Hotel check-in cannot be guaranteed until after<br />
4:00 p.m. This morning and afternoon, you will be<br />
able to use the hotel ballroom space for practice.<br />

The Knott’s <strong>Berry</strong> <strong>Farm</strong> Resort Hotel is the official<br />
host hotel of the U.S. Championship - Knott’s <strong>Berry</strong><br />
<strong>Farm</strong>. Additional room nights may be purchased at<br />
the “NCA/NDA rate” of $165 per room night (tax<br />
included). NCA/NDA will make every effort to<br />
ac<strong>com</strong>modate all resident packages. Should our<br />
room blocks fill up and additional resident package<br />
registrations are received, those customers will be<br />
contacted and instructed to secure lodging with<br />
the assistance of TravelFocus. Make note of any<br />
additional room nights on the event rooming list<br />
and/or the registration form. Your invoice will<br />
reflect these additional charges.<br />

STEP 2<br />
Within two weeks of receipt of your <strong>com</strong>pleted form(s) and deposit(s), you will receive an invoice and general<br />
information regarding the championship.<br />
STEP 3<br />
Final payment is due in the NCA/NDA office on or before each due date noted in the event information. Payments not<br />
received by these dates will result in the Team/Spectator being automatically removed from the event, which will result<br />
in a <strong>com</strong>plete forfeiture of all monies paid, according to cancellation deadlines for that specific event.<br />

Note: A $30.00 “returned check” fee will be charged for each insufficient funds check. Any payments<br />

CREDIT CARD CHARGES<br />
Visa, MasterCard, Discover and American Express may be used for payment of the package. ONLY the amount specified<br />
on the registration form will be charged to your credit card! Final payment will ONLY be charged to your credit card per<br />
written request.<br />
Charges will be made as follows:<br />
1. Initial deposit on or before the designated deadline for your event.<br />
2. Balance due on the designated final payment deadline.<br />
Note: If you want to add or change a credit card number, you must send the information in writing or<br />
fax to NCA/NDA. No credit cards will be taken over the phone. Individual credit card numbers will<br />

LATE REGISTRATIONS<br />
If space is available, late registrations will be accepted after the designated deadlines by sending in the total amount due<br />
with your registration. If you are registering late, the full payment must be submitted in the form of an official school<br />
check, cashier’s check, money order, or credit card. The standard cancellation policy will apply to all late registrations.<br />
CANCELLATIONS AND REFUNDS<br />
Notification of cancellations and request for refund must be made in writing, and emailed or faxed to NCA/NDA. No<br />
cancellations will be accepted by phone. Cancellations may be faxed to NCA/NDA at 972-840-4054. NCA/NDA is not<br />
responsible for faxes not received. All cancellations must be received during business hours: Monday-Friday 8:30 a.m. to<br />
5:00 p.m. CST. If cancellation is not received during these hours, it will be dated the next business day.<br />
ACCOMMODATIONS/ROOMMATE PREFERENCES (Resident Events Only)<br />
List the full name(s) of the individuals you would like to room together. Registrations for resident packages that do not<br />
have a <strong>com</strong>pleted rooming list will be considered in<strong>com</strong>plete, and will therefore not be processed until all <strong>com</strong>pleted<br />
information is received. Due to hotel requirements, the maximum number of people per room is four. Participants,<br />
coaches, directors and spectators may room together. In some cases, triple ac<strong>com</strong>modations may consist of a king bed<br />
and a rollaway/sleeper sofa. Double occupancy may consist of one king bed (There is no guarantee that groups of rooms<br />
submitted together will be next to each other in the hotel.) Additional room nights at the NCA/NDA rate are available<br />
before or after the actual event date. Please note any additional room nights on the squad/team rooming list and/or<br />
the spectator registration form. Your invoice will reflect these additional charges.<br />

TravelFocus is a team of dedicated travel consultants that will help you arrange your group’s travel. TravelFocus will<br />
negotiate group pricing on your preferred airline and provide fare <strong>com</strong>parisons for up to five airlines. For teams who<br />
prefer not to fly, TravelFocus can also assist with and negotiate charter bus rates. Questions regarding airline tickets OR<br />

THEME PARK<br />
The championship registration is separate from your team’s travel arrangements and cannot be paid for with one check.<br />
Separate payment arrangements must be made with TravelFocus. Airfare is determined by seat availability at time of<br />
booking. Late reservations may result in higher fares.<br />
GENERAL CONDITIONS<br />
TravelFocus in Dallas, Texas maintains no control over the personnel, equipment, or operation of any air, water or<br />
surface carrier, ship line, bus or limousine <strong>com</strong>pany, transportation <strong>com</strong>pany, hotel, restaurant or other person or entity<br />
furnishing services, products, or ac<strong>com</strong>modations as part of this trip. All of these suppliers are independent contractors.<br />
TravelFocus shall not be liable for any injury, damage, loss, expense, accident, delay inconveniences, or irregularity which<br />
