradiating apparatus without a valid licence is an offence to section 7(1) of the Radiation Ordinance, and shall be liable to a fine of
$50,000 and to imprisonment for 2 years.
