This form is to be used to record the destruction of multiple vehicles by only a registered vehicle dismantler or scrap processor. For a more<br />
detailed explanation regarding who and when an entity should be submitting a <strong>MV</strong>-<strong>907M</strong>, please refer to page 2 “Guidelines for Proper<br />
Usage of the <strong>MV</strong>-<strong>907M</strong>”.<br />
INSTRUCTIONS<br />
Record the acquisition of the vehicle(s) in the book of registry.<br />
Complete all sections of the form.<br />
Circle the appropriate letter(s) to indicate the major component parts that were missing prior to acquisition (F=Frame, E=Engine,<br />
T=Transmission, N=Nose, B=Body, D=Driver Air Bag; P=Passenger Air Bag.<br />
Attach proof(s) of ownership and lien releases for each vehicle to the form.<br />
Mail the form by the last day of each month to Title Bureau, PO Box 2105, ESP, Albany, NY 12220-0105.<br />
Keep a copy of the form for your records.<br />

GUIDELINES FOR PROPER USAGE OF THE <strong>MV</strong>-<strong>907M</strong><br />
This form is to be used to record the destruction of multiple vehicles by a registered vehicle dismantler or scrap processor.<br />
Ownership documents for every vehicle listed must be attached. The ownership documents must show appropriate transfer to the<br />
registered dismantler or scrap processor filing the form. The <strong>MV</strong>-<strong>907M</strong> is not an ownership document and it should not be used in<br />
the place of a <strong>MV</strong>-907A Salvage Certificate (<strong>State</strong>ment of Acquisition). Once the vehicle is listed on the <strong>MV</strong>-<strong>907M</strong>, the vehicle may<br />
never be titled again, the vehicle and its component parts may not be transferred to any entity other than a scrap processor. A vehicle<br />
listed on a <strong>MV</strong>-<strong>907M</strong> is considered destroyed and may not be in the inventory of a Dismantler after the date noted on the <strong>MV</strong>-<strong>907M</strong>.<br />
If the vehicle will be transferred to any entity other than a scrap processor, the registered vehicle dismantler must complete Form <strong>MV</strong>-<br />
907A, Salvage Certificate. The <strong>MV</strong>-<strong>907M</strong> with appropriate attachments must be filed with the Department at the noted address no<br />
later than the last day of each month.<br />
This form may be used:<br />
by a registered vehicle dismantler or scrap processor.<br />
upon delivery by the dismantler to the scrap processor for destruction or upon destruction by the scrap processor.<br />
once a vehicle is completely “parted out” or otherwise destined for destruction by a dismantler. The dismantler should attach the<br />
appropriately issued <strong>MV</strong>-907A to the <strong>MV</strong>-<strong>907M</strong> as ownership.<br />
This form may not be used:<br />
by any entity other than a registered vehicle dismantler or scrap processor.<br />
if the vehicle will be rebuilt (must issue <strong>MV</strong>-907A).<br />
if the vehicle will be dismantled for parts (must issue <strong>MV</strong>-907A).<br />
if the vehicle will be transferred to any entity other than a scrap processor.<br />
if more than 15 days has elapsed since the date of acquisition of a junk and salvage vehicle by a dismantler. After 15 days, the<br />
dismantler must issue a <strong>MV</strong>-907A, Salvage Certificate, for the vehicle, regardless of whether or not the vehicle is ultimately<br />
destroyed.<br />
