a lecture by Pulitzer Prize winner Charles Krauthammer, MD. Considered by the<br />

Financial Times to be the most influential commentator in America, Charles<br />

Krauthammer is well recognized for his bold, original writing on politics.<br />

Dr. Krauthammer is an active and respected contributor to today’s political<br />

thought through a syndicated column he has for the Washington Post and a monthly<br />

essay for TIME magazine. Dr. Krauthammer will provide us with his perspective on<br />

the efforts to redesign the health care system, and he will share with us his personal story.<br />

The Perspectives of Pain Plenary Session will take learners on a journey through the<br />

Half of this session is set aside for the Walter J. Zeiter Lecture. Walter J. Zeiter served<br />

as executive director of AAPM&R for 20 years. The lectureship is awarded annually in<br />

his honor to a physiatrist who shares Dr. Zeiter’s qualities of statesmanship, scholarship,<br />

executive leadership, and warm friendship. In this session, AAPM&R, the Foundation<br />

for PM&R and the American Board of PM&R also honor distinguished achievements in<br />

physiatry. The Academy’s award recipients, including the <strong>2012</strong> Zeiter lecturer, will be<br />

A Day-Long Focus on Managing Opioids and Limiting Risk<br />

As part of the pain and spine medicine track, several sessions will address managing long-acting<br />

and extended release opioids; improving patient care and safety while minimizing risk<br />

PM&R physicians take the lead per capita for opioid prescribing. This fact highlights the tremendous pressure on<br />

PM&R physicians to ensure safe and effective prescribing patterns. Further, it highlights the need for the specialty<br />

of PM&R to minimize risks to patients and practices. Hear from keynote speakers about how to effectively manage<br />

clinical and practice issues around pain management, specifically long-acting and extended-release opioids.<br />

Speakers include pain experts, law enforcement officials, and attorneys who manage risk.<br />

Physiatrists are increasingly involved in the management of individuals<br />

with acute and chronic pain including the use of controlled substances<br />

like opioids. The risks of prescribing these medications include<br />

patients taking the medications inappropriately, becoming addicted,<br />

or diverting or selling the prescribed medicines. Prudent physicians<br />

routinely prescribing controlled substances for pain patients must<br />

be concerned with the potential of being investigated by police, the<br />

medical board, the DEA and local prosecutors. This course will present<br />

strategies for developing safe practices and maintaining compliance<br />

with regulations.<br />

opioid prescribing in a physiatry practice. A detailed discussion on<br />

the pharmacology of opiate medications will include side effects,<br />

drug interactions, mechanism of action and tolerance. Concepts of<br />

dose titration, incomplete-cross tolerance, and opioid rotation will<br />

be reviewed.<br />

This course will discuss the clinical activity behind responsible opioid<br />

prescribing in a physiatry practice. The basic tenants of rationale opioid<br />

prescribing will be reviewed: effective patient evaluations, developing<br />

a sound treatment plan, utilizing appropriate screening tools, and risk<br />

management. The benefits vs. risks of prescribing ER/LA opioids will<br />

be reviewed.

This is a PM&R-specific, day and a half workshop designed to educate physiatrists, physicians’ office<br />

managers, and procedural coders about the latest coding and billing issues that have an effect on PM&R<br />

practices. The sessions will focus on correct coding of services and procedures to assist practices in<br />

securing appropriate reimbursement for physician and practice services from third-party payers and<br />

others. This will be achieved through the use of lectures, clinical vignettes, attendee participation,<br />

and question and answer sessions.<br />

• Utilize and appropriately apply recent changes to CPT codes in 2011- <strong>2012</strong><br />

that relate to physiatric practice<br />

Hands-On Diagnostic Musculoskeletal Ultrasound:<br />

In-Depth Introduction to Uses and Techniques<br />

New in <strong>2012</strong>, this course will examine the hip, foot, and ankle in addition to the shoulder! This introductory course<br />

is designed for the clinician who wishes to become more familiar with the function of diagnostic musculoskeletal<br />

ultrasound, its limitations, and its capabilities. Through didactic, interactive, and over six (6) hours of hands-on<br />

portions, clinicians will learn the basics of performing a diagnostic musculoskeletal ultrasound of various body<br />

parts in both upper and lower limbs. Participants will be exposed to both normal- and abnormal-appearing<br />

structures, the principles of interventional ultrasound, and ways to integrate this modality into practice.<br />

Additional topics include purchasing an ultrasound machine and coding and billing. Upon completion<br />

of this course, attendees will have a solid skill set to readily implement into their practices.<br />

This day and a half course geared toward a basic to intermediate level audience will highlight the practical<br />

assessment and comprehensive treatment of spasticity and related disorders. A faculty of expert clinicians<br />

will lead discussions and interactive sessions focused on teaching and updating skills for assessment and<br />

management of spasticity. This year’s course will include new formats to address patient assessment. It will<br />

involve more hands-on sessions and techniques for ultrasound-guided injection of botulinum toxins and<br />

neurolytic agents, such as anesthetics and phenol, and intrathecal baclofen management.<br />

Learning Objectives<br />

Upon completion of this course, participants will be able to:<br />

• Define the role of non-pharmacologic physical modalities and surgery in optimizing outcome<br />

for patients with the upper motor neuron syndrome<br />

• Open sessions on specific PM&R themes: Each open<br />

session is 90 minutes long and organized around a single<br />

theme. They may include several speakers, panel discussions,<br />

or group discussions. Some courses are in the form of<br />

clinical pearls or incorporate scientific paper presentations<br />

with several short clinical pearl presentations from the<br />

same topic area.<br />

• Best Research Presentations: These 60–minute<br />

educational sessions include the top reviewed abstracts<br />

in each topic area. The sessions are designed to facilitate<br />

discussion on current research and methodology<br />

of commonly themed posters.<br />

Get hands-on learning throughout the day<br />

Based on attendee feedback, more ticketed workshops have<br />

been added throughout the day. These small group learning<br />

environments allow participants to engage in procedures and<br />

skills pertinent to the physiatrist, such as physical examination,<br />

manual medicine, electromyography, and ultrasound.<br />

How to get a workshop ticket: For admittance to a workshop,<br />

you will be required to show a ticket. Workshop tickets<br />

are available for purchase on a first-come, first-served basis<br />

Meet Your CME<br />

Requirements<br />

Maintenance of Certification (MOC)<br />

The American Board of Physical Medicine and Rehabilitation<br />

(ABPMR), via its Maintenance of Certification (MOC)<br />

program, requires board certified physicians to complete a<br />

minimum of 300 AMA <strong>PR</strong>A Category 1 Credits by the end<br />

of their 10–year MOC cycle. The educational sessions offered<br />

at the AAPM&R <strong>2012</strong> <strong>Annual</strong> <strong>Assembly</strong> will help physicians<br />

meet this ABPMR MOC requirement. For more information<br />

Accreditation<br />

The American Academy of Physical Medicine and Rehabilitation<br />

is accredited by the Accreditation Council for Continuing<br />

Medical Education (ACCME) to provide continuing medical<br />

education for physicians.<br />

The Program Planning Committee (PPC) of the AAPM&R<br />

Medical Education Committee (MEC) developed the educational<br />

program this year in accordance with the ACCME Essentials<br />

for developing continuing medical education. Educational topics<br />

were selected on the basis of evaluation from previous<br />

educational offerings, advice from the AAPM&R Member<br />

Councils, and the expert opinion of PPC. In every session, the<br />

level of information, modes of presentation, and learning<br />

objectives were developed to meet the needs of physiatrists.<br />

Educational sessions that are appropriate for other physicians<br />

and health care professionals are designated accordingly.<br />

Learning Objectives<br />

The educational content for the AAPM&R <strong>2012</strong> <strong>Annual</strong><br />

<strong>Assembly</strong> is composed of courses and workshops. Each of<br />

these individual sessions has specific learning objectives that<br />

session directors, along with the help of their faculty in most<br />

cases, have created to communicate the desired outcomes<br />

for learners. Individual learning objectives for each session<br />

will be included in the Official Program, provided to all<br />

attendees onsite. In addition to providing learning objectives<br />

for each individual educational session, the AAPM&R PPC<br />

has also developed global learning objectives for the entire<br />

meeting. When planning and determining the educational<br />

content and structure of the <strong>2012</strong> <strong>Annual</strong> <strong>Assembly</strong>, PPC<br />

determined the following overall learning objectives<br />

for this year’s <strong>Annual</strong> <strong>Assembly</strong>.<br />

As a result of participating in the <strong>Annual</strong> <strong>Assembly</strong>,<br />

participants will be able to:<br />

1. Practice evidence-based, informed physiatric care<br />

2. Demonstrate the application of current techniques,<br />

procedures, and research in the PM&R field<br />

3. Apply knowledge to guide clinical decisions in<br />

practice and treatment of patients<br />

4. Become more involved in advocacy issues related<br />

to physical medicine and rehabilitation<br />

5. Utilize appropriate models and guidelines, including<br />

coding and billing; patient management and<br />

documentation; risk management; marketing;<br />

outcomes assessment to improve physician practice;<br />

and quality of patient care<br />

6. Discuss the latest advances in technology<br />

and equipment for patient care<br />

7. Incorporate effective strategies to enhance working<br />

in interdisciplinary teams<br />

8. Improve communication skills and strengthen<br />

professional relationships<br />

CME Credit Designation<br />

US Physicians: The American Academy of Physical Medicine<br />

and Rehabilitation designates this live activity for a maximum<br />

of 26 AMA <strong>PR</strong>A Category 1 Credits. Physicians should<br />

claim only the credit commensurate with the extent of their<br />

participation in the activity.<br />

Educational open sessions, workshops, and Best Research<br />

Presentations will be available for AMA <strong>PR</strong>A Category 1<br />

Credits as noted in the Official Program. One credit is<br />

TRACK: NEUROLOGICAL REHABILITATION<br />

This track highlights the latest information on spinal cord injury (SCI), traumatic brain injury (TBI),<br />

stroke, and other neurological conditions including associated co-morbidities, impairments,<br />

and disabilities. Courses focus on recent pharmacological advances; the evidence base<br />

for appropriate evaluation and management of patients with disorders of consciousness;<br />

the effects of aging in individuals with central nervous system disorders; management of<br />

agitation; metabolic and respiratory complications in SCI; and both ultrasound and stimulation<br />

techniques to enhance the effectiveness of neurolysis for spasticity. Attendees will learn new<br />

skills and management techniques for central nervous system disorders, based on scientific<br />

evidence and established clinical practice that can be incorporated into their specific practices.<br />

This track includes a blend of general medical rehabilitation topics such as amputee<br />

rehabilitation and osteoporosis, as well as geriatric rehabilitation. Focused sessions on<br />

pediatric rehabilitation are also included in this educational track, as well as<br />

electromyography with neuromuscular medicine. Instructors discuss disabilities<br />

and disease processes while addressing clinical evaluation, treatment programs,<br />

TRACK: PAIN AND SPINE MEDICINE<br />

This track addresses education pertaining to spine medicine and the broad field of pain<br />

medicine (including acute and chronic pain issues). Courses and workshops in this track<br />

focus on evidence-based clinical evaluation, interventional and non-interventional treatment,<br />

and rehabilitation. Attendees will learn how to incorporate evidence-based guidelines into<br />

their own practices, understand underlying relevant basic science and translational research,<br />

and learn about new and emerging developments in the areas of pain and spine medicine.<br />

With approximately 500 residents attending each year, the AAPM&R <strong>Annual</strong> <strong>Assembly</strong> is the largest annual<br />

gathering of PM&R residents. Balancing the clinical with the practical, the <strong>Annual</strong> <strong>Assembly</strong> is an excellent<br />

Update on the Next Accreditation System<br />

(ACGME) and Milestone Development Process<br />

Attend this session for an update on the upcoming ACGME<br />

accreditation changes, which will be phased in over the next<br />

two years with full implementation expected by 2014. The<br />

“next accreditation system” will include a totally new process<br />

of institutional and program review and a greater focus on key<br />

outcome measures. Part of this new process will include the<br />

use of milestone assessment for every resident that will<br />

document the achievement of specific skill sets in all aspects<br />

of the ACGME competencies. A PM&R working group has<br />

spent the last year developing the initial drafts of these<br />

milestones. A description of this process as well as an<br />

introduction to several of these draft milestones will be<br />

presented. This session will be interactive and allow ample time<br />

for feedback regarding the draft milestones, and the process<br />

for eventual implementation.<br />

SynchroMed® II Drug Infusion System Brief Summary: Product technical manuals and the appropriate drug labeling must be reviewed prior to use for detailed disclosure. Indications: US: Chronic<br />

intraspinal (epidural and intrathecal) infusion of preservative-free morphine sulfate sterile solution in the treatment of chronic intractable pain, chronic intrathecal infusion of preservative-free ziconotide<br />

sterile solution for the management of severe chronic pain, and chronic intrathecal infusion of Lioresal® Intrathecal (baclofen injection) for the management of severe spasticity; chronic intravascular<br />

infusion of floxuridine (FUDR) or methotrexate for the treatment of primary or metastatic cancer. Outside of US: Chronic infusion of drugs or fluids tested as compatible and listed in the product labeling.<br />

Contraindications: Infection; implant depth greater than 2.5 cm below skin; insufficient body size; spinal anomalies; drugs with preservatives, drug contraindications, drug formulations with pH ≤3, use<br />

of catheter access port (CAP) kit for refills or of refill kit for catheter access, blood sampling through CAP in vascular applications, use of Personal Therapy Manager to administer opioid to opioid-naïve<br />

patients or to administer ziconotide. Warnings: Non-indicated formulations may contain neurotoxic preservatives, antimicrobials, or antioxidants, or may be incompatible with and damage<br />

the system. Failure to comply with all product instructions, including use of drugs or fluids not indicated for use with system, or of questionable sterility or quality, or use of non-Medtronic<br />

components or inappropriate kits, can result in improper use, technical errors, increased risks to patient, tissue damage, damage to the system requiring revision or replacement, and/or<br />

change in therapy, and may result in additional surgical procedures, a return of underlying symptoms, and/or a clinically significant or fatal drug under- or overdose. Refer to appropriate drug<br />

labeling for indications, contraindications, warnings, precautions, dosage and administration information, screening procedures and underdose and overdose symptoms and methods of management.<br />

Physicians must be familiar with the drug stability information in the product technical manuals and must understand the dose relationship to drug concentration and pump flow rate before prescribing<br />

pump infusion. Implantation and ongoing system management must be performed by individuals trained in the operation and handling of the infusion system. An inflammatory mass that can result in<br />

serious neurological impairment, including paralysis, may occur at the tip of the implanted catheter. Clinicians should monitor patients on intraspinal therapy carefully for any new neurological signs or<br />

symptoms, change in underlying symptoms, or need for rapid dose escalation. Inform patients of the signs and symptoms of drug under- or overdose, appropriate drug warnings and precautions regarding<br />

drug interactions, potential side effects, and signs and symptoms that require medical attention, including prodromal signs and symptoms of inflammatory mass. Failure to recognize signs and symptoms<br />

and seek appropriate medical intervention can result in serious injury or death. Instruct patients to notify their healthcare professionals of the implanted pump before medical tests/procedures, to return<br />

for refills at prescribed times, to carry their Medtronic device identification card, to avoid manipulating the pump through the skin, to consult with their clinician if the pump alarms and before traveling or<br />

engaging in activities that can stress the infusion system or involve pressure or temperature changes. Strong sources of electromagnetic interference (EMI), such as short wave (RF) diathermy and MRI, can<br />

negatively interact with the pump and cause heating of the implanted pump, system damage, or changes in pump operation or flow rate, that can result in patient injury from tissue heating, additional<br />

surgical procedures, a return of underlying symptoms, and/or a clinically significant or fatal drug underdose or overdose. Avoid using shortwave (RF) diathermy within 30 cm of the pump or catheter. Effects<br />

of other types of diathermy (microwave, ultrasonic, etc.) on the pump are unknown. Drug infusion is suspended during MRI; for patients who cannot safely tolerate suspension, use alternative drug delivery<br />

method during MRI. Patients receiving intrathecal baclofen therapy are at higher risk for adverse events, as baclofen withdrawal can lead to a life threatening condition if not treated promptly and effectively.<br />

Confirm pump status before and after MRI. Reference product labeling for information on sources of EMI, effects on patient and system, and steps to reduce risks from EMI. Precautions: Monitor patients<br />

after device or catheter replacement for signs of underdose/overdose. Infuse preservative-free (intraspinal) saline or, for vascular applications, infuse heparinized solutions therapy at minimum flow rate if<br />

therapy is discontinued for an extended period of time to avoid system damage. EMI may interfere with programmer telemetry during pump programming sessions. EMI from the SynchroMed programmer<br />

may interfere with other active implanted devices (e.g., pacemaker, defibrillator, neurostimulator). Adverse Events: Include, but are not limited to, spinal/vascular procedure risks; infection; bleeding; tissue<br />

damage, damage to the system or loss of, or change in, therapy that may result in additional surgical procedures, a return of underlying symptoms, and/or a clinically significant or fatal drug underdose<br />

or overdose, due to end of device service life, failure of the catheter, pump or other system component, pump inversion, technical/programming errors, or improper use, including use of non-indicated<br />

formulations and/or not using drugs or system in accordance with labeling; pocket seroma, hematoma, erosion, infection; post-lumbar puncture (spinal headache); CSF leak and rare central nervous system<br />

pressure-related problems; hygroma; radiculitis; arachnoiditis; spinal cord bleeding/damage; meningitis; neurological impairment (including paralysis) due to inflammatory mass; potential serious adverse<br />

effects from catheter fragments in intrathecal space, including potential to compromise antibiotic effectiveness for CSF infection; anesthesia complications; body rejection phenomena; local and systemic<br />

AAPM&R conferences are ADA compliant. We strive to take<br />

the appropriate steps required to ensure that no individual<br />

Policy<br />

All children over the age of 5 who wish to attend Academy<br />

functions must be registered for the <strong>Annual</strong> <strong>Assembly</strong>.<br />

Children 12 and over are invited into the Exhibit Hall Friday<br />

and Saturday and must be accompanied by a registered<br />

attendee. Under no circumstances are children allowed<br />

on the Exhibit Hall floor during the hours of set up and tear<br />

down. For the safety of all attendees, infants and children<br />

are prohibited from attending any educational session.<br />

Registration for children over age 5 is $20 per child.<br />

Lunch Vouchers<br />

AAPM&R offers a networking program to help reduce<br />

luncheon breaks on Friday, November 16, and/or Saturday,<br />

This interactive hands on workshop will educate the<br />

experienced clinician on specific methods for treatment<br />

of osteoarthritis with regenerative injection therapies.<br />

The optimal system of analysis and treatment planning<br />

will include physical exam and ultrasound evaluation and<br />

guidance for injection. A comprehensive regional approach<br />

to treatment of intra-particular and per-particular pain<br />

generators will be emphasized. Pre and post treatment<br />

protocols will be reviewed.<br />
